In order to suction the nose and mouth of students, employees must complete training and demonstrate the ability to perform the following tasks: TRAINED REVIEWED 1 Review doctor’s orders, IHP, PISD guidelines, and parent consent. 2 Use Universal Precautions. Wash hands before and after suctioning. Gather equipment and supplies (suction machine and tubing, suction catheter, or tonsil 3 tip suction catheter [Yankauer], water or normal saline, container for water, and gloves). 4 Position student, upright or Semi-Fowler’s, and explain procedure. Turn on suction machine and check for suction by placing finger at end of connecting 5 tubing. 6 Encourage student to cough and expel secretions. 7 Open suction catheter being careful not to touch the inside of the package. 8 Put on gloves. Attach suction catheter to connecting tubing. Leave the other end of the catheter in its 9 covering. 10 Remove covering from end of suction catheter. 11 Hold suction catheter 2-3 inches from its tip with dominant hand and insert tip in water. Cover vent hole with thumb of non-dominant hand to suction a small amount of water 12 through catheter. 13 Monitor student’s respiratory status throughout procedure With thumb off vent hole, insert catheter gently into the nose to the prescribed depth 14 specified in the IHP. Always suction the nose first. 15 If the nose secretions are too thick, a few drops of saline can be put in each nostril. Cover vent hole with non-dominant thumb and gently rotate catheter while suctioning 16 and withdrawing. 16 Repeat if needed. Between passes, suction water to rinse out catheter. 17 With thumb off vent hole, insert catheter gently into the mouth. Cover vent hole with non-dominant thumb. Gently rotate catheter between thumb and 18 index finger while suctioning and withdrawing to minimize damage to the oral mucosa. If oral suctioning only is being done with a Yankauer suction catheter, insert Yankauer into 19 mouth along gum line and move around mouth until secretions are cleared. 20 If gurgling noises persist, repeat mouth suctioning procedure with same catheter. 21 Rinse catheter and connecting tubing with water. Disconnect catheter from suction tubing. Dispose of catheter and gloves properly. (Yankauer suction catheter if used for oral suctioning only, may be rinsed in between 22 suctioning. When suction complete for the day, wash in warm water with dish soap, rinse, soak in 1:1 vinegar water solution for 20 minutes. Rinse and allow to air dry and store in clean container for future use). 23 Turn off suction. Wash hands. 24 Document color, consistency, quantity of secretions, and respiratory status. 25 Make sure equipment and supplies are restocked and ready for next use. I have received instructions on the procedures to be followed in the suctioning of the nose and mouth of students at school following Plano ISD guidelines and understand my responsibilities.
