Applications submitted after 3:30 p.m. on February 4, 2016 will be reviewed on a space-available basis.

FOLLOWING ARE INSTRUCTIONS TO COMPLETE THE APPLICATION:

1. Read the entire application to determine eligibility and requirements.

2. Complete and submit the online application question responses, in one sitting (parent/guardian and student together), and by the aforementioned deadline. Incomplete applications will not be processed.

3. Request all school-level data reports, and recommendations early in the process to give school personnel plenty of time to gather information, complete, and submit. Get confirmation the requested items were sent to CROSSROADS Early College via the school courier and online by February 4, 2016 at 3:30 p.m. (Non-Iredell-Statesville middle school students must have the current middle school mail these items to Crossroads by the deadline.)

4. Notification of acceptance or denial to enroll in Crossroads will be delivered via U.S. Mail by April 20, 2016. (Students who are not accepted during the first selection round may still be accepted later on a space available basis.)

Thank you for considering Crossroads Arts and Science Early College (Crossroads) for your high school program. Before completing the application, we request that you carefully consider the information below to determine if CROSSROADS is the most suitable educational experience for you and your child. As you complete the application, please make special note of the expectations to which you and your child will be agreeing, if he or she is accepted at CROSSROADS.

• Demonstrates ability to make mature, independent, and productive choices and to succeed in high school honors classes and college classes.

• Desires to be intellectually challenged and committed to embrace education.

• Makes education a priority over social and recreational interests.

ATTENDANCE POLICY

Good attendance by our students in both their college classes and their high school classes is vital to our program. Families who choose CROSSROADS are asked to schedule vacations, doctor visits, and other appointments so that they do not conflict with high school or college classes.

CALENDAR

Because CROSSROADS students take college classes, our calendar is aligned with Mitchell Community College. There are days that CROSSROADS is not in session but the community college is in session. On those days, the student will still be held accountable to the professor’s attendance policy, and families will need to provide transportation. Families considering CROSSROADS are encouraged to carefully note the differences in vacation and summer schedules.

BUS TRANSPORTATION

Since CROSSROADS serves the entire county, it is not feasible for us to provide door-to-door bus service. We provide transportation from shuttle locations in each area of the county. Families are required to provide transportation to the remote shuttle locations. In addition, there will occasionally be family transportation requirements for after-school high school requirements and/or college requirements.

Step 1: Distribute recommendation requests to the appropriate school personnel for online submission, via an internal Google doc form, to CROSSROADS by 3:30 pm, February 4, 2016. (If the student attends a non-Iredell-Statesville-Schools middle school, please print the math teacher, language arts teacher, and counselor recommendation forms; ask the school to mail the forms to CROSSROADS by the deadline.)

Step 2: Distribute a request to the middle school data manager to submit the following reports to CROSSROADS via school courier: Historical Grades at semester end in January (or Transcript), Test Results from PowerSchool (or EOG scores), Explore Scores (Composite score), Incident/Discipline Report for all years, Attendance/Tardy Report at semester end in January, and IEP/504/AIG/ESL plan if applicable. (If the student attends a nonIredell-Statesville-Schools middle school, please print the data request form for the middle school personnel; ask the school to mail the forms to CROSSROADS by the deadline.)

Step 3: Together (parent/guardian and student), in one sitting, fill out all 3 sections of the online application form. Complete all signature portions of the online application. Submit the online application by 3:30 p.m. on February 4, 2016.

Step 4: A checklist of materials is provided at the end of the directions on the next page. Applications need to be completed online, and components sent from the middle school via school courier and online (or by mail for a non-Iredell-Statesville-Schools student) no later than 3:30 p.m. on February 4, 2016.
