 Commuting Survey 2015 

Dear Students, Staff and Faculty: As part of our commitment to reduce our carbon footprint, improve health & wellness, and relieve traffic & parking pressures, the University of Louisville is continuing to refine our efforts to reduce the number of vehicle miles traveled to and from campus. You have been randomly selected to participate in our Sustainability Council’s latest survey of current commuting habits, barriers to change, and willingness to consider the many transportation alternatives that we offer. Your participation is voluntary and your responses will be confidential. The survey should take no longer than 30 minutes to complete. As our way of thanking you for your time, all those who complete the survey through to the end will be automatically entered into a drawing for one of the following sustainability prizes: 1. A $400 voucher to a Louisville bicycle shop; 2. A pair of season tickets to UofL Theatre Arts' 2015-16 mainstage performance season or 3. One of four $25 gift certificates to UofL’s Farmers’ Market Your response is required by October 14, 2015, when the survey will close. To participate, you must be 18 years of age or older and currently employed or enrolled at UofL. Participation is voluntary. Your decision not to participate or fully complete the survey will not affect any benefits to which you may be entitled from UofL. UofL Human Subjects guidelines guarantee your responses will be kept confidential. The information collected may not benefit you directly; however, the information learned may be helpful to others. At the end of the survey, you will see a debriefing statement with contact information for the investigator, and campus and community counseling services.
