23rd ANNUAL REPORT 2016-17 About the organization This logo symbolizes the objectives of the organization. Ÿ The words in the outer circle are from the great Indian epic "Mahabharat", saying that "nothing is above a Human". This is also the motto of the organization. Ÿ The triangle in the inner circle symbolizes the hands of three people and stands for community development through participation. Ÿ The light from the lamp in the small hut in the centre symbolizes the development of the weakest and poorest person of the community. Founder trustees of the organization were inspired by Gandhian thinking and work of great men like Albert Schweitzer. They felt deeply the agony and hopelessness of poor villagers. They saw the plight of villagers and felt a need of medical services in these villages. Hence they brought like minded friends together and founded Gram Seva Trust, an organization dedicated to rural health and development. In 1994 the trust started a 30 bedded hospital with 5 staff members in an old dilapidated building, given by another trust. As the need arose the hospital was expanded to accommodate more patients and better services. Today the hospital can accommodate 80 patients and has all basic facilities required in a rural hospital providing health services at affordable rates and sometimes free of charge to the needy from nearly 200 surrounding villages of and Dang districts. The organization also wanted to improve health of the surrounding villages hence as and when need was identified different community projects were started in the surrounding villages with main focus on health and development of women and children. Immense efforts are put in to decrease malnutrition. Gram Seva Trust also reaches out to the tribals in deep interior villages of and Dang through satellite centers and general medical mega camps arranged every year. The organization has also started different projects for development of villages like Self Help Group project (a micro saving and micro lending project for women), trainings for income generation activities, adolescent girl project, tutorial classes to strengthen primary education and a Child Education and Development Centre to improve standard of education in children of landless labourers. Gram Seva grieves the sad demise of Shri Chaganbhai Naik (Trustee and Donor). Chagankaka has been a strong pillar of the organization. He has always strived for the betterment of the people of Kharel and Gandeva and has done many projects for their development. Chagankaka and his family also donated their ancestral land for construction of Parodh Balawas. We pay our homage to him and hope to continue his legacy and work for the surrounding poor and tribal communities as a tribute to him. Editorial ello friends, HWish you all a very Happy New year! Gram Seva has completed 23 years. Next year the organization shall be entering the silver jubilee year. Your support and well wishes have helped us in our work over these years. We are happy to present before you our annual report of activities. Gram Seva has continued its health and development projects in interior villages of Navsari and Dang districts. You may be aware of some of these activities through our newsletter “ Samvedan”. This year we have started dispensing generic medicines free of cost to all our OPD patients. We also conducted 3 mega health camps in interior areas of Dang and Vansda with the support of our donors. Free eye camps are being conducted regularly in the hospital. Parodh Balawas construction has been completed and is functioning well this year. State Bank of has donated a mobile medical van which has been very useful in our outreach health projects in interior villages. The sad demise of Shri Chaganbhai Naik this year, our Trustee and big supporter of our activities, has been a great loss to Gram Seva family. Shri Chaganbhai has been our guide and support and has contributed a lot in the development of the organization. Our experience of 23 years shows that substantial work is required in health, awareness, sanitation, education and economic empowerment in rural areas. We hope to contribute as much as we can in these activities in the community, with your support and well wishes. And at the same time continue with our present work. This was bitten by a snake while cutting grass for his cattle. Seeing this condition of the patients it was decided to waive of the charges for medicines for all patients coming to the OPD. We have started giving generic medicines free of cost to OPD patients. Every day nearly 250 patients visit the OPD and about Rs.1 Lac per month are spent on their medicines. This expense is reimbursed through donation by our friends and donors. We have also kept a donation box in the OPD so that patients, as per their capacity, can also donate some money for these medicines. But we see lack of enthusiasm in patients and no donations are generated from this box. Hesitation and lack of awareness by family could have cost Heena her life. Second time pregnant, 21 year old Heena (name changed) was a resident of Chondha an interior village of Vansda. She usually went to State Hospital, Dharampur for her antenatal services. One day Heena suffered from severe headache. Her legs and body were also swollen. She was immediately taken to Dharampur where she also suffered from convulsions. She was given medical treatment and advised immediate surgery. But her family members were not co-operative and brought her home. At home she again started convulsing so she was taken to Mankunia PHC which is not equipped to handle such emergencies. The medical officer referred Heena to Vansda cottage hospital. Seeing her serious condition she was immediately referred to Gram Seva hospital at Kharel about 65 kms away from her home. At Kharel hospital Heena was taken for emergency operation. Due to her serious condition this operation was full of risks but both Heena and her baby were saved. Importance of advocacy in community participation Bodwank nava falia is a hamlet of farm labourers. Since one year, we have started a tutorial class for the children of these farm labourers, to strengthen their basic education and prevent school drop out rate. But we found that the attendance of students from std. 1 to 4 in this class was always minimal. One or two meetings were conducted with parents but there was no improvement. We could not understand the reason for their non compliance. After a lot of brain storming we felt that, maybe the parents do not understand the importance of education and what we are doing in the tutorial classes. So we decided to do one to one counseling of parents. Our workers visited each and every child’s house and talked with his/her parents personally. They were told the importance of education and also informed about different activities done in tutorial classes for overall development of the child. Slowly the attendance has improved and now many parents personally bring their children to the class. They have also agreed to meet once a month to discuss the education of their children. We learnt that it is important to inform the community about the value of some services and generate demand for them.

Community Social Responsibility (CSR) by our health worker Ramilaben Rameshbhai Patel from Endhal village works as a tutor in our tutorial class. She also volunteers as a community health worker in our mother and child project since last eight years. Ramilaben has passed her 12th std and comes from lower income family. During her tenure as our field worker she has been trained in health and education. This has helped her understand the importance of education for any person and she tries to impart this knowledge to her community. One of Ramilaben’s student Jigar stopped coming for tutorial classes. On enquiry she was informed that his parents had migrated to Surat for work and Jigar also went with them. When Ramila went to his school she was informed that he had not taken his school leaving certificate. Hence she called his parents who told her that since the school in Surat was far away Jigar was not attending any school. Ramila felt sad that Jigar was losing his schooling and education. She spoke to her family of seven people and Jigar’s family and convinced them to keep him with her and continue his education. Thus for the last one and half year Jigar is staying with Ramilaben in her small house of two rooms, as her family member. Ramila is truly doing her Community Social responsibility (CSR)

Positive attitude and Result become Motivation for a village woman Gram Seva has been selected to conduct basic trainings for Sakhi mandals( micro credit groups of women) in villages of Gandevi and Chikhli Taluka. We encourage the group women to prepare tea and snacks for these trainings so that they earn some money. But nobody was ready to prepare tea and snacks in Navatalav village. On further enquiry it was found that these women took some rest and also watched TV serials in the afternoons when the trainings were scheduled. Hence they were not ready to prepare snacks. On further counseling and cajoling one person Apexaben agreed to prepare tea and snacks. She earned Rs.4575 in five days. This encouraged her and she agreed to prepare snacks for another five days training session. Thus she earned Rs 8675 in 10 days. This not only helped her earn additional money for her family but also increased her self confidence. Thus positive attitude helps in implementing activities, solving problems and motivation. focus on marketing, product research and development -compost. and packaging of the product. Sakhi Mandal training Project Gram Seva was selected to conduct trainings of village Vermi-compost Production( Organic level women’s micro credit groups ( Sakhi mandals) in fertilizer produced by earthworms) Chikhli and Gandevi blocks of Navsari District. About Women in the surrounding villages are associated with 1500 women from 87 of Sakhi mandals have been trained and are more comfortable doing farming activities. Hence till date. During these trainings we realised that women we try to help them increase their income by training wanted to contribute in the development of their family them in such activities. This year we trained women in but did not have the knowledge of how and what to do vermi- compost production. Out of which 46 women and and also lacked support. We feel that in the coming years 11 men are at present producing vermi - compost. H. No. 8, At & Post: Kharel, Dist: Navsari 396430] as on 31st March 2017 as well as Income and Expenditure Account for the year ended on 31st March 2017. We have conducted our audit in accordance with auditing standards generally accepted in India. These standards require that we plan and perform the audit to obtain reasonable assurance about whether the financial statements are free of material misstatement. 1. The accounts are maintained regulAl]y and in accordance with the provisions of the Act and Rules 2. The receipts and disbursements are properly and correctly shown in the accounts 3. The cash balance and vouchers in the custody of the treasurer or trustee on the date of the audit were in agreement with the books of account 4. All books, deeds, accounts, vouchers or other documents & records required by us were produced before us. 5. A Register of movable and immovable properties has been maintained and the changes therein have been furnished to us. 6. All the necessary information required by us has been furnished to us by the Trustees personally whenever called upon 7. No property or funds of the Trust were applied for any object or purpose other than for the object or purpose of the Trust 8. There are no amounts which are outstanding for more then one year and written off 9. For repairs, tenders are not invited but for construction involving expenditure exceeding ` 5000/-, tenders are invited 10. The money of the trust have not been invested contrary to the provisions of section 35 of the Act. 11. There is no alienation of the immovable property contrary to provisions of Section 36 12. There are no cases of irregular, illegal or imporper expenditure or failure or omission to recover money or other property belonging to the public trust or of loss or waste of money or other property thereof. 13. The provision of Budget to be filed under Rule in the form under Rule 16A is not applicable to the trust. However, the Trust has policy to perpare the Annual Budget. 14. The maximum and minimum number of trustees is maintained during the year. 15. The meetings are held as provided by such instrument. 16. The Minutes Books of proceedings of the meetings are maintained. 17. None of the Trustees has any interest in the investment of the trust. 18. None of the trustees is any debtor of creditor of the trust. 19. The irregularities pointed out by the Auditors in the accounts for the year under audit are have been duly complied with by the trustees. 20. The activities of the organization depend on donations and hence there is a perennial need of donations and help. We sincerely request you to join us in these activities through your voluntary donations in cash or kind for the following: Permanent Donations Scheme 1. Poor Patients Relief Fund. 2. Children’s Welfare Fund 3. Bhojnalay( Kitchen) Funds 4. Parodh Balawas Funds 5. Anniversary Donations: these are donations on birth, death or any other anniversaries for treatment or food for the patients. The interest on the amount shall be utilised on the same day every year for the same purpose. The Donors name shall be displayed on a board that day. 6. One Meal for Hospital Patients- `. 1100/- (20 U.S.$) 7. One Meal for “Parodh” Balawas children- `. 1100/- (20 U.S.$) 8. Eye Camp- An eye camp and free cataract operations of all those coming in the camp are organised on donation of `. 40000/- ( 800 U.S.$) and `. 1500/- (25 U.S.$) for one cataract operation. 9.
