Noah E. Canvasser, M.D.

Philosophy of Care Thorough communication is critical to determine optimal, individualized management strategies for each patient. I work to ensure that every patient fully understands his or her medical condition before embarking on various treatments. In addition, treatment decisions must be taken in the context of each patient's medical, social, and family environment. As such, I balance each patient's condition within their respective situation.

Clinical Interests Dr. Canvasser is a fellowship-trained endourologist and minimally invasive surgeon offering care for adults with kidney stone disease, kidney obstruction, kidney cancer, and enlarged . He has a special interest in the medical management and prevention of patients with recurrent kidney stones. His surgical expertise includes complex percutaneous and ureteroscopic management of kidney stones, laparoscopic nephrectomy for kidney cancer and non-functioning kidneys, and robotic surgery, including partial nephrectomy for kidney cancer, pyeloplasty for ureteropelvic junction (UPJ) obstruction, reconstruction for kidney obstruction, and simple for enlarged prostates.

Research/Academic Interests Dr. Canvasser's research focuses on the prevention and management of patients with complex kidney stone disease, as well as improving minimally-invasive surgical management of kidney obstruction, kidney cancer, and enlarged prostates.

Raul I. Clavijo, M.D.

Philosophy of Care For the patients I see in my clinic I strive to provide an educational setting where the patient is able to learn about his condition after which treatment options are presented. I feel a shared decision regarding treatment based on a patient's newly acquired understanding on his condition is essential to provide long term compliance and efficacy of a treatment plan. It is clear to me that this requires an individual approach as I have learned that not all patients will fit into established care algorithms.

Clinical Interests Dr. Clavijo is a urologist fellowship trained in andrology, including male reproductive medicine and surgery. He has a special interest in male reproductive tract reconstruction and treating conditions such as testosterone deficiency, varicocele, sexual dysfunction, infertility, erectile dysfunction (including the placement of penile implants) and Peyronie's disease. He is unique in the Sacramento area as he places a high volume of penile implants. He is skilled in microsurgery and performs reversals (/vasoepididymostomy), microdissection testicular sperm extraction (micro TESE), microsurgical epididymal sperm aspiration (MESA), testicular sperm aspiration (TESA) and microsurgical varicocelectomy.

Research/Academic Interests Dr. Clavijo's research focuses on the effects of lifestyle factors on sexual dysfunction, erectile dysfunction and male infertility. He also has an interest in novel therapies for erectile dysfunction such as low intensity shockwave therapy.

Christopher P. Evans, M.D.

Philosophy of Care Dr. Evans' philosophy of care is to:

• fully assess the patient's medical condition, including how it relates to family and social circumstances; • educate the patient fully about their disease process; and • include the patient in the decision-making process that best suits the outcomes they seek. In each case he looks for opportunity to enroll them in any new therapies or clinical trials that may be available and pertinent.

Clinical Interests Dr. Evans' practice is dedicated to the management of patients with urologic malignancies. He works with medical oncologists and radiation oncologists to provide multimodal approaches to cancer therapy. His surgical expertise includes robotic assisted laparoscopic radical prostatectomy with nerve sparing, complex renal surgery to include partial nephrectomy and vena cava thrombectomy, cystectomy with extended pelvic lymph node dissection and urinary diversion with orthotopic neobladders. Dr. Evans also has expertise in treating patients with metastatic testicular cancer and performing retroperitoneal lymph node dissection in post chemotherapy patients.

Research/Academic Interests Dr. Evans is the president of the Society of Urologic Oncology. His research laboratory focuses on prostate cancer; specifically, mechanisms signaling the androgen receptor to activate prostate cancer growth and progression following castration. His laboratory has developed models to study this and has identified novel mechanisms that activate the androgen receptor. His laboratory work has also tested new agents and brought them from bench studies to animal models to clinical trials.

Allen Gao, M.D., Ph.D.

Research/Academic Interests Dr. Gao’s research interests focus on understanding molecular mechanisms associated with progression of castration-resistant prostate cancer and metastasis to bone with the goal of identification of potential therapeutic targets for prostate cancer. Particular emphasis includes microRNAs, aberrant androgen receptor activation by cytokines and transcriptional factors such as Stat3 and NF-kB, targeting cell signaling pathways (AR, IL-6 and Stat3), and mechanisms of drug resistance in prostate cancer. Dr. Gao’s lab was among the first to find that selenium regulates androgen signaling and that IL-4 activates the androgen receptor mediated by NF-B and Stat6, made the discovery that IL-6 signaling in prostate cancer involves Stat3, defined Stat3 and NF-B interactions in prostate cancer, discovered niclosamide as a novel inhibitor of androgen receptor variants (AR-V7), and identified several novel resistance mechanisms to enzalutamide/abiraterone/docetaxel including p52, IL- 6/Stat3, intracrine androgens/AKR1C3, and ABCB1. Dr. Gao has published over 100 peer-reviewed articles in the area of prostate cancer. His research findings such as the discovery of niclosamide as a novel inhibitor of androgen receptor variants have translated into several clinical trials (NCT02807805, NCT03123978) that test the combination treatments with current therapies to advanced resistant prostate cancer. Another recent discovery of indomethacin as an inhibitor of AKR1C3 and synergizing enzalutamide has also translated into clinical trial (NCT02935205) to test the combination treatments with current therapies to advanced resistant prostate cancer. Dr. Gao has served numerous NIH, NCI, DOD and VA merits, and American Cancer Society (ACS) review panels including SPORE and PPG study sections.

Research/Academic Interests My current research concentrates on the study of signal transduction pathways involved in prostate cancer development and progression. This includes studies on the EGFR family of receptor tyrosine kinases, leading to the PI3K/Akt pathway and finally to the mTOR signaling pathway. In particular, I investigate the interaction of these pathways with the androgen receptor. My research involves identification of significant therapeutic targets, as well as studying the effects of inhibitors of those targets both in vitro as well as in animal models. I am currently funded to study drug combinations that target parallel signaling pathways and in combination, appear to be highly effective in inhibiting the progression of prostate cancer.
