Introduction The topic of this report is what constitutes best social care practice in maintaining the person with dementia in their own home.

Aim of the Search The primary aim of this search is to create a database with all relevant literature on home care for people with dementia.

Methods

Literature Search The search strategy was devised to take into account the broad nature of the review. No limits or filters were used as we were interested in identifying all potentially relevant quantitative or qualitative studies evaluating any services that give non-nursing, social/personal care by salaried carers to people with dementia in their own homes. The search was not restricted by language or publication status (published, unpublished). A 20 year limit was set on the search.

Search strategies were developed specifically for each database. Keywords for “home care” and “dementia” were the relevant controlled terms and text words in each database. We searched the following databases from January 1991 to July 2011 using the search terms and strategy as described in Appendix A.

• MEDLINE (PubMed); • EMBASE (www.embase.com); • PsycINFO

For further assessment and handling, identified references were downloaded to a web-based system (DistillerSR®, www.systematic-review.com). Search results (7867 references) were uploaded in DistillerSR®, each abstract was assessed in order to identify duplicate studies (n = 2732) that were discarded, and a database containing only the studies that need to be assessed for inclusion/exclusion was generated (n = 5135).

Criteria for considering studies for this review

Types of study designs All types of study designs were included, both quantitative and qualitative:

• RCT • Before-after study • Controlled trial (not RCT) • Time series study • Systematic review • Qualitative study • Narrative review • Case report or series • Cohort study • Survey or focus group • Case-control study • Other non-RCT experimental study • Cross-sectional study • Other non-comparative study

Types of participants Paid carers of people with dementia. Studies about informal care givers were not included in the database.

Types of homecare The types of home care that were included were domiciliary care, home care, house care, mixed care and personalised care.

Types of dementia All types of dementia were considered including:

• Alzheimer disease • Multi-infarct dementia • Vascular dementia • Huntingdon’s disease • Creutzfeldt – Jakob disease • Wernicke encephalopathy • Kluter-Bucy disease • Korsakoff syndrome • Lewy body disease • Other not specified • Niemann-Pick disease

Results

Search results The database searches retrieved 7867 references. After removing duplicates, 5135 articles were selected for three levels of screening. All levels of screening were performed independently by two reviewers (KSW, NM, or DP). Details of each screening level are provided in Description of Included Studies

There are 264 included studies in the database based on the abstracts of the papers. Forty two studies specified the type of dementia, 38 were about issues related to care givers of people with dementia and 184 did not specify the type of dementia.

Twenty six of the studies were experimental, 42 were reviews, 149 were observational studies, and in 47 it was not possible to identify the study design from the abstract.

The studies were grouped in the database according to the type of home care. There are 14 studies in which the abstract refers to domiciliary care, 116 on home care, two about house care and 11 on mixed care. No studies were found that were about personalised care. In 43 studies it was not possible to tell the type of home care described in the study, although the abstract was available; in a further 77 studies the abstract was not available and it was unclear from the title whether these could be excluded.

The references are listed in Appendices B to G.

Excluded Studies Seven hundred and thirty six (736) references were excluded in the third screening. Appendix H details the excluded references. Twenty one of the references were excluded because they were not specifically about dementia; five were excluded because pertained to dementia but other types of care not in the inclusion criteria; three references were duplicate references; three references had a financial focus; one reference was about researching care; and one was a Cochrane Review that had been withdrawn as it was not complete.
