5 About MPCA Promoting, supporting, and developing comprehensive, WHO WE ARE, WHAT WE DO accessible, and affordable community-based health Michigan Primary Care Association care services to everyone in Michigan. - MPCA Mission (MPCA) is a leader in building a healthy society in which all residents have convenient and affordable access to quality health care. Our mission is to promote, support, and develop comprehensive, accessible, and affordable community-based primary health care services to everyone in Michigan.

For over 30 years, MPCA has been the voice for Health Centers and other community- based providers in Michigan. MPCA began in the late 1970s as a networking organization of Health Centers and stakeholders interested in fostering primary care in rural underserved communities across the state. With the realization that urban communities also had great health care needs, the Association quickly broadened its scope to include urban Michigan communities. Today, 40 Michigan Health Centers provide quality, affordable, comprehensive primary and preventive health care for more than 615,000 Michiganders annually at over 250 sites across the state. Information about Michigan Health Centers begins on page 16. MEMBERSHIP MPCA is a membership association representing Health Centers and other community-based providers in Michigan. We offer three types of membership—Active, Associate, and Student.

Active membership is open to providers of comprehensive primary care that are community-based organizations and support MPCA’s mission. Associate membership is open to organizations that do not provide primary care and that support the mission of MPCA. Student membership is available to any student enrolled in a secondary or post-secondary Michigan institution. Visit www.mpca.net for more information. OUR SERVICES Clinical Support & Quality Improvement A core focus of MPCA’s Clinical Services Department is to support the delivery of quality health care services in Michigan through training and support in: integrated care inclusive of primary care, dental and behavioral health, Patient-Centered Medical Home recognition and sustainability, meaningful use, data driven improvement processes, informatics, preventive services, evidence-based chronic disease management, and infectious disease and immunization health care initiatives.

Communications MPCA communicates both externally and internally. Our external focus is on creating awareness of Michigan Health Centers and their services while our internal focus is on information sharing, providing communication support and educational opportunities to our members, as well as providing support for MPCA departments. Our communication

6 toolkit includes press releases, flyers, brochures, yearly publications, advertising, sponsorships, website, social media, and electronic communications.

Health Center Growth With the goal of 100% access to health care in Michigan and zero health disparities, MPCA assists Health Centers and communities in determining the best approach to providing access to care. MPCA supports assessment of assets and needs, the expansion of health care delivery capacity and efforts to strengthen safety net providers, and encourages collaboration.

Data Analysis MPCA analyzes a variety of data to highlight the need for redistributing the supply of primary care, dental health, and mental health professionals to areas with the highest need; support proposals presented to grantmakers, policymakers, legislators, and partner organizations; identify trends and interventions to improve the health of underserved communities and vulnerable populations.

Education & Training MPCA hosts education and training events throughout the year to strengthen and improve the ability of Michigan Health Centers to deliver high-quality health care to medically underserved communities. MPCA’s education and training programs are developed in response to Health Center needs and interests and are a mix of in-person, web-based, and peer-networking opportunities.

Operational Support MPCA provides direct support to Michigan Health Centers in the areas of billing and finance, emergency management, service delivery integration and excellence, compliance with program requirements, board governance, and many others. We support the sharing of best practices through numerous Peer Communication Networks which serve as a forum for staff and board members from Health Centers statewide to come together to share, learn, and improve the quality of Michigan Health Centers.

Enabling Services MPCA provides training and peer-learning opportunities, offers technical assistance, and curates resources across a wide spectrum of enabling services, including supports provided by community health workers in a patient care team. MPCA’s outreach, education, and enrollment services equip Michigan Health Centers and other partners in providing community-based assistance for patients and community members to gain and maintain access to comprehensive, affordable health coverage through Medicaid, MIChild, the Healthy Michigan Plan, and health plans offered in the Health Insurance Marketplace.

Policy & Advocacy MPCA works with policymakers and elected officials, advocating for polices that assure all Michigan residents have access to quality, affordable primary and preventive health care within their communities. Each year MPCA identifies policy priorities that will improve the ability of Health Centers to provide quality health care services to all in need, and monitors proposed policies that may affect Health Centers and access to care. MPCA works closely with members, educating them on the policy and legislative issued affecting Health Centers and the importance of grassroots advocacy.

Special Populations MPCA works with members and partner agencies to better understand and respond to the needs of Michigan’s homeless individuals and families, residents living with HIV, LGBTQ, veterans, and migrant agricultural workers.

VirtualCHC VirtualCHC provides technology-based solutions for common needs of Health Centers. It offers access to hosted applications such as electronic health and dental records, general ledger, population health tools, and more—all over the Internet. VirtualCHC also provides a powerful custom reporting solution, including working with Health Center staff on analytics and report development. Consulting services are available and include anything from assisting with vendor selection to local IT support. VirtualCHC serves as a Health Center’s Health Information Technology department.

Workforce Development MPCA places a strong emphasis on developing and monitoring workforce development initiatives to increase the effectiveness of health professional recruitment and retention programs for MPCA and Michigan Health Centers. BOARD OF DIRECTORS MPCA is guided by an 11-member Board of Directors comprised of representatives from seven regions, the Clinical Services Committee, Health Center Board Member Committee, and the Chief Financial Officer Network. The Executive Committee consists of board president, president-elect, treasurer, secretary, and the Health Center Board Member Committee chair. 11 The fundamental principles on which Health Centers were established 50 years ago set them apart from other providers of health care:

u Located in or serve medically underserved areas or populations u Governed by a community board composed of 51 percent or more of Health Center patients who represent the population served u Provide comprehensive primary health care services as well as support services (e.g. education, translation, transportation) that promote access to health care u Services available to all with fees adjusted based on ability to pay u Meet strict performance and accountability requirements regarding administrative, clinical, and financial operations as established by the federal government

Michigan Health Centers are part of a nationwide network of 1,300 Health Centers spread across 50 states and all U.S. territories that provide care for 24 million people in over 9,000 communities.

12 About HEALTH CENTERS “Health Center” is used to refer to Health Of Michigan’s 40 Health Centers, 37 are Health Center Program grantees, 1 is an FQHC Look- Center Program grantees and Federally Alike and 2 are both a Health Center Program Qualified Health Center Look-Alikes grantee and a FQHC Look-Alike.

Each Health Center’s staffing models, facilities, scope of services, and approaches are tailored to meet the unique needs of its patients and surrounding communities. Many individuals have difficulty accessing health care because of where they live, the language they speak, and their complex health care needs. Health Centers provide culturally appropriate health care close to where patients live, at convenient times, and in languages patients understand.

Health Centers have professionally trained health care teams. These teams may include board- certified or board-eligible physicians, nurses, nurse practitioners, physician assistants, certified nurse midwives, medical assistants, mental health and substance abuse professionals, dentists, dental hygienists, pharmacists and many other team members. The clinicians of each team work together to provide a core of primary and preventive health services to individuals and families from pediatric through geriatric.

Four types of Health Centers as described below and on page 14 are funded under Section 330 of the Public Health Service Act. These Health Centers are also called Federally Qualified Health Centers (FQHCs) and they must meet the following requirements: u Be located in or serve a Medically Underserved Area/Population (MUA or MUP) as designated by the Health Resources and Services Administration u Governed by a community board composed of 51 percent or more of Health Center patients who represent the population served u Provide comprehensive primary health services as well as support services (e.g. education, translation, transportation) that promote access to health care to everyone in the community through from pediatrics to geriatrics u Charge uninsured patients for services on a sliding-fee scale that is based on their family size and income

Community Health Centers // SECTION 330E Community Health Centers were developed in 1965 in response to community need for improved health services for inner city populations. As part of the federal government’s War on Poverty, funding was made available for communities to establish primary care centers to provide comprehensive health services, regardless of ability to pay. Although there have been many changes in the Health Center Program over the years, its foundation remains the same—to provide high-quality primary and preventive health care to people in rural and urban medically underserved areas. Medical underservice is defined by a shortage of professionals or health services and high rates of poverty and infant mortality. It may also include special considerations, such as lack of Medicaid providers, lack of bilingual services, or a large percentage of uninsured persons. 13 Migrant Health Centers // SECTION 330G Migrant Health Centers were established in 1962 to provide comprehensive primary care services to migrant and seasonal agricultural workers and their families in a culturally sensitive environment. Like Community Health Centers, Migrant Health Centers provide comprehensive health care services to their target population at an affordable price, regardless of insurance status. Migrant Health Centers use culturally sensitive clinical protocols, bilingual health personnel, and lay outreach workers. In 2014, Michigan Health Centers served 15,151 migrant and seasonal farm workers.

Health Care for the Homeless Health Centers // SECTION 330H Modeled after a demonstration project jointly funded by The Robert Wood Johnson Foundation and Pew Charitable Trust, the Health Care for the Homeless program was initiated in 1987 as part of the Stewart B. McKinney Homeless Assistance Act. In 1996, Congress passed the Health Centers Consolidation Act, which joined the Health Care for the Homeless program to the community, migrant, and public housing primary health care programs under a single authority called the Consolidated Health Center Program. The Health Care for the Homeless program provides federal grants to non-profit organizations to deliver primary health care and substance abuse services to homeless individuals and families. The Health Care for the Homeless program provides a multidisciplinary approach to the delivery of care by combining systems of primary care, substance abuse and mental health services, patient management, and education with aggressive outreach. In 2014, Michigan Health Centers served 25,265 homeless individuals.

Public Housing Health Centers // SECTION 330I The Public Housing Primary Care Program was established to support the provision of accessible and comprehensive preventive and primary health care services to residents of public housing. Health promotion and disease prevention are emphasized, and primary health care services are provided on the premises of public housing developments or at other locations immediately accessible to residents of public housing.

Federally Qualified Health Center Look-Alikes Federally Qualified Health Center (FQHC) Look-Alikes are Health Centers that have been certified by the Centers for Medicare & Medicaid Services (CMS), based on recommendations provided by the Health Resources and Services Administration (HRSA)/Bureau of Primary Health Care (BPHC), as meeting all Health Center Program requirements. This certification makes them, like Health Center Program grantees, eligible for fair reimbursement through Medicaid and Medicare, and for participation in the 340B federal drug pricing program, and assists in recruiting and retaining providers through the National Health Service Corps and the State Loan Repayment Program. FQHC Look-Alikes do not receive funding under the Health Center Program.

14 Indian Health Service The most prominent provider of health services to American Indians is the Indian Health Service (IHS). The IHS provides both direct care and contract care to members of federally recognized tribes on or near federal reservations. The IHS funds three MPCA members—American Indian Health & Family Services of Southeastern Michigan, Inc., Bay Mills Health Center, and Sault Tribe of Chippewa Indians—to provide urban Indians in , American Indians in Chippewa County, and Chippewa Indians in Sault Sainte Marie respectively, with comprehensive health care services, which include medical, community health, dental, and mental health/ substance abuse services. Ryan White Program, Part C Health care organizations funded under the Ryan White Early Intervention Services program provide a comprehensive continuum of outpatient HIV primary care services including HIV counseling, testing, and referral; medical evaluation and clinical care; other primary care services; and referrals to other health services. Community Health Centers, Migrant Health Centers, and Health Care for the Homeless sites funded under Section 330 of the Public Health Service Act may receive Part C grants.

64 Michigan Health Centers Provide Cost Efficient Care

In 2014, a cost-efficiency study was conducted to compare the efficiency of Michigan Health Centers to other primary care providers in providing Medicaid services with a goal to raise awareness that Health Centers save the State money and is a cost-effective model of health care delivery. The study was conducted by the Institute for Health Policy (IHP) at Michigan State University.

The report, An Evaluation of the Cost-Efficiency of Federally Qualified Health Centers (FQHCs) and FQHC Look-Alikes Operating in Michigan, demonstrated Michigan’s Health Centers save the State of Michigan $12 per member per month in State Medicaid expenditures—a savings of $144/year for each adult Medicaid beneficiary. The claims-based (managed care and fee-for- service) study looked at claims from 2011 and 2012.

The study compares the utilization of Medicaid services by adult (18-64 years) Michigan Medicaid beneficiaries who received health care services at a Michigan Health Center during this two-year period with those served by other Medicaid providers. The study also compared the total costs of services provided to two categories of adult Medicaid beneficiaries—FQHC patients and non-FQHC patients.

The results demonstrate Michigan Health Centers are 8% more cost efficient at providing services to adult Medicaid beneficiaries than other primary care providers.

91 What is a Patient-Centered Medical Home?

The Patient-Centered Medical Home (PCMH) is an approach for providing quality, comprehensive, coordinated primary and preventive care to adults, youth, and children. At its heart, this model of care fosters strong relationships with patients and their families to better care for the person as a whole.

PCMHs deliver higher quality care that comprehensively addresses an individual’s needs. The intended outcomes of a high-functioning PCMH Health Center include improved patient outcomes, patient safety, and patient experience. PCMH can potentially lower costs through the delivery of evidence-based patient care, reducing waste and improving efficiencies, increasing delivery of preventive services, and fostering population health management. Many of the PCMH goals rely on improved communication and coordination between and across health care providers and patients, and increased patient engagement through a high- functioning medical team.

The Health Resources and Services Administration (HRSA) and the Bureau of Primary Health Care (BPHC) support Health Center Program grantees in achieving PCMH recognition. More than three-fourths of MPCA members have achieved PCMH recognition and accreditation via the Accreditation Association for Ambulatory Health Care (AAAHC), The Joint Commission (TJC), and the National Committee for Quality Assurance (NCQA). MPCA’s Clinical Quality Team provides leadership, guidance, support, and customized technical assistance to our members; facilitating discussion with peers to share best practices; development of tools and resources; and staying abreast of the latest news and information on PCMH initiatives. 60% of Michigan Health Centers achieved Patient Centered Medical Home status through a national recognition body
