IT IS MY PLEASURE TO PRESENT TO YOU THE HARRIS COUNTY HOSPITAL DISTRICT FOUNDATION (HCHD FOUNDATION) 2017 ANNUAL REPORT AS WE CLOSE OUT THE 24TH YEAR AND ENTER THE 25TH YEAR OF THE FOUNDATION.

The HCHD Foundation was created to support the Harris Health System (HH). The HCHD Foundation makes grants to HH that focus on improving the lives of Harris Health patients through the incredible work Harris Health does every day through the 3 HH hospitals: Ben Taub Hospital, Lyndon B. Johnson Hospital and Quentin Mease Hospital, 19 community health centers, 6 same day clinics, 5 school based clinics and 5 specialty centers. Harris Health continues its excellent health care delivery with its partners, Baylor College of Medicine and UT Medical School. This 2017 Annual Report highlights some of the HCHD Foundation’s accomplishments. The Foundation is very grateful to our philanthropic partners – individuals, corporations and foundations – who have supported us so we in turn can grant funds to HH to be used within their health care system to assist those in our community that are uninsured and underinsured. 2016-2017 saw strong participation in several signature fundraising events: The 2017 Texas Med Run was supported by close to 1,000 walkers/runners generating improved contributions to Harris Health and our SCRUBS young Professional group‘s annual Cinco de Bingo event was supported by more young people than ever before. The nursing staff continued to benefit from education funding provided by the Foundation and the Small Grants Fund continued to support Innovative ideas presented to the committee by Harris Health employees. The new effort in Crowdfunding, the online fundraising app, raised funds for the Child Life CAPES program. We look forward to the next fiscal year which will be very exciting and will focus on the 25th Anniversary of the Foundation, the upcoming 2017 Jubilee of Caring which will honor Barbara and Corby Robertson with the Ben Taub Humanitarian Award. All proceeds of the 2017 Jubilee of Caring will go to the first floor renovation of the Harris Health Ben Taub Hospital Level I Trauma Center (Ginni and Richard Mithoff Trauma Center) thanks to the efforts of the 2017 Jubilee of Caring co-chairs: Carroll Ray, Beth Robertson and Lillie Robertson. In addition, the Foundation anticipates launching its 2nd Capital Campaign which is anticipated to raise $25 million and will also benefit the first floor renovation of the Harris Health Ben Taub Hospital Level I Trauma Center (Ginni and Richard Mithoff Trauma Center). Your generous support has allowed the Foundation to grant approximately $43,700,000 to Harris Health Programs and Services since the Foundation’s inception. Harris Health System proudly sponsored the 32nd annual Kaleidoscope Perinatal, Neonatal and Women’s Health Conference on February 22nd and 23rd, 2016 at the Galveston Convention Center. The conference was a collaborative effort organized by Houston-Galveston area hospitals and schools of nursing. The conference was a symposium designed by and for perinatal and women’s healthcare nurses. Symposium objectives included recognizing current advances in perinatology, women’s health care and nursing leadership, and exploring societal and ethical issues that impact women, newborns, and childbearing families.

The 2016 conference was an overwhelming success with over 200 conference attendees each day, 25 expert speakers, and over 30 vendors. With the strong support and expert guidance of Harris County Hospital District Foundation (HCHD Foundation), the 2016 conference offered an interactive conference website including conference information and brochure, online conference registration, online vendor registration, and a website link to the conference hotel. The HCHD Foundation provided all fiscal support that enabled seamless fund management and record keeping.

The collaborative effort between the Harris Health conference committee and the HCHD Foundation was the key to the success of the conference. The proficient communication, sharing of knowledge, professionalism, and organization provided the strong foundation that was needed to host the conference. The 2016 conference was an overwhelming success and the financial gains will be used to fund subsequent Kaleidoscope conferences.

In September 2010, the next generation began promoting solutions to the defining challenges of future generations with the expectation to raise awareness for programs within the Harris Health System by providing volunteer projects through social events and fundraising. In 2001, the HCHD Foundation hosted its first Silver Tea event in support of nursing retention. The goal was to generate private funds for retention initiatives as well as educate the community on the outstanding and dedicated nursing staff at the Harris Health System. In 2008, the HCHD Foundation, in partnership with Harris Health System administration, developed the Nursing Leadership Fund to support nursing awards recognizing professional growth and development and to highlight excellence in clinical practice.

Annually, special recognition and monetary incentives are 2016 HARRIS HEALTH NURSING awarded to outstanding nurses who have demonstrated excellence in the areas of clinical competence, professional ACCOMPLISHMENTS advancement, and community outreach. The Harris County Hospital District Foundation joined the world of Crowdfunding! For those new to the concept, crowdfunding is the practice of funding a project by raising monetary contributions from a large number of people, performed through Internet-mediated registries.

Harris County Hospital District Foundation (HCHD Foundation) hosted its 15th Annual TexasMedRun10K/5K on February 11, 2017 raising funds and awareness for the organization that we support, Harris Health System!

The 2017 Texas Med course showcased the world-famous Texas Medical Center with over 980 walkers and runners participating in our annual event. Since inception, this event has raised in excess of $1 million for Harris Health System programs – and this year’s run benefitted the Harris Health Ben Taub Hospital Level I Trauma Center (Ginni and Richard Mithoff Trauma Center) within the Harris Health Ben Taub Hospital Trauma Program.

Life as we know it can change in the blink of an eye. An on-coming car, an unavoidable tumble, or a random gunshot can set into motion a fight for life. Harris County residents are fortunate that when they experience life- threatening events, they have access to Harris Health System’s Ben Taub Hospital and, through partnership with Baylor College of Medicine, educate and train some of the best emergency medicine doctors and trauma surgeons in the nation, if not the world. With an international reputation for excellence, The Harris Health Ben Taub Hospital Level I Trauma Center (Ginni and Richard Mithoff Trauma Center) is one of only two American College of Surgeons (ACS) verified Level I trauma centers in Harris County. As a regional trauma care provider, we also serve as a primary site for mass emergencies arising from bioterrorism, chemical and nuclear incidents, and natural or weather-related events. We work with Emergency Medical Services (EMS) and surrounding medical facilities to ensure that severely injured patients have rapid access to the highest level of medical care required. The emergency room evaluation, operative care, critical care, acute care, and rehabilitative care are all integrated under the Ben Taub Trauma Program to provide the best outcomes possible.

Ben Taub Hospital also plays a central role in training the next generation of healthcare professionals. We support a host of nursing, allied health, and physician resident and fellowship programs which produce a large pool of healthcare professionals who provide care in Texas.

CHILDREN’S HEALTH FUND.

In 2002, the HCHD Foundation began managing the Children’s Health Fund. The focus of these funds has been for healthcare programs for children of Harris County. Since 2002, the Children’s Health Fund has granted 46 grants to Harris Health System related programs, 55 grants to Harris County nonprofits organizations, and 10 grants to Harris Health public programs totaling $4,441,988 all benefitting the children of Harris County.

ALL OF THESE GRANTS WORK IN NUMEROUS WAYS TO HELP CHILDREN:

BOYS & GIRLS CLUB is a positive place for our communities’ most at-risk youth people and has delivered effective, impactful afterschool programs to inspire and enable all youth, especially those who need us most, to realize their full potential as productive, responsible and caring citizens. The Healthy Lifestyles Programs for Underserved Youth helped to improve the youth’s physical and emotional health with prevention programs, nutrition education, physical activity, and positive relationships. Participating clubs adopted the culture of healthy eating and physical activity and clubs were encouraged to develop a strategic and action plan for youth and staff. 1483 students were served by being physically active and knowledgeable about nutrition and self-care.

CATHOLIC CHARITIES has provided food, clothing, shelter and a network of support services to people of all ages and from all walks of life and religious backgrounds. They also promote social justice, serving as a voice for the poor and vulnerable reaching out to more than 82,000 people a year. Catholic Charities wanted to create a program that focuses on serving Hispanic/Latino children, single parents, and families living in the Greater Houston Area with culturally appropriate outpatient behavioral health services. Within the Community Counseling Program they increased access to mental health services, clients reported a decrease in levels of symptom severity, more clients were practicing at least three new coping strategies learned and the number of clients increased. 416 children and their families were assisted through counseling.

CHILDBUILDERS trains individuals that work with children and youth in the Greater Houston Area to promote healthy decision- making skills proven to prevent risky behaviors. They wanted to create a new Personal Safety curriculum so that it is more relevant to today’s youth and families of 1st-3rd graders. With funding from the Children’s Health Fund, the curriculum was developed, fully researched and “Stand Strong, Stay Safe” was created. An artist was also recruited to create the characters that served as narrators for the lesson plans. Funds from Houston Endowment allowed them to finish the curriculum and pilot it for use.

HARRIS COUNTY PROTECTIVE SERVICES FOR CHILDREN AND ADULTS provides coordinated care for vulnerable children primarily foster children and youth placed with relatives who have trauma-related needs. For their second year of funding they were able to move forward with the Integrated Healthcare Model for children in foster care. Their newest component, the Family Support Navigation, was further developed and refined by strengthening the workflow process, documentation of engagement forging new relationships. The program was also reviewed, updated and translated into Spanish. The Resource Guide has been printed and distributed throughout the Harris Health System locations and other community based agencies. 34 families were served. HARRIS HEALTH SYSTEM is a fully integrated healthcare system that cares for all residents of Harris County, Texas. One of the largest systems in the county to achieve the quality standard, it includes 24 community health centers and school-based clinics, six same day clinics, five specialty clinics, three hospitals and two emergency trauma centers. 13 Harris Health is also a teaching hospital who trains the next generation of healthcare providers on the latest medical 2017 ANNUAL REPORT | CARING FOR OUR COMMUNITY THROUGH PARTNERSHIP procedures and technological breakthroughs. For the 2015 grant cycle, Harris Health was awarded for three different programs; School-Based Mental Health Promotion, Girls on the Run Enhancement Project and Triple P for Pasadena.

• GIRLS ON THE RUN at Pasadena PAC helps overweight teenage female patients make behavioral towards a more healthy and active lifestyle. The Enhancement Project is a running and empowerment program for girls in 3rd to 5th grade and its goal is to inspire girls to be healthy and confident. 11 girls were recruited to complete the program. This program helped addressed the need for safe and affordable physical activity option for the community and encouraged community impact projects. The program will be continued with the help of The Cleveland Ripley Neighborhood Center.

• SCHOOL-BASED CLINICS delivers mental health promotion programming in schools linked to Harris Health School-Based Clinics this year teaming up with Sheldon ISD. The Mental Health Promotion Project provides children living in high risk situations with the skill and support they need to overcome adversity, stay in school, and stay free of drugs, violence, and crime. In its’ 3rd year of funding, the Rainbow Days: Kids’ Connection curriculum was provided into seven schools from Sheldon ISD. 310 children and 372 parents or guardians received different types of services.

• TRIPLE P FOR PASADENA is the first primary care clinic in the Harris Health System devoted exclusively to provide services to children ages 0-18 years. Triple P for Pasadena is an intervention program to increase access to mental and behavioral services for children younger the 5 years old from the Pasadena PAHC. 11 providers took part in the training CHILDREN’S HEALTH FUND. and more than 550 families in the Houston area were served. Providers were trained and accredited to deliver the program.

HOUSTON AREA WOMEN’S CENTER helps individuals affected by domestic and sexual violence in their efforts to move lives forward providing shelter, counseling and advocacy to support families building lives free from the effects of violence. In their 2nd year of funding they wanted to improve the health of clients and their families by stabilizing them medically and mentally, connecting families to long-term medical care, and reducing barriers to accessing medical health resources in the community. Integrated Health Services for Survivors of Domestic Violence is in its second year of funding and has reduced barriers in accessing health care services, expanded partnerships with community providers to create an integrated health approach. They provided a range of therapeutic modalities to help survivors of domestic and sexual violence begin healing from complex traumatic histories, group counseling, advocacy, case management, transportation for adults and children residing at our residential facility to 482 kids so far.

NEIGHBORHOOD RECOVERY COMMUNITY DEVELOPMENT CORPORATION creates opportunities to raise the quality of life for residents and stakeholders by holistically revitalize physical, personal and civic infrastructure. Facilitators of Change for Underserved Students through Exercise and Diet (FOCUSED), was implemented to provide students of Cullen Middle School with physical activity opportunities, nutrition education and navigator services to link them and their families to appropriate resources. Participants completed three phases of training to improve their physical health conditions. To improve behavioral conditions of children, questionnaires were distributed to families and assessed so that staff would be able to redirect inappropriate behaviors and make referrals and recommendations. Lastly, presentations were held to promote healthy food alternatives to children and their families. 333 students participated in after school physical fitness and nutrition programming.

NORTHWEST ASSISTANCE MINISTRIES assists people toward independence while respecting racial, cultural, and religious differences. NAM wanted to add comprehensive case management to their program to oversee a collaborative process which assesses, plans, implements, coordinates, monitors and evaluates the options and services to meet an individual’s health and family needs Their model is to facilitate the continuum of care for patients needing services from multiple types of providers. Direct facilitation increases the likelihood that patients will be more compliant, and therefore more successful, by improving outcomes in the most challenging areas. The funds were utilized to provide assistance in transportation, utility expenses, housing assistance, clothing referrals, prescriptions or other essential items. 272 clients were served.

SANTA MARIA HOSTEL, INC. has more than 56 years of experience serving women in crisis in Harris County providing residential and outpatient treatment for substance abuse. Healing Hearts program in its 2nd year of funding continues its program by providing onsite mental health programming for children of mothers involved in Santa Maria’s residential, outpatient and intervention substance abuse programs. This program has been very effective not only addressing the short term critical needs of the children, but providing positive long-term family change and helping to break the cycle of addiction and poverty. These outcomes includes building children’s resiliency to trauma by improving family functioning and improving children’s positive coping skills and improving their mental health through provision of trauma focused therapy services. 76 mothers and 39 children completed these programs.

SEARCH HOMELESS SERVICES with over 25 years of experience, SEARCH works with the most vulnerable adults and children who are homeless. of Tiny Treasures for Early Childhood Education Program supports the art, speech and play therapy components of the Early Childhood Development program. With the Children’s Health Fund support every student had access to the critical therapeutic services they needed at no cost to the family. Their program included a report form completed by parents and caregivers to have a better understanding of behavior at home.
