Cardiovascular disease (CVD), mainly heart attack and stroke, is the leading cause of death in . In Baw Baw, between 25-30% of the adult community have CVD. The rates of Type 2 Diabetes in Baw Baw are also high and are among the highest in the state.

Despite these disturbing statistics, both CVD and Type 2 diabetes are largely preventable. By understanding your risk factors and taking steps to change your lifestyle, you can reduce your risk of developing these diseases.

Making changes to your lifestyle can be hard and there are many services out there in your community that can help you. This document provides information about the range of programs and activities available to help Baw Baw residents adopt a healthier lifestyle. It has basic details and contact information for a wide range of programs - from intensive, structured programs to casual, low‐impact exercise opportunities. Understand your risk

There’s no single cause for CVD or Type 2 diabetes, but there are risk factors that increase your chances of developing these diseases. The more risk factors you have, the higher your risk.

Understanding your risk is the first step you should take if you wish to avoid a heart attack, stroke or Type 2 diabetes. There are different ways to assess your risk:

Speak to a GP or health care professional and request a risk assessment:

- For those aged over 18 years your risk of Type 2 diabetes can be checked with a tool known as AUSDRISK and a Body Mass Index (BMI) assessment that considers your height and weight

- Those aged 45 years and over, or 35 years and over of Aboriginal and/or Torres Strait Islander descent, without a history of heart disease or stroke, can request a Heart Health Check using the Absolute CVD Risk tool

Once you understand your risk factors, the next step is to make changes to decrease

your risk. Addressing the risk factors for CVD and Type 2 diabetes is important for

everyone. For most people this will involve four seemingly simple, yet often hard steps

to take:

1 Being more active (exercising) by moving more and sitting less

2 Quitting smoking

3 Maintaining a healthy weight, and

4 Eating healthy

If you have heart problems or think you might have heart disease, it’s really important that you check with your doctor before starting an activity program. The Heart

Foundation’s Get Started with More Physical Activity has tips for how you can get active safely.

The following programs are designed to support people to make lifestyle changes that will improve their general health and wellbeing and reduce their risk of Type 2 Diabetes and Cardio Vascular Disease (CVD). Programs and/or elements of the programs are based on evidence and best practice.

Please note that there are several ‘secondary prevention’ programs in operation across Baw Baw, for people who have been diagnosed with heart disease or Type 2 diabetes. They include Cardiac and Pulmonary Rehabilitation programs conducted at West Gippsland Hospital, as well as programs and support offered by diabetes educators, dieticians and a range of other health professionals.

The following programs and activities are ‘general’ in nature and include casual activities to increase physical activity and fitness and promote healthy eating.
