Twice a Year Home Visit Forms Please send a copy twice a year (required in fall and spring). Parent Teacher Conference Information Please send a copy twice a year (required in fall and spring). DECA Katharine will work with classrooms to collect a DECA form for all Head Start children in fall and in spring. And FYI… Checklists: Health & Safety, Meal A Head Start staff person will visit your classroom Monitoring, Creative Curriculum, Playground several times per year to complete our monitoring Safety, etc. checklists. We will send you a report with our observations. Staff may need to ask you questions to complete the forms. Assessment Data Head Start requires that teachers complete assessments at 3 checkpoints. You do this via GOLD. We will pull assessment data for EHS and HS children. Please send any assessment information you share with parents. Inventory/Inventory Transfer Emily will visit classrooms in the fall and spring to compete an inventory of big items purchased by Head Start. If you give HS items to another HS site, please let us know by completing Head Start Inventory Transfer form. Health Screenings (health, dental, Head Start requires many screenings. FES staff will developmental, social emotional, etc) work with families to complete these screenings to meet our deadlines.

Staff may ask you on occasion to remind a parent or send a form home (and a form may come to your classroom). *Note: Head Start families do sign an interagency release form so we can share information.
