Introduction ICD-10-CM Mappings

Equivalent Mapping: The arrow symbol Ê designates a mapping relationship that is considered an “Equivalent Mapping.” The clinical concept between the two codes in ICD-9-CM and ICD-10-CM are considered completely equivalent. Users should be aware that just because a target ICD-10-CM code does not have an arrow, it does not mean that the clinical concepts are completely dissimilar. There may be additional clinical concepts that add specificity in ICD-10-CM that should be considered. For example, ICD-9-CM code 595.0 represents acute cystitis. In the ICD-9-CM to ICD-10-CM forward mapping, the code is mapped to two ICD-10-CM codes, N30.00 and N30.01. Example ICD-9-CM ICD-10-CM 595.0 ACUTE CYSTITIS N30.00 ACUTE CYSTITIS WITHOUT HEMATURIA N30.01 ACUTE CYSTITIS WITH HEMATURIA

ICD-10-CM combines the clinical concepts of cystitis and hematuria, which ICD-9-CM does not. Therefore, although both N30.0- codes are mapped to 595.0, it is not considered an equivalent mapping because an additional code for the hematuria would be required in ICD-9-CM coding for the clinical concepts to match. Both of the examples above are considered one-to-one mapping relationships, yet one is equivalent and the other approximate. One code from the source system is mapped to one code option in the target system

There are four coding scenarios for this particular example: tuberculosis of the spine maps back to arthropathy associated with tuberculosis, inflammatory spondylopathy due to tuberculosis, other infections (tuberculosis) involving bone, and curvature of the spine due to tuberculosis.

• Scenarios: Use of “and” and “or” Operators: GEM scenarios are the combinations of ICD-10-CM codes that are required to satisfy the meaning of an ICD-9-CM code. The user must take special note of the “and” and “or” operators found throughout the mapping guide. Whenever these operators appear, they are shaded in red as an alert that additional selection logic must be applied. The equivalent clinical concept of the ICD-9-CM code for ornithosis with pneumonia requires both ICD-10-CM codes A70 and J17 to be assigned. In the following example, the ICD-9-CM code specifies that the traumatic amputation is bilateral but does not specify whether it is partial or complete. Because both pieces of information — left or right foot and partial or complete amputation — are specified in separate codes in ICD-10-CM, the entry in the ICD-9-CM to ICD-10-CM GEM is a “scenario” entry. Because two ICD-10-CM codes are required to satisfy the equivalent meaning in the ICD-9-CM combination code, and because the injury can be partial on one side and complete on the other, both sides partial, or both sides complete, there are four different scenarios from which to select based upon available documentation.

Because the ICD-9-CM coding system combines superficial foreign body codes for shoulder and upper arm into a single code and ICD-10-CM does not, the user must make a selection between the separate ICD-10-CM codes related to body site. Regardless of that choice, the secondary code (L0 8.89) indicating the infection must be assigned in addition to the primary code for the foreign body. • Brackets: Some ICD-10-CM codes appear in this manual with brackets as a space-saving convention.
