FUNCTION: Performs the duties and responsibilities of a community health nurse functioning in an expanded role by combining direct nursing service with leadership responsibilities designed to assist the PHV Clinical Manager in the development, operation, and evaluation of the Diabetes Program. Responsible for staff, agency, team based and patient diabetes education.

REPORTS TO: Director of Special Programs

RELATIONSHIPS: Other Agency CDE’s; PHV staff and Area Managers; patients/families; Administrators; Patient Services Secretaries; Clinical Specialist; Community Service Agencies; Physicians, Vendors, Dieticians.

HOURS: 40 hours/week

MANAGES/COACHES: LPN’s, HHA’s, PCA’s

RESPONSIBILITIES:

1. Provides direct nursing care to the complex diabetic population with a minimum visit expectation of 3.0 visits per day.

2. Participates in orientation of new agency and program staff by conducting orientation and inservice education classes; providing observation field visits; co-visiting to assess and demonstrate clinical knowledge; assisting in case management; performing skill training; participating in evaluation process.

3. Participates in the ongoing development of the Diabetes program through quality management activities such as identification of potential or present case management problems, review of clinical documentation, consultation, policy and procedure development.

4. Participates in the development of patient/family teaching and documentation tools for the Diabetes Program.

5. Provides leadership in the development of staff/agency diabetes education and provides ongoing education to PHV staff and the diabetes core

6. Provides support to off shift teams (weekends, evenings) to meet education/co-visits needs and potential patient needs.

7. Assumes responsibility for his/her continuing education using formal and informal resources which contribute to professional self growth.

8. Applies and interprets current community health practices and Agency Diabetes Program policies and procedures to PHV Staff as they relate to diabetic care of home care patients.

9. Participates in professional, community, and interagency activities that involve the delivery of diabetic care; represent the Agency through participation o committees, conferences, and community activities.

QUALIFICATIONS:

1. Completion of an accredited Registered Nursing Program.

2. Possession of a current RN license to practice in New York State.

3. Possession of a current American Association Diabetes Education Certification.

4. Minimum of two years experience in Diabetes teaching and community health nursing preferred.

5. Computer proficiency with WORD, Power Point, Excel strongly preferred

5. Ability to instruct individuals and groups in formal and informal settings.

6. Ability to work with people from different socio-cultural backgrounds and/or standards of living in an effective manner.

7. Ability to communicate effectively verbally and in writing.

8. Ability to incorporate new ideas or concepts into nursing practice.

9. Ability to work with community agencies as appropriate.
