Each new staff member and their immediate supervisor are to complete the Health & Safety Orientation checklist within one (1) month of hire to ensure all topics have been reviewed and are clearly understood by the employee.

1. Know where to access the Occupational Health & Safety Program Manual?
2. Understand the need to review safe work procedures specific to job? (ie., when working with chemicals, machinery, handling sharps, etc.)
3. Have received instruction and know how to obtain first aid services?
4. Have received instruction and understand what to do in the event of a hazardous spill?
5. Know how to report hazardous conditions and practices?
6. Know procedures to follow to report a work-related injury and/or exposure?
7. Know what to do in the event of a fire? (Escape plan, location of fire extinguisher, etc.)
8. Have completed the Workplace Hazardous Material Information System (WHMIS) education and training, if applicable.
9. Know how to access MSDS to obtain information, if applicable.
10. Have received instruction on management of blood borne pathogen exposures (needle stick and splashes)? (If applicable)
11. Know proper use and location of personal protective equipment? (ie., eye protection, respiratory protection, gloves, hearing protective devices, etc.) (If applicable)
12. Have received instruction and clearly understand “point of use” sharps disposal? (If applicable)
13. Have received specialized training and clearly understand all lock-out procedures, confined space entry procedures? (If applicable)
14. Know proper procedure for the safe disposal of bio-hazardous waste? (If applicable)
