Instructions for Completion of the Proposed Change/Directed Change Order Summary Sheet

INSTRUCTIONS FOR COMPLETION OF THE PROPOSED CHANGE/DIRECTED CHANGE ORDER SUMMARY SHEET AND TAKE OFF SHEET FOR CONSTRUCTION CHANGES

To enable Sandia to properly evaluate your quotation for this proposed change, please complete the attached forms entitled: Proposed Change/Directed Change Order Summary Sheet and the Take Off Sheet for Construction Change.”

Complete this form as follows:

1. Enter your firm’s name in the space indicated at the top of both the Proposed Change/Directed Change Order Summary Sheet and the Take Off Sheet.

2. List the types of labor necessary as a result of this proposed change. Give estimated hours for each labor category per unit, the total hours, and the hourly direct labor cost (this is the gross amount the worker is actually paid). Multiply the rates by the total hours to calculate the extension for each labor category.

3. Total the direct labor extension and multiply the sum by your firm’s Labor burden factor. The labor burden factor includes the costs of payroll taxes and benefits that the employer pays, but does NOT include profit.

4. On the Proposed Change/Directed Change Order Summary Sheet, add the direct labor and the labor burden for the total labor cost.

5. On the Take Off Sheet, list all the materials necessary for the Proposed change, give unit quantities and unit costs. Multiply unit quantities by unit cost to calculate material extension.

6. On the Proposed Change/Directed Change Order Summary Sheet, sum the extended prices of the materials listed.

7. Give the name of and the price quoted by each subcontractor. Each Subcontractor must complete a Take Off Sheet for Construction Changes form, and fill in the Individual Subcontractor Summary (on the Proposed Change/Directed Change Order Summary Sheet) regardless of tier.

8. Sum the total Labor, Material and Subcontract(s).

9. Multiply the sum arrived at in step 8 by your firm’s overhead rate, and enter in the space provided

10. Enter the amount of the profit that is being requested in the appropriate space.

11. If your firm is required to submit Payment and Performance Bonds to Sandia for this Contract, enter the rate you firm is charged for those bonds and the amount that will be paid or refunded as a result of this change. 12. Sum the amounts computed for Steps 8, 9, 10, and 11. This is the total amount of your quotation for this Change.

If your firm used a different accounting method than that which is described above, you may use your firm’s method only if it provides an equivalent level of detail and is acceptable to the SCR. PROPOSED CHANGE/DIRECTED CHANGE ORDER SUMMARY SHEET
