SPONSOR/EXHIBITOR AGREEMENT This document confirms the agreement of the following company/institution to become sponsor and/or exhibitor of the 4th International Conference and Exhibition on 3D Body Scanning Technologies, to be held from the 19th to 20th November 2013, in Long Beach CA, USA. Note: the different sponsor/exhibitor levels are shortly described at the end of this document. Please fill this form, save it and send it by e-mail to the conference office (see below). You will then receive an invoice with details for the payment of the total fees. Note: you can paste text into the grey areas and use the tab key for a fast navigation between them.

Payment Terms Prices are in US Dollars (USD). Payment methods: bank wire transfer (with IBAN number and SWIFT/BIC code) on our USD/EUR/CHF Swiss bank account or credit card (VISA, MasterCard, American Express) by Saferpay secure e-payment (+5% taxes) or through PayPal payment system (+6% taxes). Invoicing and payment are possible in USD, EUR or CHF.
