The recognition of the prevalence and importance of patients with endoscopy-negative reflux disease as well as those refractory to proton pump inhibitor therapy have led to an increasing need for objective tests of esophageal reflux. Guidelines for esophageal reflux testing are developed under the auspices of the American College of Gastroenterology and its Practice Parameters Committee and approved by the Board of Trustees. Issues regarding the utilization of conventional, catheter-based pH monitoring are discussed. Improvements in the interpretation of esophageal pH recordings through the use of symptom-reflux association analyses as well as limitations gleaned from recent studies are reviewed. The clinical utility of pH recordings in the proximal esophagus and stomach is examined. Newly introduced techniques of duodenogastroesophageal reflux, wireless pH capsule monitoring and esophageal impedance testing are assessed and put into the context of traditional methodology. Finally, recommendations on the clinical applications of esophageal reflux testing are presented. Doctors can use various techniques for this procedure while your are sedated during your EGD. Flexible Sigmoidoscopy - A sigmoidoscope is a slender, flexible tube with a light and a very small video camera at the end of it. It is shorter than a colonoscope and is only able to evaluate the lower third of the colon. This allows a look at the inside of the rectum and lower part of the colon for cancer or polyps. Before the test, you will need to take an enema or other prep to clean out the lower colon, but a full cleansing solution is not needed as in colonoscopy.

    ACS/ASE Medical Student Core Curriculum Abdominal Pain - Gastroesophageal Reflux Disease ABDOMINAL PAIN - GASTROESOPHAGEAL REFLUX DISEASE Epidemiology and Pathophysiology Gastroesophageal reflux disease (GERD) is one of the most commonly encountered benign foregut disorders. Approximately 20-40% of adults in the United States experience chronic GERD symptoms, and these rates are rising rapidly. GERD is the most common gastrointestinal-related disorder that is managed in outpatient primary care clinics. GERD is defined as a condition which develops when stomach contents reflux into the esophagus causing bothersome symptoms and/or complications. Mechanical failure of the antireflux mechanism is considered the cause of GERD. Mechanical failure can be secondary to functional defects of the lower esophageal sphincter or anatomic defects that result from a hiatal or paraesophageal hernia. These defects can include widening of the diaphragmatic hiatus, disturbance of the angle of His, loss of the gastroesophageal flap valve, displacement of lower esophageal sphincter into the chest, and/or failure of the phrenoesophageal membrane. Symptoms, however, can be accentuated by a variety of factors including dietary habits, eating behaviors, obesity, pregnancy, medications, delayed gastric emptying, altered esophageal mucosal resistance, and/or impaired esophageal clearance. Signs and Symptoms Typical GERD symptoms include heartburn, regurgitation, dysphagia, excessive eructation, and epigastric pain. Patients can also present with extra-esophageal symptoms including cough, hoarse voice, sore throat, and/or globus. GERD can present with a wide spectrum of disease severity ranging from mild, intermittent symptoms to severe, daily symptoms with associated esophageal and/or airway damage. For example, severe GERD can contribute to shortness of breath, worsening asthma, and/or recurrent aspiration pneumonia.

When contemplating bariatric surgery, patients with obesity type III undergo esophagogastroduodenoscopy (EGD) and also esophageal manometry (EMN), and prolonged pHmetry (PHM) as part of their pre-operative evaluation. Objective – Description of endoscopy, manometry and pHmetry findings in patients with obesity type III preparing for bariatric surgery, and correlation of these findings with the presence of typical GERD symptoms. Methods – Retrospective study in which clinical symptoms of GERD were assessed, focusing on the presence of heartburn and regurgitation. All patients underwent EMN, PHM and most of them EGD. Results – 114 patients (93 females–81%), average age 36 years old, average BMI of 45.3, were studied. Typical GERD symptoms were referred by 43 (38%) patients while 71 (62%) were asymptomatic. Eighty two patients (72% of total) underwent EGD and 36 (42%) evidenced esophageal abnormalities. Among the abnormal findings, hiatal hernia was seen in 36%, erosive esophagitis (EE) in 36%, and HH+EE in 28%. An abnormal EMN was recorded in 51/114 patients (45%). The main abnormality was a hypotensive lower esophageal sphincter (LES) in 32%, followed by ineffective esophageal motility in 25%, nutcracker esophagus in 19%, IEM + hypotensive LES in 10%, intra-thoracic LES (6%), hypertensive LES (4%), aperistalsis (2%) and achalasia (2%).

    Peptic Ulcers Peptic ulcer disease (PUD) is a very common ailment in the US, affecting one out of eight persons in their lifetime. Over half a million new cases are diagnosed each year, afflicting some 25 million Americans and costing over $3 billion annually. Recent breakthroughs in the causes of ulcers now give your doctor new ways to treat ulcer disease and help prevent ulcers from ever coming back. Normal Stomach Function To understand how ulcers form, it is first important to understand how the normal stomach and intestines function. The stomach produces acid in response to eating food. This acid serves to start the digestive process by activating the enzyme pepsin, which can then break down proteins in food. In addition, this acid provides a hostile environment that is extremely difficult for most bacteria to survive in. After the food is mixed up and broken into small particles in the stomach, it is released into the first portion of the small intestine, or duodenum. It is here in the small intestine that the actual process of digestion and absorption of nutrients occur. To avoid digesting itself, the stomach and duodenum have special protective mechanisms. A protective coating of mucus covers the inner lining of the stomach and duodenum. There is always a delicate balance between the destructive forces of acid and the protective forces of the stomach and duodenum. This balance is such that just enough acid is made to digest food, but not enough to overwhelm this protective layer. Causes of Peptic Ulcer When the delicate balance between acid and the protective forces is interrupted, ulcers may form.
