2. Contract # - List CDBG contract number (one contract per each Attachment C-1)

3. National Objective – Select from drop-down menu

4. Activity Matrix Code – Select from drop-down menu

5. Accomplishment Narrative - Provide narrative for accomplishment(s)

6. Total Persons Assisted – This field will auto-populate

7. How Many Veterans – List the number of Veterans assisted

8. How Many Disabled – List the number of disabled served

9. Provide the number of persons served as identified by “Required” 10.The total number of persons identified will auto-populate (calculate) in box for “Total Persons Assisted”
