POSITION PURPOSE:

The position (under limited direction and supervision of the Facility Manager) is responsible for the provision of clinical leadership, advice, coordination, planning, review and assessment of all aspects of clinical care delivery at the facility to ensure an effective and customer focused service delivery for all residents.

The role is critical in driving the clinical performance of the facility to ensure it achieves all KPIs with particular emphasis on ACFI, risk management and quality assurance, whilst ensuring that all customer service requirements and accreditation obligations under the Aged Care Act 1997 and Accountability Principles are met or exceeded.

The incumbent is expected to assume responsibility for the overall management of the facility in the absence of the Facility Manager.

The position is one of eight pool positions and incumbents in this role may be requested to undertake CCC duties and/or clinical projects at other sites on a short or long term basis as operational need arises. Incumbents will also be expected to undertake weekend, afternoon or night shifts as directed by the Facility Manager for the purpose of ensuring consistency in care standards delivered and staff competency across all shifts.

Dimensions of the Position:

The position is full time (Mon-Fri) and requires excellent knowledge of ACFI, Aged Care legislation including the Aged Care Standards and Accreditation Systems, OHS legislation, Infection Control mechanisms and nursing scope of practices as per AHPRA whilst applying sound business management principles including resource management, leadership, continuous improvement, and financial management.

This position reports directly to the Facility Manager and will liaise closely with all clinical staff, management, support staff, carers, residents, families, allied health professionals, GPs and relevant external organisations and/or agencies, in relation to the provision of a range of holistic care services to residents in a timely manner that is in accordance with:

Key Accountabilities/Responsibilities:

ACFI

 Maintain a sound working knowledge of best practice in ACFI and funding subsidies;  Coordinate, facilitate, oversee and actively participate in the implementation of assessments and care planning for new residents (including respite residents) upon admission to the Facility;  Oversee and implement all scheduled mandatory and voluntary ACFI reappraisals as required;  Ensure resident assessments, initial care plans/reviews and other relevant clinical documentation are completed accurately and reflect the complete individualised needs of the resident and legislative requirements;  Maintain and update ACFI files and packs as required;  Participate in ACFI validations and liaise with external stakeholders as appropriate;  Advise the Facility Manager, of any documentation anomalies or discrepancies that impact on, or have the potential to impact on, appropriate funding for each resident;  Work with all clinical care staff, to facilitate, support, and coordinate a comprehensive ACFI approach to resident care and ensure integrity in DGAS ICARE resident care database;  Promote a culture of staff undertaking responsibility for completing documentation accurately, completely and in required timeframes;  Participate as required in the facility orientation and induction programme for new clinical staff to ensure they understand the ACFI process and DGAS policies and procedures in relation to clinical care documentation;

Resident Focused High Quality Care:

 Promote a resident focused approach in all care provision and staff behaviours  Directly involve residents and their families/carers in all aspects of care delivery including clinical care, ancillary services and leisure and lifestyle programs.  Promote a holistic approach to care service provision that incorporates the residents personal believes, values, needs and culture in their individual care plans  Ensure residents rights to dignity, privacy and confidentiality are strictly maintained at all times  Ensure consistency in the provision of clinical care for all residents including respite, permanent and extra-services residents at the facility.

Clinical Care Provision:  Ensure all care plans are complete, accurate and current in accordance with resident’s clinical needs and individual preferences  Monitor compliance with and quality of care provision to all residents by clinical staff and initiate appropriate corrective action where required.  Ensure staff allocations are based on relevant resident’s individual clinical needs, staff skills and experience and capacity to care appropriately for the resident.  Initiate and undertake individual and group education, training, coaching and mentoring for clinical staff at the facility.  Provide guidance, advice, assistance and support to all clinical staff in a manner that is empowering, encouraging and driven by best practice clinical principles and procedures.  Participate in the development of nutritious and varied menus with catering staff and the facility dietician in line with resident’s needs and individual preferences.  Foster and encourage a multidisciplinary approach to care provision for all residents  Actively consult with medical practitioners and other health professionals to ensure best practice outcomes for each resident.  Actively manage and participate in care service provision to residents with complex clinical care needs with particular regard to wound management, diabetic management, behavioural management, medication management, nutrition and hydration management, pressure care management and incontinence management.  Record changes in resident health status and act on staff information regarding Residents in a timely manner.  Participate in scheduled annual resident review conferences and/or adhoc resident and family meetings as required  Ensure medications are administered, documented, stored and disposed of in accordance with regulations and policy

Management:

 Efficiently undertake management of day to day clinical operations  Effectively deputise for the Facility Manager when they are absent  Maintain and improve documented procedures in Doutta Galla manuals covering personal care planning, ACFI classification, activities programs, infection control, wound management, medication management, continence management, nutrition and hydration and other areas assigned by the Facility Manager in line with corporate requirements.  Actively participate in Fire and Emergency training, drills, and response in accordance with the responsibilities of the CCC role.  In consultation with the Facility Manager participate in the monitoring and review of facility performance against key KPIs  Promote and actively demonstrate open and honest communication exhibiting excellent listening and verbal and non-verbal skills  Promote Doutta Galla in a positive manner at all times  Actively participate in the selection, orientation and engagement of new residents and their families in consultation with the facility manager and admissions coordinator.  Comply with all reporting requirements including incident reporting and elder abuse/mandatory reporting  Participate in incident investigations and root cause analysis where required  Participate in risk assessments and hazard identification  Liaise with health and safety representatives, OHS Advisor and Injury Management Advisor in relation to health safety and work environment requirements.  Provide support to residents and their families and ensure any queries, comments, suggestions and / or concerns are managed promptly, effectively and efficiently;  Participate in the recruitment and selection of appropriately skilled and experienced clinical staff in line with Doutta Galla’s policies and procedures  Undertake assessments, monitoring, evaluation and review of new clinical staff in accordance with Doutta Galla’s Probationary Policy and procedures.  Assist with achieving and maintaining the facility’s occupancy targets including developing strategies to meet and sustain all targets

Staff Supervision:

 Directly supervise and manage all clinical staff including registered nurses and personal care staff  Ensure all new staff, contractors, volunteers or other relevant persons are appropriately inducted and orientated in accordance with relevant policy and procedure.  Monitor staff attendance and participate in Doutta Galla’s attendance management processes and procedures  Monitor staff performance and conduct annual reviews of clinical staff as assigned by the Facility Manager  Immediately address any poor/non performance in accordance with Doutta Galla’s performance improvement and Employee Counselling & Disciplinary Procedures  Monitor staff compliance with Accreditation Standards and legislative obligations  Arrange and conduct regular staff meetings and tool box talks with clinical and support staff at the facility to ensure effective communication, consultation and training in care provision  Manage, monitor and implement staff rosters in accordance with skill requirements to achieve required standards of clinical care delivery.  Manage and enforce Doutta Galla’s policies and procedures in relation to shift transfers and swaps of rostered staff.  Ensure staff compliance with Doutta Galla’s harassment, discrimination and bullying policy  Ensure staff compliance with Doutta Galla’s occupational health and safety policies and procedures including enforcement of Doutta Galla’s ‘no lift’ policy.  Participate in and monitor return to work and rehabilitation programs and activities for injured employees ensuring compliance with any work restrictions.  Supervise nursing and other work experience students on placement at the facility

Continuous Improvement and Quality Management:

 Undertake all duties in a manner that ensures the facility meets or exceeds accreditation requirements  Actively identify areas of risk and work collaboratively with all staff to identify and implement risk mitigation strategies  Actively identify opportunities for improvement in clinical care provision adopting a quality focused approach  Personally initiate, undertake and complete relevant training and education  On a daily basis monitor performance in operational areas and discuss proposed improvements and goals with the Facility Manager  Schedule, undertake and comply with all standard audits and audit procedures  Actively participate in quality improvement initiatives and audits designed to evaluate the quality of care and services;  Actively participate in the Accreditation process;  Conduct quality audits as directed by the Facility Manager

Occupational Health and Safety:

 Ensure a safe, healthy work environment for all staff that meets regulatory requirements;  Undertake all duties in a manner that demonstrates, at all times, due regard for the wellbeing and safety of self, colleagues and residents;  Adhere to the relevant Occupational Health and Safety legislation and associated policies and procedures;  Monitor and guide staff practices in order to minimise the risk of workplace injury and illness and, where required, assist injured employees in their return to work through positive support and guidance;  Ensure all incidents, dangerous occurrences, near misses, accidents, injuries or hazards are immediately actioned and documented in accordance with organisational guidelines;  Respond appropriately to situations of risk or potential risk to the health, safety and wellbeing of staff, residents, volunteers, contractors or other relevant persons;  Monitor the safety of equipment and ensure equipment is maintained and used in a safe manner in line with manufacturer’s instructions and organisational guidelines, and report any malfunction, disrepair, wear and tear or breakdown to the relevant Facility Manager or Director of Residential Care as appropriate;  Ensure all equipment repair requests are documented in accordance with Doutta Galla policies and procedures;  Develop, coordinate and/or participate in relevant training and education sessions regarding occupational health and safety in the workplace;  Understand and/or be accountable for emergency code management, fire and evacuation procedures and implement if required.

Key Performance Indicators:  100% Resident Care Plans are complete, accurate and up to date with review schedules met  100% compliance with all charting requirements for wounds, nutrition and hydration, incontinence management, behavioural management and diabetic management  100% compliance with incident reporting (including mandatory reporting requirements and occupational health and safety)  100% compliance with all scheduled audits  ACFI funding targets for the facility are met or exceeded  100% compliance with mandatory/refresher training requirements for self and direct reports  Facility budgets in relation to labour costs for permanent, casual and agency are met or bettered  100% compliance with scheduled staff competency assessments including, but not limited to, medication competency assessments

Professional Responsibilities:

 Maintain professional and ethical practice in line with legislative requirements, Victorian Nurses Board requirements and the Nurses Code of Conduct with accountability for own service delivery and clinical practice;  Maintain abreast of current practice and trends in the provision of aged care services as well as other areas such as occupational health and safety and infection control;  Comply with all Privacy Legislation requirements and Doutta Galla confidentiality statements when communicating information pertaining to residents, staff and the operations of any Doutta Galla facilities;  Recognise the need for and actively participate in continuing education and development designed to maintain and improve competencies;  Where care delivery issues are unclear or beyond own abilities and qualifications, seek assistance and clarification.

Key selection criteria

Mandatory

 Current Registration with AHPRA as a Registered Nurse, Division 1  Demonstrated knowledge of contemporary nursing theory and practice in relation to the care of complex, high and low care residents in a residential environment  Current First Aid and CPR certificates  3-5 years Experience in Aged Care, preferably in residential and / or community setting in a senior clinical role  Demonstrated experience, knowledge and understanding of relevant legislation including Aged Care Act, Accreditation Standards, Accountability Principles as well as ACFI assessment, care planning, and validation procedures  A strong commitment to excellence and quality in the provision of aged care related services with a sound working knowledge of continuous improvement, quality principles and accreditation.  Highly developed leadership skills and the ability to act as a positive role model for staff  Strong planning, time management and organisational skills with the ability to prioritise accordingly.  High level interpersonal, written, presentation and negotiation skills including ability to liaise and negotiate with clinical staff, facility managers, allied health professionals and Government representatives.  Excellent people management and interpersonal skills to ensure the achievement of quality outcomes  Well developed communication, problem solving and conflict resolution skills  Sound computer skills in Microsoft Office suite – intermediate level  Current unrestricted full Victorian Drivers Licence  Eligibility to work in Aged Care in accordance with legislative requirements  Demonstrated ability and willingness to be flexible in undertaking duties at different sites as required

Desired  Tertiary qualifications in gerontology or a related discipline  Post graduate qualifications in a related Nursing discipline.  Middle management experience in a health care setting  Membership in industry reference groups, committees and/or associations

Physical Requirements

Doutta Galla Aged Care Services Ltd could require staff to work a variety of shifts across a twenty-four hour period. Due to the geographic layout and nature of work employees will be required to undertake a diversity of tasks, which may require various forms of mobility.

Physical Requirements of the Position The following requirements will be needed to carry out the essential elements of the position as outlined in the position description. The frequency descriptions relate to the following time frames:

PHYSICAL FREQUENCY COMMENTS DEMAND Standing and Continuous Standing and walking are major components of the Walking position. They are required for the delivery of care to residents and movement around the facility. It will require the ability to fully use both legs on a variety of surfaces. Sitting Infrequent Sitting is required when carrying out documentation or administration requirements.

Climbing Infrequent Climbing is required when ascending or descending stairs or ladders. It will require the ability to utilise both legs and feet and/or hands and arms. Body Balancing Continuous agility is required.

Balancing is required to maintain body equilibrium to prevent falling when walking, standing, crouching Lifting and Infrequent and/or lifting whilst carrying out resident care and job carrying requirements.

Lifting and carrying is required in assisting and transferring of residents and in the movement of objects around the facility including movement from Frequent lower to higher and/or horizontally and/or place to Pushing and place. It will require the ability to fully use both arms Pulling and hands.

Infrequent Pushing and pulling are required to draw, drag, push or Bending and tug objects around the facility including but not limited Crouching to wheelchairs, meal and linen trolleys, and also in the delivery of care to residents. Infrequent Kneeling Bending at waist level in the delivery of resident care, Frequent and whilst carrying out job requirements in the facility. Reaching and Ability to crouch to floor level when required. stretching Kneeling can be required in the delivery of resident Infrequent care. Twisting Reaching and Stretching is required in the delivery of resident care and in carrying out administration duties, Frequent and in the movement of objects in the facility. Grasping/Finge r Movement Twisting may be required in the delivery of resident care and in the movement of objects in the facility. The ability to reach in all directions and to twist at the Continuous waist is required. Handling and Feeling Grasping and finger movement (pinching, picking, typing) is required to holding onto objects and in the delivery of care to residents in the facility. It will require the ability to do repetitive motions with hands, Continuous wrists and fingers. Talking Handing and feeling are required in the delivery of resident care, finger dexterity and hand –eye coordination are required, along with full hand and wrist movement, also the ability to perceive attributes Continuous of objects or residents by touching with skin, Hearing particularly that of the fingertips.

Required for the delivery of resident care and job requirements. An excellent understanding of the Continuous English language is required in the delivery of care to Vision residents and when dealing with allied health professionals and other facility staff. Also the ability to communicate effectively.

Ability to maintain hearing acuity, with correction, if required, and the ability to hear and understand whispered conversations at a distance of Frequent up to 1 meter.
