Jean Battle opened the meeting and the minutes of the previous meeting were approved and seconded. The Healthy Living Festival was postponed until a later date as it was agreed that there was not enough time left to make all the necessary arrangements. With regard to the practice noticeboards which are on order, only the doctor’s names and photographs are to be displayed in the main Peel House area, and the photographs are all in hand.

Details regarding the survey are to be discussed further following a subsequent meeting as Jane Eccles is on maternity leave, but figures so far show 95% are satisfied or very satisfied. 44% were OK to provide information for a more detailed survey. 20% preferred extended hours in the evening. 37% are with a named doctor. Further work is required and this item will be deferred to a future meeting.

Integrated Neighbourhood Teams have been set up in Hyndburn in an attempt to improve communication and collaboration between various agencies, including, for example, community nursing teams. The project is in its very early stages but will be subject to ongoing monitoring and refinement. Further updates and feedback can be provided to the group as things develop.

Over 75s Nurses have been introduced throughout the locality and Peel House is employing two Nurses and two Healthcare Assistants in collaboration with other Practices in the area. The role of these nurses is to ensure that housebound patients over the age of 75 have equitable access to Practice services and hopefully reduce unnecessary hospital admissions and attendances in this group of patients. Contracts have initially been issued through to 2017 with a caveat that they will be extended providing the roles are a success. First quarter evaluation has shown a decrease in attendance at hospitals. These nurses will work with other integrated services and multi-disciplinary team meetings are already taking place within the INTs (Integrated Neighbourhood Team).

Dorothy asked about how housebound is defined and Carole stated that the term had been kept deliberately fluid as circumstances can often dictate whether or not someone is housebound, and also that circumstances of individuals can change rapidly.

Geoff mentioned the Patient Partnership Board which is a new concept introduced by the CCG. The purpose is to provide a strategic overview of patient engagement and involvement and to act as an advisory board to the CCG body. Specifically the CCG would be looking to receive regular reports from each locality patient network and to consider commissioning intentions. The first meeting will be held on 16th September at 4pm at Walshaw House if anyone is interested and able to attend. Dorothy raised the question of whether the receptionists could give out copied of the newsletters when patients attend for their flu appointments and it was agreed that this could be done. Carole will speak to staff. Katy also reminded the group that volunteers were still needed to attend some of the Flu Clinic Saturdays.
