CUPE Local 15 - Health and Safety at the VSB

What forms do we need to complete? If a CUPE Local 15 VSB employee is injured at your site, there are several forms that should be completed.

1. All Injuries: First Aid Record Book No matter what the cause of your injury, your first job is to notify one of your site’s first aid attendants and fill out a page in the first aid record book. This shows the time and date of your injury and is signed by the first aid attendant. You may think you were lucky that your slip and fall didn't cause any injuries, but several days later symptoms might show up and you may need to see a doctor. This record gives you the proof you need to proceed with a WCB claim.

2. If you see a doctor or miss work – WCB Forms If you end up seeing a medical doctor about this situation, let your work site first aid attendant know, and they will give you a WCB “Worker's Report of Injury” form to complete. They will send the form to Health and Safety at the VSB, who in turn will forward it to WCB. The first aid attendant will also complete a WCB “First Aid Report of Injury”.

3. When do you need a Violent Incident Report? If a student (or a parent or someone who is not a School Board employee) was also involved in your injury, you need to fill out a “Violent Incident Report”. Please don’t let the name of this form put you off. For example, if you have been scratched or bitten by a student (regardless of whether the act was intended or accidental) you should complete a form for each instance. This form is not just for physical injuries, but also for verbal threats, or if a weapon of any kind was used to threaten you, whether or not you believe the individual could carry out their threats. The District H&S Committee sits down monthly and reviews each Violent Incident Report. As with the accident reports, we do not see the employee’s name or site location, or even the student’s name; however, we are able to see how many Violent Incident Reports involving that student have been submitted, and we are able to follow up with Learning Services to make sure both employee and student are receiving help.

4. When do you need an Employee Safety Plan Form? If you work with a student who has, or could potentially cause injury, it is important that you speak with your administrator so a written “Employee Safety Plan” can be put in place. This is not a student safety plan or behavior plan. It is a separate document that is put together by a team to plan how you can do your job in a safe and injury free environment. Depending on your unique situation, it could cover everything from safe lifting to protecting yourself from such things as spitting, biting, or hitting. Every worker (teacher, SSSA, or office worker) who feels they need a plan of action can have an Employee Safety Plan. If you have submitted a violent incident report, it is likely that there now should be an Employee Safety Plan either put into place or updated. A brief summary of this plan should also be made available to your EOC to give them guidelines on how to deal safety with the students in your charge.

5. When do you need a Workplace Bullying or Harassment form? If you believe you are being bullied or harassed at work you need to take appropriate steps to report it, which is an obligation of all workers who are the targets of workplace bullying and harassment. Forms are available on the VSB Website, from your administrator, your supervisor, office staff (if you are comfortable requesting one from them), or you can contact H&S, or email me directly and I will email you a copy.

Also, if you are a witness of what you believe is another member being bullied or harassed at work, you are to report it.
