Locum Consultant in Intensive Care with sessions in Anaesthesia January 2020

APPOINTMENT

1 year fixed term Locum Consultant in Intensive Care Medicine for United Lincolnshire Hospitals NHS Trust based at Pilgrim , Boston. The duties include supporting the intensive care rota in line with the Intensive Care Society guidelines on Standards for Consultant Staffing of Intensive Care Units.

Current Intensive Care Consultants also undertake regular sessions in anaesthesia services for 1-3 sessions/ week and the illustrative job plan includes anaesthesia sessions.

Any Consultant who is unable to work full time will be considered for the post. If such a person is appointed, modification of the job content will be discussed on a personal basis with the Trust in consultation with consultant colleagues.

GENERAL

Lincolnshire is scenically and geographically varied area. It includes the Lincoln escarpment on which the beautiful medieval Cathedral stands, the North Lincolnshire Wolds, a designated area of outstanding natural beauty, the extensive beaches of the coastal strip, and the vast farmlands in the south of the county

Boston (population 50,000) is one of the two main hospital centres for South Lincolnshire and is an attractive and important market town which is sited on the River Witham and has been a flourishing port since the thirteenth century. The majority of Boston's industry is linked either with agriculture or with the trade of the port, but there is also a certain amount of light industry.

The area offers the prospect of affordable housing in quiet surroundings and excellent primary and secondary schools. South Lincolnshire has retained the grammar school system, and has a reputation for providing excellent state school education.

Other urban centres in South Lincolnshire are Grantham, , Stamford, Spalding, Bourne and . Skegness is a seaside resort situated 25 miles from Boston with a population of 14,000, but during the summer months there is an influx of approximately 200,000 visitors to Skegness and other resorts on the Lincolnshire coast.

HOSPITALS

Pilgrim Hospital, Boston

Pleasantly situated on the outskirts of Boston, Pilgrim Hospital (which became fully operational during 1977) is a modern District General Hospital providing a full range of clinical specialities to a catchment population of 190,000. It includes diagnostic facilities of

1 the highest standard and a full range of support services. It has a highly regarded postgraduate medical centre with modern and extensive IT facilities.

Pilgrim Hospital is part of the United Lincolnshire Hospitals NHS Trust, which includes Grantham, Lincoln and Louth Hospitals.

The hospital provides beds for general medicine, elderly care, general surgery, orthopaedic surgery, vascular surgery, ENT, ophthalmology, paediatrics, breast, urology, maxillo-facial surgery, obstetrics and gynaecology.

The hospital has a strong identity underpinned by a philosophy of personal care to patients and staff. The Directorate has well developed Clinical Audit, Quality and Risk Management programmes.

The medical community are encouraged to actively participate in management at all levels and clinical innovation is supported.

THE INTENSIVE CARE DEPARTMENT

Pilgrim Hospital Intensive Care Unit is a mixed level 3 / level 2 facility providing comprehensive critical care to all adult inpatients at Pilgrim Hospital. The activity and outcome profile for the Department is included in the appendices to this job description

The unit currently consists of a maximum 12 beds which are used flexibly between level 2 & level 3, (Currently funded for 5 level 3 and 4 level 2 beds) beds. The unit is a new refurbishment via a capital project worth approximately £4 million and was completed in April 2012. This includes new patient care and staff facilities and equipment and the introduction of an electronic patient management system on Critical Care (MetaVision).

The Critical Care Unit staffed by the Department of Critical Care on an 8 consultant ICM rota.

The current nursing complement is over 60 WTE. The medical care is provided by 8 consultants in anaesthesia with an interest in critical care with middle grade support from the Department of Anaesthesia. The eight consultants provide 24/7 cover in a flexible rota and have no other duties when on call for ICU . The anaesthetic team out of hours comprises of a consultant for ICU, a general duties consultant for theatres and obstetrics, 2 middle grades (Associate Specialist/Specialty Doctor / ST3+ ) one covering ICU and one covering theatres/ Obstetrics and a CT / speciality doctor covering theatres. There is a 24/7 CCOT service as part of the acute care support service. The department delivers follow-up clinics following discharge from Critical Care after six months. These clinics are Consultant-led once a month and nurse-led once a month.

THE ANAESTHETIC DEPARTMENT

The Department of Anaesthesia at Pilgrim Hospital provides anaesthetic services for all the surgical specialties based at Pilgrim Hospital, plus services to the Accident and Emergency department and the Department of Radiology (including CT and MRI scanning)

2 Theatre facilities include 11 operating theatres; a block of four for general surgery, ENT, dental surgery and urology, a block of six for orthopaedic surgery, ophthalmology and gynaecology, plus one operating theatre in the maternity block.

The Anaesthetic and Critical care Departments actively support training in ICM and Anaesthesia and have the following trainee allocation.

4 Specialty trainees from the East Midlands School of Anaesthesia 7 Core trainees from the East Midlands School of Anaesthesia 1 Trust grade doctors 5 Foundation trainees

The Department has 2 secretaries. The office accommodation, including computing facilities and 2 open plan Consultants offices, are on the first floor of the hospital.

JOB PLAN AND APPRAISAL

There will be an annual appraisal and job plan review conducted by the Clinical Lead and Divisional Director

The proposed structure of the job plan will be as follows.

It is for a basic 10 PA weekly contract composed of:

The Unpredictable Direct Clinical Care out of hours Consultant On-call rota is on a 1:8 weekday night and weekend night basis and is job planned as annualised prospective cover. The on-call supplement is 5%.

This post is advertised as a 10 PA contract. Additional DCC PAs or SPAs are available by negotiation/ agreement for specific DCC activities or defined SPA roles such as an educational supervisor, trust appraiser in accordance with the ULHT job planning guidance and limited by a maximum averaged 48 hour working week.

On call weekend 1:8 with prospective cover for unpredictable on call.

ICM scheduled care is 08:00 – 21:00 7 days/ week- other speciality scheduled care would generally be 08:00-18:00 weekdays only On call is from residence within 10 miles / 30 minutes travelling time of the hospital and is for ICM only.

The Job Plan is reviewed annually, and the department has adopted a standardised generic approach to the job planning process

DUTIES

The main duties of the post will be in Intensive Care with supplemental weekday daytime DDC duties in Anaesthesia.

The appointee will have a continuing responsibility for the care of patients in his/her charge and for the proper functioning of the department.

There is a requirement to participate fully with the clinical governance programme the department and to undertake annual appraisals to support ongoing revalidation. There is a rolling half day audit/clinical governance/CPD programme including morbidity/mortality meetings.

The appointee will be expected to take an active part in the education and training for both the ICM/ Anaesthesia trainees and other members of staff at Pilgrim Hospital.

There is ample opportunity and support for research in the department and the applicant will be encouraged in projects they may wish to undertake. CPD activities are actively supported both by provision of allocated weekly SPA time and by the proviso of 30days / 3 years (typically taken as approximately 10 days/ annum) of paid and funded study/ professional leave time based on an agreed annual appraisal and PDP

A flexible rota day time ICM on-call and anaesthesia sessions is in operation providing a varied mix of surgical specialities on a week to week basis, most consultants have some regular lists or special interests they undertake on a regular basis with the remainder made up of flexibly allocated lists according to the needs of the service. For consultants wishing to pursue another speciality interest the non ICM duties would be by negotiation/ agreement with the relevant clinical team and colleagues wishing to pursue that option are strongly encouraged to contact us for further discussion.

When on-call for ICU the responsibilities include duties limited to intensive care medicine and associated work (resuscitation, ward and ED referrals, and trauma calls), without concurrent cover for other disciplines.

RESIDENCE

The person appointed will be required to maintain his/her private residence in contact with the public telephone service and to reside within easy access and not more than 10miles / 30 minutes travelling time from Pilgrim Hospital, Boston, unless prior approval to a greater distance is given.
