1. Forms of nomination for Parish Elections may be obtained from Clerks to Parish Councils or from the Returning Officer, 131 High Street, Needham Market, Ipswich, Suffolk, IP6 8DL.

2. Nomination papers must be delivered to the Returning Officer, 131 High Street, Needham Market, Ipswich, Suffolk, IP6 8DL on any day from the date of this notice, on Monday to Friday, 9 am to 5 pm (excluding bank holidays) but no later than 4 pm on Thursday, 9 April 2015.

3. If any election is contested the poll will take place on Thursday, 7 May 2015.

4. Applications to register to vote must reach the Electoral Registration Officer, 131 High Street, Needham Market, Ipswich, Suffolk, IP6 8DL, by 12 midnight on Monday 20 April 2015.

5. Applications, amendments or cancellations of postal votes must reach the Electoral Registration Officer at 131 High Street, Needham Market, Ipswich, Suffolk, IP6 8DL by 5 pm on Tuesday, 21 April 2015.

6. Applications to vote by proxy at this election must reach the Electoral Registration Officer at 131 High Street, Needham Market, Ipswich, Suffolk, IP6 8DL by 5 pm on Tuesday, 28 April 2015.

7. Applications to vote by emergency proxy at this election on grounds of physical incapacity or for work/service reasons must reach the Electoral Registration Officer at 131 High Street, Needham Market, Ipswich, Suffolk, IP6 8DL by 5 pm on Thursday, 7 May 2015. The physical incapacity must have occurred after 5 pm on Tuesday, 28 April 2015. To apply on the grounds of work/service, the person must have become aware that they cannot go to the polling station in person after 5 pm on Tuesday, 28 April 2015.
