STA-21 Applicant Check List *FOLLOW APPLICATION PROCEDURES* Application/Package should be assembled as listed below before mailing to NSTC (OD2/STA-21).

 _____ Commanding Officer’s Endorsement (Signed and Dated)  _____ Commanding Officer’s Recommendation Form (Signed and Dated)  _____ Officer Interview Appraisal Sheets (Internal Board) (3 URL Officers 03 or above) (Name Signed/ Printed with Grade/Designator/Billet Title/Branch of Service)  _____ Nomination Chairpersons Recommendation (Signed and Dated)  _____ Nomination Interview Board Appraisal Sheets (External Board) (3 URL Officers 03 or above) (Name Signed/ Printed with Grade/Designator/Billet Title/Branch of Service)  _____ Interview Verification Form (Signed and Dated by both COs)  _____ Applicant’s Cover Letter (Signed and Dated)  _____ Applicant’s Data Form Completed on Web Site (when applicable)  _____ Applicant’s Personal Statement (Signed and Dated)  _____ Certified to be True Copies of Last (5) Evaluations (No missing time)  _____ Certified to be a True Copy of SAT/ACT Scores from test taken within the last 3 years (Must have minimum required scores to be recommended for the program.)  _____ Certified to be a True Copy of ASTB Scores (Pilot/NFO Option)  _____ Certified to be a True Copy of High School Transcript (GED, if applicable)  _____ Certified to be True Copies of College Transcripts  _____ An unofficial copy of SMART/JST that has been certified to be a true copy by your command.  _____ Certified to be True Copies of Other Additional Documents (Certificates, Flag Letters of Commendation, or Page 4, is required for all awards and special qualifications).  _____ Certified to be True Copies of last 3 PFA cycles printed from PRIMS Report w/RAW Data (example: Spring & Fall 2010 and Spring 2011). Must have passed PFA taken within year of application - example: (1 July 2010 - 1 July 2011).  _____ INTEL/IP/IW/OCEANOGRAPHY selectees must have TS/SCI clearance or proof that clearance has been requested.  _____ Approved NAVPERS 1306/7 as required by NAVADMIN 073/13 (Nuclear personnel only.)  _____ Photograph (Nuclear Option Only)  _____ Page 13 Statement of Understanding for each OPTION applied for
