WELCOME TO ANOTHER AWESOME YEAR AT CPMS. PLEASE TAKE TIME TO READ AND UNDERSTAND THE FOLLOWING INFORMATION, AS IT IS DESIGNED TO HELP YOU CLIMB ABOARD THE SUCCESS EXPRESS AS WE MEET THE NEEDS OF EVERY CHILD, EVERY DAY. CPMS MISSION STATEMENT At Carolyn Park Middle School, our mission is to develop students’ social, moral and intellectual abilities to become life-long learners and productive citizens.

CARE OF ROOM AND EQUIPMENT

Each teacher is responsible for keeping his/her room and furniture as neat and clean as possible. Desks, bulletin boards, and walls should be checked frequently for writing, and steps taken to remove all writing as soon as it appears. These items should be inspected and repaired regularly. Students are not permitted to be in classrooms after school hours unless as part of an approved club or organization event.

Technology

All iPods, iPads, active votes, etc. will be inventoried on a daily basis. This will help eliminate missing devices.

Teacher’s Lounge and Workroom

The Teacher Workroom, Lounge, and adjoining areas are for staff use only. Students are prohibited from entering these areas or using the equipment. Staff and faculty are responsible for cleaning up after themselves and keeping equipment in good order for the next user. For assistance with any kitchen or workroom equipment, contact the office.

All requests must be made in writing and approved by Administration. The following chart will assist in knowing where to go for certain requests.

Teachers will receive 1100 copies per month for the copy machine. Paras will receive 300 copies per month. Use double sided copies when possible to save paper. The office staff will teach you how to create double sided copies if needed. This will assist with our resource management.

ACCOUNTING PROCEDURES

A receipt must be issued for all money collected. Teachers will be given one receipt book to be used to issue receipts to all students who turn in cash or checks. Receipts will be sent to the office in your money envelope. It is imperative to write a receipt for all money collected (especially cash); however, please do not receipt PTA money or picture money.

Any money collected as a grade level or school activity (student fee $10.00, field trips, fundraisers, etc.) should have a “receipt” placed in your money bag. This bag should be turned in to the office daily before 10:00 A.M. Receipt books will be turned in to Mrs. Dutton during the check out procedure at the end of the year.

Faculty members will not be allowed to cash checks or make change from funds collected for school purposes. Staff members are not permitted to sell anything while on campus without the prior approval of Administration.

SNACK SALES

Snack sales will be conducted only during recess time. Snack sales will be suspended on inclement weather days.

SCHOOL DOCUMENTS

All current and correct documents, forms and schedules will be saved to the CPMS Organization Content folder on Blackboard. Faculty and staff are expected to access Blackboard for all documentation needs. Communication

STAFF MAILBOXES

To ensure smooth communication to all staff members, e-mail and mailboxes have been provided. Staff members are required to check both once in the morning and once in the afternoon at a minimum.

ANNOUNCEMENTS

Faculty members will be sure to listen to overhead announcements and to watch Broadcast announcements each day. Faculty and staff will require that students be attentive to all announcements. Requested morning announcements should be received in the office prior to 8:00AM. Afternoon announcements should be received in the office prior to 2:30PM. Many important announcements are made for students and staff, so please ensure your class is paying attention. If the equipment in your area or classroom does not permit everyone to hear or see announcements clearly, staff should immediately submit a maintenance request for repair of that equipment.

Revised 5/29/2018 4 PERMISSION SLIPS AND EVENT FLYERS

Permission slips and event flyers are vital forms of proactive communication with parents and guardians. All documents that are sent home to inform or request consent must include necessary information, deadlines, restrictions and cost information. In addition, the Principal’s approval should be indicated on each page. Information or Event Flyers should be sent home in the Thursday Eagle Eye no later than one week prior to the event. Permission Slips should be sent home no later than two weeks prior to the return deadline.

To receive approval of a flyer or permission slip, submit the document to Mr. Mallory no later than the Monday prior to the Eagle Eye in which you want it to be sent home.

DAILY COMMUNICATION

Individual students may be required to check in and out with a faculty member at the beginning and end of each day. Those students will be identified by Administration and their homeroom teacher will be notified. Students should be issued a hall pass before leaving the classroom. They should check in between 8:15 and 8:30 AM, and check out between 2:45 and 3:00 PM. These times will ensure minimal disruption of classroom instruction.

WEEKLY COMMUNICATION

Every homeroom teacher will be responsible for packing and sending home the Eagle Eye folders every Thursday afternoon. Most documents from the office will be placed in your mailbox on Wednesdays.

PARENTAL CONFERENCES and COMMUNICATION

Teachers should contact parents and guardians using a school phone landline, not their personal cell phone. All voice mail messages should include caller’s name, date, time and reason for the call. This improves our ability to track communication with parents and strengthens the partnership between the school and our families. If you are not able to make a phone call and need immediate assistance in contacting a parent or guardian, please contact the Office.

Teachers must notify the Office when a parental conference, IEP or SAT meeting is scheduled. A parental conference form is to be completed and a copy turned in to the front office by the next business day. For IEP and SAT meetings, please email Administration. Professional Responsibilities

ATTENDANCE

Teachers are required to sign in on the designated sheet for their grade level (in the flip folder across from the mailboxes) each morning when they arrive. Teachers are allowed to leave campus during their lunch period, however, you must sign out with the office (in the green folder) before leaving.

In addition to district guidelines on being absent, contact Mrs. Dutton as soon as possible to ensure your classroom is covered.

You must notify the Principal twenty four hours in advance when you are planning to take a personal day.

Revised 5/29/2018 5 Teachers must be in their classrooms 5 minutes before the bell rings in the morning and remain 15 minutes after the bell rings in the afternoon except on duty days. Teachers on duty will stay until the last student has left.

CAMPUS SAFETY

Teachers will issue a Hall Pass to any student who will be outside the classroom without direct adult supervision. Students must sign the Emergency Check In/Check Out log in the classroom if they are taking an unscheduled bathroom break. Students are not permitted to walk outside the school gates at any time during school hours, even if supervised by Faculty or Staff. All visitors, including staff family members, will check in at the front office when they arrive on campus. If you observe someone on campus without a STPSB-issued badge, you will accompany them to the office or contact Administration immediately.

CELL PHONE USE

Cell phones are not to be used during class time. Cell phones are not to be present in the classroom during state testing. No earbuds or headphones may be worn on campus except for custodial personnel. Team Participation

GRADE LEVEL MEETINGS

Grade level meetings must be held once a month. Minutes and sign in sheets from these meetings need to be turned in to Administration. Grade Level Chair Responsibilities:  Recommend the ordering of materials for their grade level, including all instructional materials.

 Conduct monthly meetings with grade level members and submit minutes and agenda to Administration.

 Plan and review lessons with grade level members

 Serve as a consultant in their respective grade levels to new teachers and others needing assistance.

 Other instructions given by Administration COMMITTEES

It is expected that all faculty members will serve on two committees. Committees are a great way to show support for our community and allow our students additional opportunities to grow. In addition, it is expected that all staff and faculty will participate in and attend at least one Title 1 Event each year.

DUTY

A duty schedule will be created and distributed at the beginning of each year. Promptness is important in arriving at your duty station. Morning duty begins at 7:30 AM. If you are going to be out on your scheduled duty day, it is your responsibility to make arrangements for your duty position. If you call in sick, the substitute will assume your duty.

Revised 5/29/2018 6 Each duty area should have a radio for communications. Radios can be obtained in the Assistant Principal’s office and should be returned there at the end of each duty shift. All homeroom teachers are expected to be at their classroom door during afternoon dismissal, to monitor student activity and ensure clear communication.

PBIS

PBIS is integral in the success of our behavior program. PBIS meetings will be held monthly. Agendas, sign-in sheets, and minutes will be given to the Assistant Principal. The proper use of Eagle Bucks is an important factor in PBIS. Class-wide distribution of Eagle Bucks is not permitted. Eagle Bucks should be distributed on an individual basis. Discipline Procedures

INFRACTIONS When students do not meet the school-wide expectation of being respectful, responsible, and safe, teachers will follow the following discipline procedure: 1. Complete a CPMS Infraction Form. Complete the form in its entirety, indicating the school quarter and number of this form (first, second or third infraction within specific quarter), student information, incident description, and intervention(s) taken (1 – 6). a. Have the student sign the Infraction. If a pull-out teacher writes an infraction, it should be given to the student’s homeroom teacher (pull-out teacher either hand-deliver s or places in homeroom teacher’s mailbox). b. Homeroom teacher keeps the Yellow, Pink and Gold copies, and sends Green and White home for parent signature. Parent will keep the Green copy and will return the White copy to the teacher.  Upon the 3rd (Third) Infraction within a specific quarter, the teacher will contact the parent via phone or schedule a conference to discuss the student’s behavior. Record the date of phone call or conference on the Third Infraction Form in the space provided.  Teacher Interventions are extremely important prior to referring a student to Administration.  Make sure you have documentation available of interventions attempted. Consistency is key! 2. An alternative to writing an Infraction, at the discretion of the teacher, is to assign the student to a Cross- Buddy Teacher for a maximum of 30 minutes. This procedure gives the student the opportunity to change behavior outside of the setting in which the school-wide expectations were not met. a. Complete the Cross-Buddy Room Checklist – complete student and time information; indicate reason for Cross-Buddy assignment and describe the work to be completed during the Cross-Buddy time. b. Cross-Buddy Teacher will indicate how the student behaved, sign the form, and return the form to the sending teacher. c. Student will sign, indicating receipt of the form. d. White copy is sent to the parent, pink to the Cross-Buddy teacher, Yellow and Goldenrod are kept by the referring teacher. e. Sending a student to the office for a “Cool Down” is not an option. Students who do not respond appropriately to an Infraction or Cross Buddy will be met, in the room, by Administration. Please contact the Office and request the assistance of an Administrator.

Revised 5/29/2018 7 3. Following the third infraction within a specific quarter, students who do not follow school-wide expectations will be issued a Level 3 Report. Major discipline issues should also be reported on a Level 3 Form. Teachers should complete the Level 3 Report in its entirety and attach minor infraction form(s), if applicable. The Level 3 Form should be kept all together and sent to the office. Level 3 reporting procedure: a. Drop form in Discipline basket in front office for administrator pick up. b. DO NOT send students to front office with Infractions or Level 3 forms. c. Administration will investigate and take disciplinary action within 48 hours, returning yellow and pink copies to your mailbox for your records. d. Teachers are NOT to have students write statements. Administration will complete the investigation process. CRISIS TEAM

Follow the District Crisis Plan for major incidents.

Student crisis situations will call for the Crisis Intervention Team. Crisis Intervention Team Members will be named at the beginning of each year.

The code for calling this team is “I have a light out in room…”. Only members of the crisis team should respond to the event. All other members please refrain from interfering with the team. Curriculum & Instruction

LESSON PLANS

Lesson plans are due by 9:00 AM on Mondays. All lesson plans must be sent electronically though Blackboard in the designated format.

Lesson plans may be submitted individually or by group.

When major technology is used with the lesson, it must be indicated on the lesson plan.

Any movies that are going to be put in the lesson plan must have Administration approval first.

Sub lesson plans must be submitted to the office and updated periodically. Sub-lesson plans should be completed for the first week of school.

Work for students serving In School Suspension (ISS) should be coordinated with the teacher’s current lesson plan, designed to take up the entire school day, and sent to the front office prior to the start of the instructional day with any necessary materials (textbooks, calculators, special paper or copies, etc.) It is the teacher’s responsibility to check in on their student during his/her planning to monitor the quality of work completion.

BULLETIN BOARDS

All teachers will be assigned bulletin boards in their respective hallways. Ensure that these boards are updated on a monthly basis.

Revised 5/29/2018 8 PROFESSIONAL LEARNING COMMUNITY (PLC)

Time will be allotted each week for PLC meetings. These meetings are to consist of assessment planning, student tracking and data collection.

All teachers must attend their designated PLC meetings. A sign-in roster will be kept and turned in after each meeting.

TEACHER ASSISTANCE TEAM (TAT)

Purpose: TAT is designed to use the resources available at the school to help teachers evaluate, design, and implement interventions for children in their classroom. Focusing on instructional and behavioral strategies, TAT aims to lower the amount of students needing to participate in the SAT process.

Meetings and Team Members: TAT will meet Thursdays at 2:45 in room 228. The members of the TAT will be designated at the beginning of each school year.

TAT process: Once teachers have identified students needing assistance, the teacher will call for a parental conference and maintain a written record of the meeting. If a student has a failing grade on an Interim Report or a Report Card, a parental conference must be held and the teacher is required to refer the student to the TAT.

The teacher will schedule a meeting with the TAT via e-mail.

The teacher will complete the TAT teacher questionnaire (stored on Blackboard).

The teacher will gather work examples, grades, and other pertinent information for the meeting.

STUDENT LEARNING TARGETS (SLT)

SLTs will be written and approved with the assistance of Administration. The targets will be set in the fall and reviewed during the mid-year assessment.

The pre-test will be administered and graded by the classroom teacher.

The mid-year assessment will be administered and graded by the classroom teacher.

The end of year assessment will be given and graded by a partner teacher. All end-of- year assessments must be turned into Administration. All SLT assessments must be kept for 3 years.

Dates for giving each assessment will be decided by Administration.

ICU

The ICU program assists in students completing their work. Teachers will keep up to date records on what assignments students owe. This list will be distributed to all staff members to use with assisting students in completing assignments. Revised 5/29/2018 9 Each grade level will be assigned a coach to assist with parental notifications of assignments due and assist with record keeping.

Students may be sent to the ISS room to complete work after coordinating with ISS monitor. Do NOT send work with student to complete in an enrichment class.

Students with assignments due cannot attend any after school functions or participate in PBIS activities.

IN SCHOOL TUTORING PROGRAM

With Title I funds, Carolyn Park has decided to have tutors during the school day to assist students in academic need. A tutor for math and a tutor for ELA have been placed on staff to fill this need.

Tutors will assist students, in their regular classrooms, based on their progress toward academic success. The teacher will provide Administration with a list of students who are recommended/in need of tutoring assistance within the classroom. Teachers may request that a student be pulled out and tutored one-on-one in an alternate location.

Tutors will have default schedules with times and classroom locations, however if a student is referred to tutoring by TAT, the TAT schedule will take priority over the tutor’s regular classroom schedule. (TAT will determine the time the tutor will meet with the student.) Tutors will keep track of the days and times they meet with each student referred by TAT. The Testing tutor will assist with pulling these students and giving them an assessment periodically to determine the success of the tutoring intervention.
