The 10th and Mission Family Housing Program consists of 136 affordable apartments for low-income families and disabled adults, of which 44 are referred by Human Services Agency through the Local Operating Subsidy program for formerly homeless families with multi-diagnoses.

Catholic Charities provides a multitude of on-site and integrated services to this community in conjunction with Mercy Housing, the onsite property manager. These services assist families in their progress towards stability: residential, financial and psycho-social. The services include on-site case management, and active resident services and activities program, crisis intervention and a host of family-specific services. The support services team offers linkage to mainstream benefits, coordination of varied wrap around support services and direct coordination with property management to ensure housing security.

The Bilingual Case Manager provide crisis intervention, counseling services, and referrals as needed for residents, with a particular emphasis on immediate needs such as budget and financial assistance, transportation, Mental and Physical healthcare and children’s services. The position works dih on-site property management Mercy Housing to resolve any client issues related to housing stability and acts as a liaison to outside community service providers.

 Provides case management services for up to 55 families and their children.  Services include, but are not limited to, intake assessments, crisis intervention, counseling and support services, information and referral, family budget development, issuing of vouchers and coordination of services so the immediate needs of the family are addressed.  Provide ongoing support services by developing and monitoring a case plan with each family.  Coordinate and supervise program volunteers.  Maintain continuity of services by coordinating with new and existing service providers.  Assist families in accessing appropriate resources and take appropriate actions relative to pre-vocational training, job development/placement, housing stability, and childcare.  Maintain close communication with other family service providers involved with each family.  Design and implement meaningful community building activities for all building residents.  Participate in related program, organization and community meetings as assigned.  Collaborate with outside agencies  Maintain proper client and program documentation.  Work in collaboration with case management team members and Support Services Manager in the design and implementation of program operations and policies.  Ability to be flexible with schedule including some late evenings.  Represent the program and organization at outside meetings as needed.

Knowledge, Skills & Abilities:

 MANDARIN/CANTONESE LANGUAGE FLUENCY IS REQUIRED  Knowledge of community resources for population  Strong knowledge of substance abuse and mental health issues and treatment models, including Harm Reduction  Ability to assess emergency situations and react accordingly by accessing appropriate medical/social systems.  Ability to prioritize tasks with strong organizational skills  Knowledge of San Francisco County social services networks  Ability to design systems and processes to track data and monitor progress  Functional knowledge of Microsoft Office Products  Knowledge of mandatory reporting requirements for people working with ageing adults  Knowledge of issues facing homeless individuals  Ability to communicate well in spoken and written English.  Demonstrates the necessary attitudes, knowledge and skills to deliver culturally competent services and work effectively in cross-cultural situations. If driving a car is required for the position, incumbent must have a valid California driver's license and be able to provide proof of DMV record and personal insurance (if required.)
