The Director of Public Health is professionally accountable to the Chief Executive of South Gloucestershire Council and to the Secretary of State for Health (through the Chief Medical Officer). Accountable to The DPH is a key member of the Council’s leadership team with direct access to members and professional accountability to the Chief Executive. Day to day reporting and housekeeping arrangements are managed through The Director of Children, Adults and Health and has been agreed with the DPH

NHS Consultant (dependent on experience and qualifications) or NHS Agenda for Change Very Senior Managers Scale (appropriate point). The salary range for a medically Grade qualified Director of Public Health in accordance with Pay Circular (M&D 2/2014) will be £75,249 to £101,451 (plus the appropriate Director of Public Health supplement). The Very Senior Manager salary for this role is £82,296. 1.0 wte Substantive Post (due to the seniority of the role the role holder will be expected to work such hours as necessary for the full performance of the role.

About South Gloucestershire South Gloucestershire has a rich natural and cultural heritage. It is a mixed urban and rural authority of great diversity located in the South West of England which covers 53,665 hectares. It is an area of diversity and contrast with a variety of communities, characterised by the special relationship between town and country. Residents are fortunate to have a high quality local environment which they greatly value. Attractive and historic landscapes link unspoilt market towns, villages and established rural and urban communities with major new residential areas, industrial and commercial developments. The area stretches from the Severn Estuary in the west to the Cotswolds Area of Outstanding Natural Beauty (AONB) in the east. Its southern boundary borders the core city of Bristol. It abuts the River Avon and extends almost to Bath. The area is well served by the M4 and M5 motorways and has good rail links. Both the first and second crossing of the River Severn lie within South Gloucestershire. South Gloucestershire has seen substantial levels of development with 62,000 jobs and 21,000 homes created since 1991. Over the longer term, the Core Strategy makes provision for up to 26,400 new homes in the period up to 2026 and beyond. The Area has a diverse economy ranging from rural and home grown small and medium enterprises to world leading companies in key high growth sectors vital to the national economic recovery. These include aerospace, advanced engineering, defence, micro electronics and silicon design. The new Science Park, a world class environment for businesses in science and advanced technology is based in South Gloucestershire and the Area is also a centre of excellence for green and environmental technologies and IT solutions. The district has a population of around 264,800 of which around 6% are estimated to be of Black and Minority Ethnic origin. This is projected to reach 308,100 by 2026, representing a 20% increase of the present population. Population projections also show a significantly higher proportion of older residents and a growth in the number of young children. 60% of the population live in the built up areas immediately adjoining Bristol, namely Filton, Patchway, Bradley Stoke, Kingswood, Downend, Staple Hill and Hanham. Around 19% live in the towns of Yate, Chipping Sodbury and Thornbury and the remaining 20% live in the more rural areas of South Gloucestershire. This is an interesting dynamic given that 80% of the geographic area is rural and only 20% is urban. South Gloucestershire is generally a healthy place to live with life expectancy higher than the national average. Over the last 10 years, all cause mortality rates have fallen. Early death rates from cancer and from heart disease and stroke have fallen and are better than the England average. On the whole, South Gloucestershire is a relatively affluent area and deprivation is low compared to national levels. However, pockets of deprivation do exist at a sub ward level, mainly in our urban areas.

Our Vision for Public Health South Gloucestershire Council is widely regarded as an excellent authority, having a strong track record of achievement and innovative working over the years. We have good member/officer relationships and effective consensus working across political groups. Recently, the Council was one of the first authorities in the country to use powers under the Localism Act to revert to a committee system of governance. This has resulted in the establishment of a number of new committees with decision making responsibilities. One of these new committees is the Public Health and Health Scrutiny Committee. As part of the national changes within the NHS, we have also established a Health and Wellbeing Board. We have adopted a positive approach to the national policy agenda and the transfer of public health responsibilities to the local authority. The Public Health team relocated to the council offices in March 2011 which helped improve understanding and joint working ahead of the formal transfer of staff in April 2013. We have also undertaken a range of work with members and officers to raise awareness and discuss opportunities for the future. We are absolutely clear however, that we want to use the opportunity to establish a stronger, more effective public health service across the local area. We want to make public health part of our core business and ensure we maximise opportunities to develop holistic approaches to health and wellbeing, embracing the wide range of services for which we are responsible. The Director of Public Health will therefore have a key role in raising the profile of public health and driving forward the health and wellbeing agenda. In January 2013 the Department for Children, Adults and Health (CAH) was created. This new department brought together the department for Children and Young People and the Community Care and Housing departments. In April 2013 public health was integrated into CAH as part of a wider health and wellbeing division headed up by the Director of Public Health. The Director of Public Health reports on a day to day basis to the Director of Children, Adults and Health but with professional accountability to the Chief Executive and with direct access to members. The creation of the new Public Health and Health Scrutiny provides an excellent opportunity for the DPH to develop strong relationships with members and shape the future of public health across the area. Similarly, as a member of the Health and Wellbeing Board, the DPH will have a significant influence with health service partners and the commissioning of cost effective health and social care services across South Gloucestershire. This is a time of major change for the Council. Undoubtedly there are challenges, but the new Director of Public Health will be in a unique and privileged position of working in a new environment with a significant opportunity to lead and influence the Council’s vision for health and wellbeing. Job Summary

The Director of Public Health is a registered public health specialist and is the Council’s designated lead officer for health. They will be the professional advisor to the Council and the Health and Wellbeing Board on all public health matters. They will be responsible for developing a clear, strategic approach to public health that will deliver maximum improvements in the health and wellbeing of the people of South Gloucestershire. They will be expected to make the most effective use of the health and well being division budget to deliver this goal. They will also be expected to influence other expenditure and strengthen capacity across the whole public sector to maximise public health benefit. The DPH will have responsibility for driving forward the health improvement agenda across the area with a focus on developing healthy, sustainable and cohesive communities, developing healthy lifestyles for individuals and communities and tackling specific issues based on local needs assessment. They will have a clear focus on reducing health inequalities and tackling the wider determinants of poor health. This will involve embedding public health thinking and practice across the authority as a whole, working collaboratively with other council services and partner agencies to promote effective joint working and ensuring delivery of public health outcomes and the achievement of key performance targets in line with the priorities set out in both the Sustainable Community Strategy and the Council Strategy. They will also have a key role in protecting the health of the local population, dealing with incidents (including infectious disease threats such as food and water borne disease), providing public health protection advice (including advising on environmental health threats such as pollution, noise and contaminated land) and working with local and national partners (including the Local Resilience Forum and Public Health England) to develop comprehensive multi-agency plans for emergencies and the anticipated threats to public health such as pandemic influenza. As the lead officer for health, the DPH will be expected to work closely with the Public Health and Health Scrutiny Committee and the Health and Wellbeing Board to develop the health and wellbeing agenda. The development of the Joint Strategic Needs Assessment and the Health and Wellbeing Strategy will be key elements of this. They will also produce an independent annual report on the health of the population including providing progress on improving health and reducing inequalities and making recommendations as appropriate. They will take a lead role in driving forward population health care including immunisation and screening programmes. They will also have a key role in the integration of health and social care services including providing specialist public heath advice to commissioners on priorities for health and social care spending and the appropriate configuration of effective and equitable health care services. This will involve leading the delivery of the ‘core offer’ to the local clinical commissioning group.

A visionary leader The DPH is a senior management position within the Council and will be expected to take a lead role across the wider authority as well as within public health. As such, they will require a range of skills and abilities, including the need to:  be a transformational and visionary leader;  demonstrate high levels of political awareness, be able to work with different organisational cultures and to plan and implement programmes for short and long term health gain;  have exceptional people management skills both in relation to developing the public health community and in helping to develop the public health leadership of organisations and the wider workforce;  be highly visible to ensure in depth knowledge of communities and better working between the public and local organisations;  be a skilled and trusted communicator at all times  fully understand and be committed to addressing the relationships and cultures of organisations that impact on the wider determinants of health;  bring a high level of intellectual rigour and personal credibility to the collaborative and commissioning agendas;  be able to coordinate high quality advocacy across all three domains of public health – health improvement, health protection and health service quality;  be trained and experienced in all areas of public health practice and with a proven record of previous accomplishments in improving the health of communities.

Management arrangements

The Director of Public Health is professionally accountable to the Chief Executive of South Gloucestershire Council and to the Secretary of State for Health (through the Chief Medical Officer). The DPH is a key member of the Council’s leadership team with direct access to members. Day to day reporting and housekeeping arrangements are managed through The Director of Children, Adults and Health.

Professional appraisal will be required. An initial job plan will be agreed with the successful candidate prior to that individual taking up the post. This job plan will be reviewed as part of the annual job planning process. The Director of Public Health will: 1. Manage 126 staff, of which there are 60 mentors, family activators and community support workers on zero hour’s contracts. The DPH has four direct reports (for whom s/he is responsible for line management duties, including appraisals, disciplinary and grievance responsibilities). The DPH will also participate in recruitment to key posts in the Health and Wellbeing division.

Manage budgets with a total value of £10 million comprising Physical Activity, Nutrition & Obesity, Breakthrough & Inclusion, Public Health Services, Sexual Health, NHS Health Checks & Tobacco Control, Strategy, Advice & Public Protection, Leadership & Business Support, Drug and Alcohol Services, Health In Schools, YPDAS, Health Inequalities and Mental Health & Emotional Wellbeing

Contribute actively to the training programme for Foundation Year Doctors/Specialty Registrars in Public Health as appropriate, and to the training of practitioners and primary care professionals within the locality (if the postholder designs and delivers core training, specify as applicable and give details of postholder’s involvement e.g. lead trainer, trainer on a module, develops training for others, etc)

Pursue a programme of CPD, in accordance with Faculty of Public Health requirements, or other recognised body, and undertake revalidation, audit or other measures required to remain on the GMC/GDC Specialist Register or the UK Public Health (Specialist) Register or other specialist register as appropriate.

The Role The Director of Public Health is a registered public health specialist (i.e. included in the GMC Specialist Register/GDC Specialist List or UK Public Health Register (UKPHR)). S/he is a visionary and transformational leader with a full understanding of relationships and culture of organisations that impact on the wider determinants of health as well as health services. S/he leads a team within the local authority responsible for the development of a strategic needs assessment for the local population and for the delivery of: 1. Health Improvement  developing healthy, sustainable and cohesive communities through Health and Wellbeing Board  developing healthy lifestyles for individuals and communities  tackling specific issues based on local needs assessment such as childhood obesity, smoking,  developing a strategy for reducing health inequalities 2. Health Protection  dealing with infectious disease threats including food and water borne disease supported by local Public Health England  preparing for emergencies including pandemic influenza  advising on environmental threats including pollution, noise and contaminated land 3. Health services public health  population health care such as immunisation and screening programmes  supporting the commissioning of appropriate, effective, and equitable health care from the NHS locally  leading the integration of health and social care services

The Director of Public Health will: 1. Produce an independent annual report on the health of the population, progress on improving health and reducing inequalities and making recommendations 2. Be principal adviser to Health and Wellbeing Board in developing a Health and Wellbeing Strategy based on the assessed needs of the population and proven interventions to improve health 3. Provide specialist public heath advice to commissioners on priorities for health and social care spending and the appropriate configuration of services within and between local authorities. 4. Support Local Resilience Forum in developing comprehensive multi agency plans for the anticipated threats to public health.

Main responsibilities Surveillance and assessment of the population’s health and well-being . To ensure the proper design, development and utilisation of major information and intelligence systems to underpin public health improvement and action for the population across disciplines and organisations. . To receive, interpret, provide and advise on highly complex epidemiological and statistical information about the health of populations to the NHS, local authority and voluntary organisations. . To ensure the use of the best available evidence base to support the assessment of health needs, health inequalities, health impact assessment and the identification of areas for action within the local population. . To produce an annual report on the health of the local population.

Assessing the evidence of effectiveness of health and healthcare interventions, programmes and services. . To provide expert public health advice and leadership to support and inform an evidence-based approach within ethical frameworks for commissioning and develop high quality equitable services, across primary, secondary and social care, and across sectors including voluntary organisations, etc, in potentially contentious and hostile environments where barriers to acceptance may exist. . To be responsible for advising on service development, evaluation and quality assurance governance in specific areas and for preparing and adjusting action plans in line with changing needs and changing geographical boundaries. . To provide expert advice to support evidence based commissioning, prioritisation of services for the population (and in some circumstances for the individual) in order to maximise opportunities for health.

Policy and strategy development and implementation . To lead on behalf of South Gloucestershire Council on the communication, dissemination, implementation and delivery of national, regional and local policies and health strategies, developing inter-agency and interdisciplinary strategic plans and programmes, with delegated board or organisational authority to deliver key public health targets.

. To act in expert advisory capacity on public health knowledge, standards and practice, across the spectrum of public health at board or equivalent level. . To be responsible for the development and implementation of multi-agency long-term public health programmes as required, based on identification of areas of potential health improvement, the diversity of local needs and the reduction of inequalities. . To ensure proper linkages between the health agenda and strategies related to the wider determinants including for example, community safety, the environment and sustainability.

Leadership and collaborative working for health . To take the lead role on behalf of South Gloucestershire Council in developing inter- agency and interdisciplinary short and long-term strategic plans for securing health improvement both in the general population and in vulnerable groups at high risk of poor health and reduced life expectancy, in partnership with a range of agencies such as those in the statutory, non- statutory, voluntary and private sectors and by taking lead responsibility with a defined local authority. This requires the ability to work cross-directorate and across other agencies and voluntary organisations. . To work with primary care professionals and community staff to raise awareness of their public health role. . To lead on the integration of health, social services and voluntary organisations to promote effective joint working to ensure delivery of the wider government targets. . Influence external agencies in their public health policy decisions by working with complex professional, managerial and population groups and other organisations in the statutory, non-statutory and private sectors.

Health Improvement . To be responsible for designated areas of health improvement programmes, public health surveillance or population screening or geographical areas. . To take a leadership role in specified areas with local communities and vulnerable and hard to reach groups, helping them to take action to tackle longstanding and widening health inequality issues, using community development approaches as appropriate. . To provide expert knowledge to ensure effective community involvement with regard to all the work of the organisation including commissioning and prioritising high cost services to ensure that policies and strategies are interpreted, developed and implemented at all levels.

Health Protection  To take responsibility for safeguarding the health of the population in relation to communicable disease, infection control and environmental health, including delivery of immunisation targets.  To ensure that effective local arrangements exist for covering the on call rota for the effective control of communicable disease, environmental hazards to health and emergency planning, as detailed in local health protection agreements.  To communicate effectively and diplomatically with a wide audience including the media and the public to change practice in highly challenging circumstances such as communicable disease outbreaks, chemical incidents, immunisation and screening.

Service Improvement . To provide expert advice to support evidence based commissioning, prioritisation of services for the population (and in some circumstances provide highly specialised advice on preferred treatment options or protocols based on the evidence for individual patients) in order to maximise opportunities for health. . To be responsible for implementation of NICE/National Service Frameworks or equivalent national standards / guidance and frameworks. . To lead the development of clinical networks, clinical governance and / or audit. . To review evidence and providing highly specialised advice on preferred treatment options or protocols based on the evidence for individual patients.

Public Health Intelligence . To analyse and evaluate quantitative and qualitative data and research evidence from a range of sources to make recommendations and inform decision making which has long term impacts. . To compare, analyse and interpret highly complex options for running projects identified as key public health priorities, and communicate this information across organisations and the local community. . To be responsible for the identification and implementation of appropriate health outcome measures, care pathways / protocols and guidelines for service delivery across patient pathways for the local population . To work with other organisations to strengthen local, regional and national public health intelligence and information capacity.

Academic Public Health/Research and Development . To undertake and commission literature reviews, evaluate research surveys, adults and other research as required informing equitable service delivery and reducing health inequalities. . To develop links with academic centres as appropriate and/or the public health observatories to ensure the work of the organisation is based on a sound research and evidence base. . To develop public health capacity through contribution to education and training and development within the directorate, and within the wider NHS and non NHS workforce.

Proposed organisational arrangements

The current Public Health Organisational Structure is attached. Public health has been integrated into the new Children, Adults and Health department as part of a wider health and wellbeing division. The Director of Public Health will head up this division, reporting on a day to day basis to the Director of Children, Adults and Health. They will be a key member of the Council’s leadership team with direct access to members and will be professionally accountable to the Chief Executive. General Conditions

Terms and conditions of service The post is subject to general NHS Terms and Conditions of Service and relevant organisational employment policies.

Those candidates who meet the requirements for appointment as a Consultant in Public Health Medicine will be eligible for the NHS Consultant Contract salary scale.

Those candidates appointed as Consultants in Public Health will be eligible for NHS Agenda for Change Very Senior Managers Scale (appropriate point).

On call arrangements The postholder may be expected to be on call for health protection and public health and to participate in the communicable disease and environmental hazards control and emergency planning arrangements for South Gloucestershire. Indemnity As the post holder will only be indemnified for duties undertaken on behalf of South Gloucestershire Council the post holder is strongly advised to ensure that he/she has appropriate professional defence organisation cover for duties outside the scope of South Gloucestershire Council and for private activity within South Gloucestershire Council. For on call duties provided to other organisations as part of cross cover out of hours arrangements South Gloucestershire Council has confirmed that those organisations will provide indemnity for the post holder. Flexibility The post holder may, with their agreement - which should not reasonably be withheld - be required to undertake other duties which fall within the grading of the post to meet the needs of this new and developing service. The employing organisation is currently working in a climate of great change. It is therefore expected that all staff will develop flexible working practices both within any relevant local public health networks and at other organisational levels as appropriate, to be able to meet the challenges and opportunities of working in public health within the new and existing structures. Shortlisting notes The Faculty of Public Health advises that in order to be shortlisted for a consultant post applicants who are not yet on the GMC Specialist Register/GDC Specialist List in dental public health/UK Public Health Register(UKPHR) must provide verifiable signed documentary evidence that an application for inclusion on one of these specialist registers is in progress as follows: 1. Applicants in training grades Public health Specialist Registrars and Specialist Trainees in a recognised UK public health training scheme must provide evidence to confirm that they are within SIX months of award of their certificate of completion of training (CCT) and inclusion in the GMC Specialist Register/GDC Specialist List in dental public health/UKPHR at the date of interview (i.e. the expected date of award of their CCT must fall no more than six months after the date of interview). Please note that from January 2005 in England, May 2005 in Scotland and November 2005 in Northern Ireland and Wales, this period has been extended from the three months required previously. The documentary evidence should be: Either a RITA Form G (Final Record of Satisfactory Progress) or a letter from the postgraduate dean (or Faculty Adviser) specifying the expected date for completion of training (which must be not more than six months after the date of interview). 2. Applicants in non training grades 2.1 Doctors (i.e. medical practitioners) Doctors outside recognised UK public health training schemes fall into a number of categories:  those who have trained outside the UK, who may have specialist training and qualifications which they are seeking to have recognised by the Postgraduate Medical Education & Training Board (PMETB) in order to gain registration with the GMC: these doctors may be shortlisted according to the following 2005 guidance from the Department of Health and Scottish Executive which indicates that There will be some instances (for example when considering applicants trained outside the UK) where an AAC may choose to interview a candidate prior to [GMC] Specialist Register entry. In these circumstances, it will wish to be satisfied that subsequent Specialist Register entry is likely . Employers should ask the applicant to provide documentary evidence that he/she has submitted an application to the PMETB which is eligible for consideration at the time of application (for shortlisting).  those who have not completed specialist training in the UK who are seeking entry to the GMC Specialist Register through Article 14 of the European Specialist Medical Qualifications Order (ESMQO), which allows the PMETB to consider not only training but also relevant experience: these doctors may have trained in or outside of the UK. Again, employers should ask the applicant to provide documentary evidence that he/she has submitted an application to the PMETB which is eligible for consideration at the time of application (for shortlisting). 2.2 Applicants from a background other than medicine  Other than trainees (see 1 above), applicants from a background other than medicine would normally be expected to have gained full registration with the UKPHR. However, exceptionally, individuals who can demonstrate that they have submitted a portfolio application to the UKPHR may be considered for shortlisting. Suitable evidence will be a letter from the UKPHR acknowledging receipt of the portfolio application.  Other than trainees (see 1 above), applicants from a background in public health dentistry must be included in the GDC Specialist List in dental public health. However, those who can demonstrate that they have submitted an application for inclusion on the GDC specialist list in public health dentistry may be considered for shortlisting. Employers should ask the applicant to provide documentary evidence that he/she has submitted an application to the GDC which is eligible for consideration at the time of application (for shortlisting). Employers are advised that individuals should not take up consultant in public health medicine or consultant in public health posts (including DPH posts) until such point as they have gained entry to the GMC Specialist Register/GDC Specialist List in dental public health/UK Voluntary Register for Public Health Specialists. Although applicants will be able to provide documentary evidence that an application is in progress, no guarantee can be made as to the outcome of an application to the GMC/GDC/UKPHR specialist registers. The above guidance applies to applications for both general and defined specialist registration with the UKPHR. Individuals with defined specialist registration are eligible for consideration for shortlisting for, and appointment to, consultant posts including those at DPH level. In all appointments, employers will wish to ensure that an applicant’s areas of competence meet those required in the person specification.
