Advanced Vacation Leave Request for the February 2014 Inclement Weather Closure
The University was closed from 2 p.m. Thursday, February 6, 2014, until 10 a.m. Monday, February 10, 2014.
Employees without available leave balances, including new employees, will be allowed to take an advance of future vacation leave earnings for time missed due to the Inclement Weather Closure, Friday, February 7th through Sunday, February 9th, 2014, by completing the following vacation advance request form to cover any time remaining during the closure after all other available leave is exhausted.
Vacation leave advanced will be deducted from your “banked” vacation leave balance. If you should terminate employment with PSU while your vacation leave is still “banked” this vacation leave advance will be deducted from your final payroll check.
To request the vacation leave advance please complete the following by indicting that you are requesting an advance of vacation hours and the number of hours you would like to use in advance.

REQUEST AND AGREEMENT FOR THE ADVANCE USE OF BANKED VACATION LEAVE
I request the advance use of ______hours of vacation leave for the February 2014 weather closure. I understand that these leave hours are borrowed from the vacation leave to be credited to me after having completed trial service and/or my future vacation leave accruals. I further understand that if my employment is terminated for any reason before this advanced vacation leave has been earned in full, I will be in an overpaid status and I agree to repay the University for the advance vacation used and/or to have these advanced hours deducted from my final payroll check.
I do not want to use advanced vacation leave for the winter break closure. I will take ______hours in unpaid leave.

This form is due by 5 pm on February 14, 2014. After the form is completed please return it to Human Resources in Suite 518, Market Center Building, fax it to 5-5896, or scan and email to [email protected].
