1. MODEL OF SERVICE

The base doctor (1st on-call) in general adult psychiatry will take initial responsibility for young people aged between 12 – 17 years of age *except for paediatric inpatients in Paisley and the Royal Hospital for Children. These paediatric facilities have direct access to the 2nd on-call in Child and Adolescent Psychiatry whatever the age of the patient.

The 1st on-call doctor will be provided with advice from the NIC in Skye House and supervision by the 2nd on call (higher trainee) in Child and Adolescent Psychiatry and Consultants in Child and Adolescent Psychiatry in and Clyde will provide a 3rd on-call.

Under 12 years old Greater Glasgow and Clyde

All referrals of children are taken by the 2nd on-call ST4-6 in Child and Adolescent Psychiatry accessed via the Royal Hospital for Children switchboard.

Children under the age of 12 referred by all referrers including GEMS, Social Work, Paediatrics at the Royal Hospital for Children site and at the Royal Alexandra Hospital in Paisley will be managed by the 2nd on-call in CAMHS.

Glasgow under 12’s will be seen at the Royal Hospital for Children and all Clyde under 12’s will be seen at Paediatrics at the Royal Alexandra Hospital, Paisley.

Referrals for clinical assessment will be undertaken by the 1st on-call at their base site. Referrals that present at Emergency Departments including those aged over 12 years at the Royal Hospital for Children (RHC) will be transferred to the sector psychiatric site for assessment by the 1st on-call.

If the patient cannot travel to the psychiatric base site then the 1st on-call will discuss with the NIC how best to proceed. This may occur if the young person has been admitted to the ward of the Royal Hospital for Children or a district general hospital and for Psychiatric/medical/surgical reasons cannot be transferred. This may require the 2nd on-call to undertake psychiatric assessment in the Royal Hospital for Children or the District General Hospital.

If the patient presents to an Emergency Department and refuses to travel or is generally non- compliant with regard to assessment, Emergency Department medical staff will make a clinical decision whether to discharge the patient without assessment.

If it is felt that it is unsafe to transfer a young person to the adult psychiatric hospital for assessment, the 1st and NIC should discuss this to find the best way forward. It may be that a psychiatric assessment by the 2nd on-call is required.

If the 1st On-Call requires a second opinion from the 2nd On-Call there will be a discussion between the 1st and 2nd On-Call as to where this will take place.

Note: 16 and 17 years olds living in Cardross and G84 postcodes will be transferred to an Adult Mental Health team responsible for these postcodes (this is due to the fact that Argyll and Bute HSCP CAMHS team will not be increasing the age range. West Dunbartonshire CAMHS have withdrawn service to Argyll and Bute as of May 2012.

1.1 REFERRAL CRITERIA

Patients from other Health Board areas are generally not accepted unless presenting to a Glasgow and Clyde Emergency Department or as a current inpatient. Referrers of patients resident in Glasgow and Clyde who present at sites in other Health Board areas should be advised to contact the CAMH Service for that area.

Referrals from other Health Boards may be considered if the referrer is a Consultant Child and Adolescent Psychiatrist seeking inpatient admission of a child or young person in an emergency. Inpatient units are hosted on behalf of national and regional commissioners. Consideration for admission to the inpatient units would depend upon the bed status and the nature of the referral and such referrals should be discussed with the on-call Consultant Child and Adolescent Psychiatrist for Greater Glasgow and Clyde.

Self referrals are not accepted. Referrals from staff at secure units are not accepted. Secure units should refer to GEMS.

Please note that we do not offer an Out-of-Hours service to Argyll and Bute patients but we will see any patient who presents to Inverclyde Royal Hospital Emergency Department Out- of-Hours.

1.2 REFERRAL CONTACT DETAILS

The Royal Hospital for Children switchboard will differentiate between under 12’s and 12–17 year olds - re-directing under 12’s in North and South Clyde to the Royal Alexandra Hospital and directing 12–17 year olds in Glasgow and Clyde to the appropriate base psychiatric hospital.

1.3 TRANSFER OF PATIENT PROTOCOL

A safe and efficient system to transport patients between sites for assessments has been agreed:

Under 12 Years Old Can only be transferred to the Royal Hospital for Children or Paediatrics Ward at the Royal Alexandra Hospital.

12 – 17 years olds Greater Glasgow and Clyde Most District General Hospital sites have no 1st on-call available and young people (over 12) presenting there should be transferred to the appropriate site as below. Hospital (Skye House) has a resident first on-call available.

All other patients who are transferred to Gartnavel Royal Hospital for assessment will be seen by the resident 1st on-call in general adult psychiatry for Gartnavel Royal Hospital.

The young person/carer will be told where to present to if a telephone call is made prior to presenting. Patients presenting with families or carers will be asked to make their own arrangements to travel to the appropriate site, unless there are clinical reasons to indicate that a Nurse escort is required. This is the responsibility of the referring clinician to determine.

For young people presenting alone who need to be transferred, the NIC from Skye House should be phoned and will take responsibility for organising a member of Nursing staff to collect and accompany the young person to the appropriate site (and only if the young person is agreeable to the transfer, is compliant with the assessment process and is not currently violent or aggressive - a single nurse escort would not be suitable in these situations). This service is for transfer only and is not for the purpose of providing a level of enhanced nursing observations for the young person once they have arrived at site of transfer. The transfer will be by taxi and will be accessed through either Ward 4, Royal Hospital for Children or Skye House budget code.

Detained patients will require a Registered Mental Health Nurse (RMN) escort for transfer and this will be organised by the NIC from Skye House.

1.4 BED MANAGEMENT SYSTEMS

If a bed is required the On-Call Nurse at Skye House will be contacted (via mobile number) to facilitate the location of a bed. In the first instance a bed will be sought at Skye House or Ward 4, Royal Hospital for Children depending on the age of the child or young person. If a bed is not available a bed should be sought within NHSGGC Adult Mental Health or if no local bed is available appropriate contact should be made with one of the other two Young Persons’ Units in .

The Consultant-on-Call should contact the CAMHS Manager On-Call (via switchboard) for verbal agreement to admit a patient to a bed within a psychiatric hospital outwith NHSGGC (Refer to On Call Management Rota). Out-of-Area Referral Form to be completed and submitted to the Clinical Director and Service Manager on the following working day.

The CAMHS bed manager should be updated/receive handover the following morning by the NIC.

1.5 SELF-HARM

N.B. Patients aged 12-15 who present at the Royal Alexandria Hospital after 18:30 hours should be transferred to the Clinical Decision Unit at Royal Hospital for Children.

Under 12 years old

Children should be admitted by the on-call paediatric service, either to the Clinical Decision Unit (CDU) at the Royal Hospital for Children for an evaluation the next working day by the Paediatric Liaison Psychiatry team (Royal Hospital for Children) or Renfrewshire CAMHS.

The responsibility for the transport of the patient to the NHSGGC Royal Hospital for Children will lie with the Royal Alexandria Hospital.

It is possible that some children will require re-evaluation over a weekend by the 2nd on-call doctor.

ICAMHS will see under 12 year olds at the weekend who have self harmed.

12 – 17 year olds in Greater Glasgow and Clyde

Patients presenting with self-harm in Greater Glasgow and Clyde between 9:00am and 18:30 Monday-Friday and between 9.00am and 17:00 Saturday-Sunday should be referred to the Intensive CAMHS Team.

Those presenting after 18:30 Mon-Fri and after 17:00 Sat/Sun aged under 16 years should be admitted to the Emergency Department at Royal Hospital for Children.

Patients under 16 years with presentations of self harm who present at The Royal Alexandra Hospital after 18:30 should be transferred to the Emergency Department at the Royal Hospital for Children for triage, psychiatric assessment if necessary and follow-up by ICAMHS the following day.

The responsibility for the transport of the patient to NHSGGC Royal Hospital for Children will lie with the Royal Alexandria Hospital.

16 and 17 year olds should be considered on a case by case basis and can be discharged home if deemed medically fit.

Other mental health presentations will require to be processed as per protocol.

All calls for Nursing assistance to go via the NIC in the first instance who can contact ICAMHS who are available to provide other psychiatric crisis support out of hours until 18:30 Monday- Friday. (or 17:00 Saturday/Sunday)

1.6 SUITABILITY OF UNITS FOR YOUNG PEOPLE

Some units within Greater Glasgow and Clyde where the young person will be assessed also provide services to adult psychiatric patients. It is important to assure the safety and protection of any young people who will attend an assessment at these sites.

Confirmation of suitability for assessment for young people from the sites has been obtained.

1.7 RHC PAEDIATRIC SUPPORT

Royal Hospital for Children colleagues in medical paediatrics or the Emergency Department may be asked for urgent medical support for Ward 4 patients when the Higher Trainee is already committed to another task and medical input is required. If there is a psychiatric emergency and the 2nd on-call is not available, the 3rd on-call will be called.

N.B. It is the responsibility of the hospital to which the patient has been admitted to organise Nursing cover (principle set out in NHSGGC Psychiatric Emergency Plan) unless the patient has been transferred to the hospital by Skye House in which case the responsibility will lie with Skye House.

It would be expected that a psychiatric assessment by the 2nd on call would be made in the first instance to determine the level of Nursing input required, unless this has been arranged in advance. If there are difficulties getting access to appropriate nursing input the On Call manager for GGC CAMHS should be contacted.

1.8 PSYCHIATRIC NURSING SUPPORT FOR PATIENTS IN GENERAL WARDS

If psychiatric nursing support for a child or young person admitted to a ward in district general is required it is the responsibility of the hospital to which the patient has been admitted to organise Nursing cover (principle set out in NHSGGC Psychiatric Emergency Plan) unless the patient has been transferred to the hospital by Skye House in which case the responsibility will lie with Skye House.

It would be expected that a psychiatric assessment by the 2nd on call would be made in the first instance to determine the level of nursing input required, unless this has been arranged in advance. If there are difficulties getting access to appropriate nursing input, the on call manager for GGC CAMHS should be contacted.

1.10 HANDOVER

1st on-call doctors should make sure that details of any clinical contact from the on-call period are given to the appropriate CAMHS team the next working day. The CAMHS Bed Manager should be informed of any Out of Hours child and adolescent admissions. The 2nd on-call can advise about this and decide together who will give handover.

The immediate handover will be verbal, with written information sent the morning of the following working day.

1.11 Communication - Out of Hours Service to Day Service Teams

Following an out of hours episode the on call band 6 NIC will ensure there is an appropriate handover to the duty person in the Locality CAMHS team the following morning in conjunction with the duty doctor All CAMHS teams operate a duty rota, where a member of staff is available to deal with urgent referrals. The band 6 NIC will, when required, contact and handover to the Intensive Community and Homebased CAMHS Service (ICAMHS) to pick up should this intervention, alongside Community CAMHS referral, be required. Self Harm cases should be referred to ICAMHS or to their local Tier 3 CAMHS team (if they are open to them) who should make contact with the patient the following day.

The band 6 NIC will complete a joint initial assessment to indicate the need and interventions required and scan the information on EMIS together with a risk assessment and initial safety plan to the relevant CAMHS HSCP service or Hosted Team. This information would also be accompanied by an explanatory phone call from the day shift NIC who would take over at 7.15am. Information can also be handed over by email. ICAMHS referrals should be emailed to ICAMHS administration support and copied to ICAMHS team lead and discussed further with the ICAMHS duty nurse at 9am. The NIC will not be expected to create a new EMIS record if one has not already been created. New cases will be scanned and emailed to the relevant Duty person.

1.12 REFERRAL INFORMATION

The 1st on-call should seek advice from the NIC and seek supervision from the 2nd on-call if required. The 1st on-call doctors must gather the information below before contacting the 2nd on- call to discuss assessment. This service provides an emergency social care service for the 13 Local Authorities in the West of Scotland and provides such a service for all care groups, predominantly child care/child protection, mental health and frail adults, along with a number of other duties.

When requesting a Mental Health Officer from the service, the senior manager on shift will make the decision to release a Mental Health Officer whilst considering other statutory competing priorities and the expectation would be that an estimate of response time would be given.

It will be the duty of the Mental Health Officer from Standby, in the case of a detention having been made, or otherwise, to advise the relevant Social Work Area Office when office hours are resumed, of the intervention in each case and resulting outcome and requirement for further action. This information will be sent to the relevant Operations Manager for Mental Health in the relevant Area Team for Glasgow, and to the local office in the case of the other 12 Local Authorities.

In Glasgow the community sector child and adolescent teams all operate a Duty System whereby there is an available clinician on duty during office hours to take emergency phone calls. If required the Duty Worker will access the team consultant and if they are not available then there is a Greater Glasgow integrated child and adolescent day time rota which can be accessed by the community teams.

The CAMHS Tier 4 hub and Paediatric Liaison have internal duty systems relating to their own specialist group of patients.
