STATE PRESCHOOL PROGRAMS PRE-REGISTRATION Dear Parents, The Centralia School District (CSD) State Preschool Programs at Centralia, Danbrook, and San Marino Schools are seeking children for placement during the 2017/2018 school year. The Preschool offers a variety of activities designed to meet the development needs of children. This is a wonderful opportunity for your child to enjoy a positive school experience, which will better ready him/her for kindergarten. Enrollment is open to children who will be 3 years of age by September 1st and reside in the Centralia School District attendance area, with 1st priority going to children 4 years of age in the Centralia, Danbrook, and San Marino School attendance areas and 2nd priority to children 3 years of age and 3rd priority to children who reside within other school districts. Eligibility is determined in accordance with State Income guidelines and not a first come first serve basis. Enrollment is limited, so please fill out the attached Preschool Pre-Registration form and attach copies of the required documents (listed below). Return to the Neighborhood Resource Center at Danbrook School or CSD Child Development Office at the Walter Knott Education Center, 7300 La Palma Ave., Building 5, Buena Park, CA 90620. Pre-Registration applications must be turned in no later than March 31th, 2017. If your application is turned in after this date or is incomplete you will be placed on the Preschool Waiting List. If you are placed on the waiting list and you get called you have a 48 hour period to respond or else the spot will be given to the next applicant. The preschool program will follow the regular Centralia School District school calendar. There will be two (2) class sessions. The classes are held daily with an AM Session and a PM Session. If you are accepted you will be given the time your child will be attending at your parent orientation meeting. To apply for a placement in the Centralia School District’s State Preschool Programs at Centralia, Danbrook, and San Marino Schools the following items are required to accompany your application. We cannot place your child without them.

 Completed Application ( Print clear and legible with ALL INFORMATION COMPLETED )  Birth Certificates for every child in the family. Preschool child must be 4yrs. old by Sep.1 st  Copies of your most recent pay check stubs for a one month period. These stubs are necessary to determine eligibility in the State Funded Preschool Program. If both parents are working we will need monthly pay stubs from both parents. If self-employed – a Bank Statement is needed, plus Business Card, Business Advertisement, Business Tax ID #, and Business License (if applicable). If income is in the form of public assistance, family’s Medi-Cal number is needed. A copy of the most recent Notice of Action from AFDC needs to be sent as it includes income and Medi-Cal number.  Copy of your child’s immunization records (Little Yellow Booklet)  Proof of residence indicating that the parent is the responsible party: lease or rental agreement  Physical form filled out by the child’s primary doctor.  Documentation that confirms the presence or absence of a parent
