Developmental Observation Form- Preschool

Observe and/or interview a preschool child. Ages may be 3 years to 5 years old. You will staple your observation notes to this (typed) form. Developmental Characteristics Description/Comments

1. Social and Cultural Context Name of the child, age and gender, family characteristics: Social and Cultural context (Urie Bronfenbrenner’s Model), Siblings and their ages, cultural background and socioeconomic status

2. Developmental Characteristics Depending of the age of the child (What fine vs. gross motor skills can the child demonstrate?)

3. Physical Appearance / Biological Domain Describe the child’s physical characteristics. Child’s approximate height and weight

4. Child’s Nutritional habits and eating habits Recommendation observe a meal, if it is not possible, interview parent about eating habits

5. Child’s Movement and Activities

What level of Parten’s play does the child display? Examples. Developmental Characteristics Description/Comments 6. Cognitive Domain According to Jean Piaget’s Cognitive Development Theory, which stage would you place the child? Why? Give one example (i.e. – conservation, seriation, decentration, animism, artificialism, egocentricity).

7. Social and Emotional Domain Describe how child/ communicates gender roles & emotions. How

does the child relate and interact with other people? Provide two examples.

At the end of observation, with your partner collect notes and attach to this completed form. You are expected to present a 5 to 10 minute oral presentation in class. Please provide a visual aid to enhance your oral presentation. For the presentation Choose at least 3 of the areas (from # 1 to # 7 above) you observed to report to the audience (presentations on 5/28/15).
