1 Part II

Sustainable Development Community Project Award The major theme of the programme will be ‘Sustainable Consumption of Biological Resources’ to echo the Council for Sustainable Development (SDC)’s public engagement process’.

The school should initiate a project with a collaborating partner (local non-profit making organisation) at local district level to involve parents as well as local residents and/or local institutions and organisations in the community in promoting SD awareness and practices to qualify for award under this programme. All the activities should be closely related and organised under a central theme to enable effective dissemination of SD messages and to create impact on the participants. Target number of persons to be reached in the community should be a minimum of 4 00 people count.

Project to be organi s ed for the promotion of Sustainable Development to the community Please give an account of a project to be organised for the promotion of SD to the local community during the programme period. The campaign should target parents, local residents and/ or institutions and organisations with a view to raising their awareness of SD and encourage SD practices in the community.

All the activities should be organised under a central theme and listed out in sequence on the following tables.

Collection and Use of Personal Data

1. The personal data provided in this proposal will be used by the SDC or any persons, organisations or agents appointed by the SDC for the purpose of record keeping and assessment on the Sustainable Development School Award Programme.

2. Provision of personal data in this proposal is voluntary. However, if participating schools fail to provide sufficient information, the assessment work of the SDC may be affected.

3. Data subjects have a right to request access to and correction of their personal data as provided in Section 18 and 22 and Principle 6 of Schedule 1 of the Personal Data (Privacy) Ordinance. Application by a data subject for correction and access of personal data as provided in this proposal may be directed to the Secretariat, Council for Sustainable Development (Address: 46/F, Revenue Tower, 5 Gloucester Road, Wanchai. Part IV Declaration: - On behalf of the school, I agree to be abided by the rules of the Guidelines on Application for Allowance of the Sustainable Development School Award Programme as set out by the SDC. I also understand that all allowance will be offered on reimbursement basis and based on the actual expenses on the activities and confirm that all the content stated in the Expenditure Forecast is true and correct.

- On behalf of the school, I agree that an Expenditure Report listing out all items of expenditures and being attached with invoices, receipts (original copies) of expenditures, and quotations (if applicable) should be submitted to the SDC on/before 31 January 2018 for allowance claim.

- On behalf of the school, I agree that if my school fails to complete the programme by the end of January 2018, the SDC may consider reimbursing the expenses on individual case basis and reserve the right to reclaim the disbursed allowance from my school.

10 Part V Collection and Use of Personal Data

11 Council for Sustainable Development 2016-2018 Sustainable Development School Award Programme Guideline s on Application for Allowance

The Council for Sustainable Development (SDC) will offer allowance to the participating schools of the 2016-2018 Sustainable Development School Award Programme on reimbursement basis. The allowance is applicable for subsidising schools’ expenditures on joining fees and transportation cost for participating in the relevant sustainable development (SD) related activities, and/or material costs for organising SD projects in the school and/or the community. For details of the application procedure, please refer to the following guidelines. Maximum A mount of Allowance

1. The maximum amount of allowance will be offered on reimbursement basis, to each participating school according to the levels of award enrolled. SD Community Project Award – HK$10,000 SD Participation Award – HK$2,000

Application P rocedure of Allowance

2. Allowance will be offered and based on the actual expenses on the activities. Participating schools (SD Community Project Award) are required to submit an Expenditure Forecast to the SDC for approval on / before 20 December 2016. A sample format of Expenditure Forecast will be provided by the Secretariat of SDC.

3. If participating schools (SD Community Project Award) wish to join and start the programme activities before 20 December 2016, they are required to submit a Programme Proposal and an Expenditure Forecast to SDC for approval by at least 2 weeks before the activity commences. No allowance will be granted for any expenses incurred before the approval of the Expenditure Forecast.

4. Participating schools (SD Community Project Award) are required to list out all possible expenses in relation to participating in SD activities and/or organising SD projects in the Expenditure Forecast, including joining fees, transportation costs, material costs and so on.

5. For any purchase of goods or services exceeding HKD$1,000, except being exempted for special reason, participating schools are required to obtain at least two written quotations and submit the quotations together with the Expenditure Report to the SDC Secretariat at the end of the Programme for reimbursement. Besides, the SDC Secretariat recommends the participating schools to accept the lowest offer for any purchase of goods/services. In case that the lowest offer is not adopted, the schools are required to account for the reasons in the Expenditure Report and confirm that the cost quoted by the selected supplier is reasonable.

6. If the Expenditure Forecast includes any costs on refreshment, the schools (SD Community Project Award) are required to state the target groups and purpose of providing such refreshment in the Expenditure Forecast in order to ensure that the expenditure is relevant to the SD activities organised.

7. Reimbursement for transportation costs is applicable for schools to hire coaches for transporting teachers and students to participate in SD-related activities.

8. Participating schools are required to observe that all procurement should be conducted on a fair and 12 competitive basis and complied with the relevant guidelines on tendering and purchasing procedures set out by the Education Bureau.

9. If there is any change in the project proposal which causes changes in the Expenditure Forecast submitted by the school earlier, the participating school (SD Community Project Award) is required to submit a revised Expenditure Forecast for SDC’s approval.

Timeline for Applying Allowance

10. Participating schools are required to submit an Expenditure Report (a sample format of Expenditure Report will be provided by the Secretariat of SDC) listing out all items of expenditures and being attached with all invoices, receipts (original copy) of expenditures and quotations (if applicable) to the SDC on/before 31 January 2018. All applications of allowance claims will be processed by the end of May 201 8.

Remarks 11. If any participating school fails to complete the programme by the end of January 2018, the SDC may consider reimbursing the expenses on individual case basis and reserve the right to reclaim the disbursed allowance from the school concerned.

12. If the total amount of an individual category of expenditure in the Expenditure Report (Activity Fees/Transportation Costs/Material Costs/Others) exceeds the total amount of that category of expenditure stated in the Expenditure Forecast by 50%, schools are required to account for the reasons in the report, for SDC’s approval.

13. The SDC has final decision on whether to grant the allowance to the participating schools of the Sustainable Development School Award Programme.
