NHS CITIZENSHIP – EVALUATION FORM

Enter the due date in the space provided at the bottom of this page. You need the recommendations of at least four of your teachers (two from the current term and two from previous terms, minimum.) Feel free to provide more recommendations from on campus or from other persons off campus who know your character if you feel that they will help provide a clearer picture of your individual qualities to the Faculty Council.

TO THE EVALUATING TEACHER: The student named above is submitting his/her name as a candidate for membership in the Temescal Canyon Chapter of the National Honor Society. As the name suggests, membership in this organization is an honor, and we are looking for honorable students who demonstrate the qualities of scholarship, leadership, citizenship, and service to the community. If you would take a few minutes to rate this student on the following characteristics, it will allow the NHS Faculty Council to make a more informed decision on this candidate’s qualifications for NHS membership. Letters of recommendation in lieu of this form are also welcome. Please use the following scale for evaluating the candidate.
