ATHLETIC INFORMATION Are you interested in the Dobson High Interscholastic Athletic and Activities program? In order to establish eligibility, the student athlete is required to have on file with the Athletic Director’s office all forms and documents as required by the State, the School District, and the Arizona Interscholastic Association. ATHLETIC FEE: A Participation Fee for all athletic programs in Mesa Public Schools will be implemented for the 2011-2012 school year. The cost will be $100 per sport with a maximum of $300 per school year per individual. Non-cut sport payments are due at the beginning of the season. Cut sport payments are due upon making the team. Payments are made through the school bookstore. Contact the school Athletic Director if in need of financial assistance. The eligibility requirements listed below will simplify the process of clearing. A sports packet may be obtained from the Athletic/Activity desk in the administration office or from your coach. Upon completion, return your completed packet to Mrs Koch, the Athletic Secretary. PHYSICAL CLEARANCE: All students participating in sports must have on file a record of a physical examination on a yearly basis. As per AIA Bylaws 15.7.A/B, the physical for the following school year shall be given on or after March 1st. The medical provider must be one of the following: M.D., D.O., N.P., or PA-C and licensed to practice in Arizona. The physical has to be documented on the approved AIA form, which is located within the physical packet. A parent must sign the physical packet. PHYSICAL: Get your sports physical done on campus! Physico Health Assessment will be conducting sports physicals in our Staff Lounge Area on Wednesday, August 3rd from 430 pm-730 pm. Cost will be $25.00 cash or money order. Please pick up a physical packet from Dobson Athletic Office prior to the physical date. Please contact Mrs. Koch in the front office (472-3054) for further info. PARENT/GUARDIAN PERMISSION AND CONSENT FOR EMERGENCY CARE: This form must be completed by the parent/guardian prior to eligibility. NEW: Parents and students must review and sign these forms prior to season (return with physical packet). CONCUSSION STATEMENT AND ACKNOWLEDGEMENT FORM: (As per AIA bylaw 15.7c) all athletes and parents are required to understand and acknowledge the contents, consequences and implications of the Mild Traumatic Brain Injury. CODE OF CONDUCT FORM: Mesa Public Schools – Code of Conduct all athletes and parents must review and acknowledge this new policy. BIRTH CERTIFICATE: A copy of your birth certificate or equivalent proof of your birth date and place of birth must be on file in the Athletic Director’s office prior to clearance. ATHLETIC/ACTIVITIES HISTORY: This information is necessary prior to eligibility clearance: Student will be living in Dobson’s attendance zone as designated by the Mesa Public School District; or you are enrolled at Dobson with an open enrollment and will not be attending the high school within the attendance zone in which you live. ATHLETIC PARTICIPATION: Student athletes participating in team sports are not allowed to practice or compete in any other group, club, organization, association, etc. in that sport during the interscholastic season of competition (AIA Bylaw 14.4.1). Students violating this rule will lose up to one year of eligibility and the school team will be penalized as well. STATEMENT OF AWARENESS: Parent/Guardian and Student must view the informed consent film. The Parent/Guardian and Student must read, understand and sign the Harassment/Hazing statement. ACADEMIC RULE: The student must be enrolled in a minimum of five (5) courses the first six semesters of high school and a minimum of 4 classes during the 7th and 8th semesters. ATHLETIC FAMILY PASS Being a fan provides hours of excitement and fond memories but can put a strain on the wallet. A Family Pass can be purchased for $85 and allows entrance for 2 adults and 2 students to all home games. Passes may be purchased in the Dobson Bookstore. STUDENT ACTIVITY CARDS By purchasing a student activity card the student receives many discounts throughout the school year for home athletic events, dances and yearbook. PE LOCKS Students are issued PE locks and should a lock be lost or damaged there will be a $5.00 replacement fee.
