Hawthorne Day Vendor Registration Form

VENDORS are needed for Hawthorne Day on August 18, 2007, Event is 11:00am – 7pm. Celebrate the end of construction on Hawthorne, and the placement of bronze sidewalk medallions and historic plaques at each end of Hawthorne. Live music on several stages, vendors, art, balloons, sidewalk sales, free samples, with a special children’s area at 35th. Vendor areas at 36th and 32nd Avenues. Vendors will receive up to a 10-by-10-foot space for the day. Setup starts at 9:00am. No food items can be sold. Vendors are responsible for their own items and sales. Fill out the form below and send a check to HBBA, Hawthorne Boulevard Business Association. To sign up, fill out this form & enclose payment . A confirmation letter will be emailed. Locations assigned only after payment & registration form are received. Vendors must sign the liability waiver below.

RELEASE AND INDEMNITY AGREEMENT (Vendor/tabler), hereby agrees that he/she is an independent vendor or representative of a non-profit and is not and will not be an employee of the Hawthorne Day Committee, during the performance of this agreement.

The Hawthorne Day Committee and The City of Portland and their agents and assigns assume no responsibility or liability for injury or damage. Vendors release, indemnify, and hold harmless The Hawthorne Day Committee, the Hawthorne Blvd. Business Association and The City of Portland and their agents and assigns from all loss, injury, damage, or liability to persons or property sustained by reason of presence of the vendor at the 2007 Hawthorne Day or any portion thereof on Aug. 18, 2007.

I hereby acknowledge that I am eligible to participate in the 2007 Hawthorne Day event and have read and agree to comply with the established rules of this Fair. I further understand that the Committee may revoke such authorization at any time for violation of these rules or other actions which do not conform to Board Policy.
