3 Alameda County COVID-19 Epidemic Key Points

• Daily case rate is 4.4 per 100,000 (improved) • Adjusted Case Rate is 3.2 • Between 60 and 210 cases reported daily over past 10 days • Test positivity 1.3% • 1.9% in lowest HPI quartile • Hospitalized COVID cases 69; ICU 20 • Testing 8,000 per day • Parts of the US, world with high case rates • Alameda County, region, state remain stable

4 COVID-19 “Vaccine Breakthrough”

• COVID-19 infections in a fully vaccinated person • An expected occurrence with COVID-19 vaccines (and any vaccine) • COVID -19 vaccines are highly effective but not 100% • Community transmission of COVID-19 is still at moderate level • Many reported breakthroughs are asymptomatic infections • Some are likely not new infections • Clinical trial effectiveness was based on symptomatic disease • Concerning breakthrough infections • Hospitalizations, deaths, severe illness due to COVID-19 • Also expected to occur, but should be uncommon • Might represent variant strains for which vaccines are less protective*

5 Vaccine Breakthrough Monitoring

• CDC reported 5,813 breakthrough cases as of 4/13 • 7% hospitalized and 1% died • Includes non-COVID hospitalizations and deaths • 29% asymptomatic • 75 million people in the US fully vaccinated • ACPHD had 93 reported breakthrough cases as of 4/14 • No hospitalizations or deaths due to COVID-19 among these cases • 65% asymptomatic – mostly tested for routine screening • 340,000 full vaccinated • CDC and CDPH shifting to focus on hospitalizations and deaths • COVID vaccines are excellent - breakthroughs are rare but expected events

6 2. Reopening/Blueprint

7 Shelter in Place Update

• AC in Orange Tier • We continuing to follow state restrictions • Newly permitted: private gatherings, indoor live events, private events • Increased capacity limits with vaccinated persons for certain activities • State announced plan to lift Blueprint restrictions in mid-June • Contingent on sufficient vaccine supply and low disease burden • Face covering mandate, general risk reduction guidance would remain • CalOSHA regulations and certain other industry specific rules would remain

8 CDPH Modified Guidance for Fully Vaccinated Persons

• Fully vaccinated people may: • Spend time with other fully vaccinated people, including indoors, without wearing masks or physical distancing. • Spend time with unvaccinated people from a single household who are at low risk for severe COVID-19 disease indoors without wearing masks or physical distancing. • Refrain from quarantine and testing following a known exposure if asymptomatic (outside the workplace setting). • Full vaccinated people should continue to • Wear face coverings and maintain physical distance when in public or with persons at high risk for severe COVID • Follow gathering guidance when gathering with any unvaccinated persons • Get tested for any COVID symptoms and isolate if testing positive

10 11 Johnson & Johnson Vaccine Pause

• On 4/13 CDC and FDA recommended pausing administration of J&J vaccine • Alameda County suspended administration of J&J the same day • 6 reported US cases of blood clots in individuals who received J&J vaccine • Cases among women ages 18 to 48 • Symptoms occurred 6 to 13 days after vaccination • One death reported • 6.8 million doses of J&J vaccine have been administered in the US • 1.4 million among women ages 20 to 50 • CDC Advisory Committee on Immunization Practices met 4/14 • No recommendation yet; next meeting 4/23 after more data review • Western States Scientific Safety Review Workgroup also reviewing data

16 Vaccine Supply Update

• Current County-wide supply ~ 70,000 doses per week • 33,000 – 36,000 first doses directly and via TPA • Only 11,300 first doses directly to HCSA/ACPHD in the current week • 10,530 for Coliseum • No J&J doses this week • Also getting 5,670 second doses directly to HCSA/ACPHD • Supply is expected to increase beginning next week into June • Increases expected regardless of J&J issues
