How to Complete an Imprest Trial

Equipment Program - Children & Adults How to complete an Imprest Trial / Prescription record form

The imprest store is a small selection of category 1 equipment available to be taken out for trial with clients. If the equipment is suitable, it can be left with the client, if not then it can be returned to the store. The imprest store will be replenished by DES on a regular basis. Novita: Imprest equipment is not be couriered to other Novita sites, if this is needed please request items from DES using usual category 1 process.

Before taking imprest equipment, please fill in the imprest form as outlined below:

1.Fill in the client section including program/team and client weight

2. Complete the prescriber section with your details

3. Tick your imprest site (ie NOVITA Regency Park)

4. Complete details of the items you are taking for trial by filling in the left hand side of the imprest form to include details from the ASSET sticker (see below).

5. Place partially completed form in the PENDING tray until the completion of the trial (Max 7 days)

6. Once trial completed please indicate trial outcome by completing right hand side of record form.

7. Please complete the outcome section for each equipment item trialled.

8. Once the form is completed, please place form into the COMPLETED tray.

9. Store personnel will fax completed forms to DES.

10. DES staff at Netley will book items to client in ESIS and restock issued items to imprest weekly.

Equipment Returns

Please attach an orange toe tag to each item requiring return to DES and place in the red-taped designated area to be collected weekly for return to DES.
