Applications are invited from trainee doctors having completed or about to finish either UK Foundation Programmes or equivalent to apply for the position of Clinical Development Fellow (CDF) in Paediatrics.

This exciting post offers working in the Paediatric department at Forth Valley Royal Hospital. In addition, the CDF posts will have 6 hours per week of protected development time and 2 hours of in-house training. The remaining period will be distributed between clinical work in Paediatrics and Neonates, acute receiving both in and out of hours. The post offers experience in General Paediatrics and Neonatology, which would include responsibility for assessment and treatment on the Tier 1 rota for acute paediatric inpatients, neonatal intensive care (Level 2), Postnatal and Labour wards and in our short stay assessment area and Day care. The posts will also allow for exposure to patients managed by our subspecialty teams, which includes Diabetes, Endocrinology, Cystic Fibrosis, Respiratory, Gastroenterology, Nephrology, Rheumatology, Neurology and Neuro disability or Community Child Health.

3 Introduction to NHS Forth Valley

NHS Forth Valley is one of 14 regional health boards in Scotland and serves a population of nearly 310,000 in a diverse geographical area which covers the heart of Scotland and covers three council areas, Clackmannanshire, Stirling and .

NHS Forth Valley has an annual budget of approximately £570 million and is the largest employer in the area, with approximately 8000 staff from a wide range of professional and support occupations. Acute Services are based in our modern, purpose built 860 bedded, Forth Valley Royal Hospital, which opened in 2011 and provides inpatient and outpatient services. It is supported by a network of four community hospitals, 56 health centres, day centres providing care and support for patients with mental illness and learning disabilities and a wide range of community based services.

We are home to the Scottish Centre for Clinical Simulation and Human Factors, one of the most advanced training facilities of its kind where medical staff are able to hone their skills using computerised mannequins. 4 The Forth Valley Area

Forth Valley, in the heart of Scotland, is an exciting place to live and work. Transport links are second to none with excellent motorway and rail links and two international airports nearby, Edinburgh and Glasgow. House hunters in the Forth Valley area are spoiled for choice. Job Description

Job Title: Clinical Development Fellowships (CDFs) – Paediatrics Posts: 2 posts, fixed term for 12 months. Hours of Work Full time or Part time

Working with Clinical Director and the Directorate Management teams for NHS Forth Valley, these posts will give successful applicants the opportunity to gain experience in general Paediatrics and Neonates with opportunities of outpatient subspecialty exposure before application to Paediatric training. Successful applicants will gain further specialty experience with the potential to develop their clinical experience while having dedicated development time in which they can strengthen their portfolios with involvement with undergraduate and postgraduate medical education, quality improvement and the chance to get involved in training and course development and the Scottish Centre for Simulation and Clinical Human Factors. It is envisaged that the job will help implement the themes of Realising Realistic Medicine.

These posts are aimed at FY2+ level i.e. those doctors having successfully completed the UK Foundation programme, or overseas equivalent who are looking for some experience of acute Paediatrics and Neonates in which they may have a longer term interest, or to provide a professional development adjunct to any future clinical activity if interested in another non-acute specialty longer term.

The directed clinical activity will be dedicated to complement Tier 1 rota in acute Paediatrics and Neonates in hours and out of hours, The clinical development component will allow for 8 hours development per week, 2 of which it is anticipated will be participation in local departmental teaching and the remaining 6 hours linked to an area of mutual professional interest including clinical teaching, quality improvement and safety, simulation, management and leadership development or informatics. The successful candidates will undertake quality improvement work within the Paediatric department to develop pathways and service with a view to realising the ethos of Realistic Medicine.

6 Duties of the Post

The exact format of each post will be agreed with the successful applicant, the Clinical Director for Paediatrics and General Manager for Women and Children’s Directorate and the Director of Medical Education NHS Forth Valley. However, the following general principles will apply. Within a full time contract, each fellow will contribute approximately:

 1 day per week of protected development time  Supervised clinical activity linked to Paediatrics and Neonates. Acute unselected on call activity including out of hours.

There will be expectation to cross cover colleague’s short notice absences (e.g. sickness absence) as is the case with the remaining trainee workforce.

CPD can take the form of some of these components (details to be negotiated on commencement):

Background

Acute Paediatrics Hospital-based medical Paediatric services are provided on a single inpatient site at Forth Valley Royal Hospital. This includes: general Paediatrics, ambulatory and day case investigations unit, neonatal intensive and special care baby unit, Paediatric surgery from age 3 years, and anaesthetics (general surgery, urology, ENT, ophthalmology, orthopaedics), accident and emergency service, and radiography services for children (plain X-ray, contrast studies, Isotope scans, ultrasound, CT and MRI).

7 Inpatient General Paediatrics Approximately 5000 children are admitted every year. As well as standard in-patient beds, there are also high dependency facilities for close observation and stabilisation for transfer to PICU in Glasgow or Edinburgh.

The average length of stay for our admissions is less than 1.1 days demonstrating our commitment for children to be cared for at home whenever possible. The Specialty Doctor, Specialty Trainees, alongside the Consultant and Associate Specialist staff, provide continuous support and supervision to the inpatient Children’s ward and ambulatory unit during normal working hours Monday to Friday.

The Consultant Paediatricians and one Associate Specialist share the on-call rota for out-of- hours cover for the Paediatric service at nights and weekends and are available to support the trainees, nurse practitioners and Specialty Doctor when required.

Daycase Paediatric Care Around 2000 children per year are seen in the ambulatory care unit or are admitted as day- cases. The service is in a separate area adjacent to the children’s ward and caters for surgical day cases as well as phlebotomy service and Paediatric subspecialty Day case admissions.

Care of the Newborn Over 3000 babies are born in Forth Valley Royal Hospital each year. The specialty trainees, nurse practitioners and Specialty Doctor are supported by Consultant Paediatricians and an Associate Specialist who have advanced neonatal skills and share the weekly rota to provide continuous support to the antenatal services, labour ward, neonatal intensive care unit and the postnatal wards. These Consultant Paediatricians also share an out-of-hours nights and weekends on-call rota and provide regular support to Specialty Doctors, trainees and nurse practitioners covering the neonatal service.

Neonatal care in Forth Valley is provided at BAPM Level 2 and lies within the West of Scotland Neonatal Managed Clinical Network with shared clinical guidelines, audit and education activities being well established. Planning for at risk deliveries or postnatal need for ongoing intensive care is coordinated through the Perinatal Advisory Service which is currently regional but may extend to a national service in the future.

Outpatient Clinics Around 1700 new medical Paediatric patients and between 5000 and 6000 return patients are assessed every year at the outpatient clinics in Forth Valley Royal Hospital, Stirling Community 8 Hospital and Clackmannanshire Community Healthcare Centre. Consultant Paediatricians and Associate Specialists as well as Specialty Doctor, nurse practitioners and Specialty trainees provide general outpatient general Paediatric clinics and also specialist clinics in neurology, respiratory, endocrine and diabetes, Cystic Fibrosis, gastroenterology, and neurodevelopment. Tertiary clinics are also provided locally by visiting Consultants from Glasgow (cardiology, nephrology, rheumatology, clinical genetics, cleft lip and palate and Paediatric surgery and urology). Cystic Fibrosis care is shared with the Lothian CF team.

Community Children’s Nursing Community children‘s nurses are attached to the department providing out of hospital care to children with chronic disease and life limiting conditions.

NHS Forth Valley has embraced the principles of GIRFEC (Getting it Right for Every Child) and the Early Years Collaborative that supports this approach to coordinating and integrating the services that provide for infants and children’s needs.

Community Child Health The department covers a population of 60,000 children under the age of 16 years. The population of Forth Valley lives mainly in the city of Stirling and several large towns (Falkirk, Alloa, , Bo'ness) as well as the rural areas of west Stirlingshire. The living standards in Forth Valley are high with small areas of severe deprivation. The region has diverse ethnic backgrounds, but with no established ethnic communities.

The department of community Child Health works closely with the education and social services in the three local authorities. The department of community Child Health operates within the combined Child Health service and offers the following services under supervision and co- ordination of Consultant Paediatricians. 3 Paediatric consultants, one Associate Specialist and 3 Specialty Doctors contribute to the delivery of Community Child Health Services at present.

Disability & Additional Needs Services for children with disabilities are provided from within multi-disciplinary teams that operated at different localities. No formal Child Development Centres are operational yet, but plans have been discussed. Services are also provided for neonatal follow up of babies with increased risk for disabilities and also for adolescents in transition to adulthood. Early assessment teams are in place for multi-disciplinary approach to children with suspected developmental problems. The department also works closely with National Children's Homes locally and the two children’s hospices in provision of services to children in respite care. 9

Specialist Clinics Services include children with constipation, encopresis, enuresis, neurodisability, neurodiversity, behavioural problems, autism, hearing impairment, visual impairment, botox assessments and therapy (ultrasound guided) and school health complex needs clinics.

Child Protection All Consultant Paediatricians provide early assessment and examination of children supported by nurse advisors. One Consultant Paediatrician takes the role of clinical co-ordinator for child protection and takes responsibilities for compiling agreed protocols and guidelines. This person represents the department in the Managed clinical network for the West of Scotland and locality Child Protection multiagency committees and groups. The Paediatricians have a close working relationship with Forensic medical examiners (FME) in Forth Valley. All children referred with concerns regarding possible physical abuse or Child sexual abuse are seen jointly by our Paediatricians with the FMEs. All examinations take place in dedicated forensic facility (The Meadows).

Child Death Reviews Preparations are underway to establish Child death review processes for all deaths of children and young people under 18 y of age (under 26 y for care leavers in receipt of after care) as of October 2021. Whilst ensuring multiagency learning and governance, minimal data set shall be reported nationally to allow collation of themes and extraction of learning Scotland wide.

Adoption and Fostering One Senior Paediatrician is designated medical advisor for each of the Local Authorities’ Resource panels and provides assessment and reports on accommodated children. The department also provides health and medical input to “looked after children” in local authorities' accommodation and also for the adoption and fostering services. A nurse-led service for Looked After and Accommodated Children has been developed and co-ordinates services with partners in the local authorities.

Relationships with external agencies Close relationships have been well established with all 3 local authorities (Stirling, Clackmannanshire and Falkirk), in particular Social work and education and the Police and other statutory and non-statutory agencies.

10 Mental Health Services CAMH services are based within the Mental Health Unit there are 4 Consultant Child Psychiatrists providing an area service, who also occasionally utilise in-patient Paediatric beds in conjunction with the Paediatric Consultants. Separate Paediatric clinical psychology staff are also based in the hospital, working in close association with child psychiatry. There is a close link with regional educational psychology services.

Relationship with other departments There is a close relationship with the Royal Hospital for Children, Glasgow, our principal tertiary referral centre for childhood haematology/oncology, neonatal surgery, cardiology and other supra-regional services. Close working and networking with this tertiary centre and with RHSC Edinburgh is encouraged.

The Consultants and Associate specialist are supported by 1 Specialty Doctor in acute Paediatrics and Neonates, 3 Specialty Doctors in Community Paediatrics, training grade Paediatric doctors in hospital and community services comprising Paediatric Specialty Trainees (ST1-8), General Practice Specialty Trainees (GPST2), Foundation doctors (FY2), and Clinical Development Fellows and 1 Advanced Paediatric Nurse Practitioner (APNP), 1 Enhanced Neonatal Nurse Practitioner (ENNP) and 2 Advanced Neonatal Nurse Practitioners (ANNP).

Non Medical Staff

The service is supported clinically by nursing staff and non-clinically by medical secretarial support, unit administrator and a dedicated Service Manager.

Training and Education

NHS Forth Valley places a high priority on training and development across all staff groups and strongly supports continuing medical education. There is a purpose built education service with a library, lecture theatre and meeting rooms with PACS and video-conferencing facilities.

Forth Valley Royal Hospital hosts the national Scottish Centre for Simulation and Clinical Human Factors. The most technologically advanced simulation centre in Scotland with permanent technical and administrative support. The centre runs national courses for a variety of specialties including Surgery, Paediatrics, Anaesthetics, Medicine and more. The centre also runs the national faculty development programme for simulation-based education.

Individual teaching in preparation for DCH and MRCPCH examination is offered to all trainees. Forth Valley has hosted the MRCPCH examination on an occasional basis.

Forth Valley has supported the training of midwives in examination of the newborn and has established the NES Scottish Maternity training course in Resuscitation of the Newborn.

The Scottish Centre for Simulation and Clinical Human Factors is collaborating with our department offering regular additional training opportunities in acute Paediatric and neonatal clinical management.

Training of senior neonatal and Paediatric nurses as extended or advanced neonatal/Paediatric nurse practitioners is underway.

There is a formal teaching programme in place for both acute Paediatrics and Neonates. In house Child protection Peer reviews and Community Child health Peer reviews are held on a two to three monthly basis. Attendance at external education events in CCH and Child protection is encouraged and supported.

Undergraduate Students from the University of Edinburgh are attached regularly for their Child Health block and students from University of Dundee attend for special study modules.

Staff are encouraged to participate in research relevant to their work both as lead investigator or contributing to multi-centre research and to support colleagues who are so engaged.

13 Contractual Details

The post holder will be professionally responsible to the Associate Medical Director, Women and Children’s Directorate and managerially responsible to the General Manager, Women and Children’s Directorate.

These posts are available on a full time basis or part-time basis (fixed term). The appointment will be subject to the new terms and conditions of service for Doctors in Training. Formal appointment to the post will be conditional upon satisfactory completion of all pre-employment checks, including satisfactory medical screening.

This post is offered on a fixed term basis for 12 months from 4th August 2021 to 2nd August 2022

Criminal Convictions - Protecting Vulnerable Groups (PVG) Scheme The Protecting Vulnerable Groups (PVG) scheme has replaced the previous disclosure arrangements for people who work with vulnerable groups.
