Leadership Message

St. Joseph’s Medical Clinic Celebrates 40 Years! We are proud of our 40 years together serving uninsured, underinsured, vulnerable, and low-income community members in Waukesha County. St. Joseph’s Medical Clinic is a critical member of a nation-wide network of approximately 1,200 free and charitable clinics. We are the longest- standing free clinic in the state of WI. In our 40 years, health inequity within our community continues to evolve affecting individuals and families. The average household income of patients is $10,000 - $11,000, though the majority of patients come from working households. Often times, a free clinic is the only available resource for working families to access primary care. Our goal is to provide comprehensive healthcare through a multi-disciplinary approach including primary care, specialty care, medications, labs and diagnostic testing, and health education. For four dec- ades, we have successfully achieved our mission in reducing health inequity, unnecessary Emergency Department visits, and preventable hospital stays. Patient Healthcare as We Move Forward It seems that everyone is talking about the future these days. With so many changes within our nation and across the globe, it is hard to say what the future holds with regard to healthcare. Certainly, what is clear to the leadership team of St. Joseph’s Medical Clinic is that currently there is at least 5% or over 20,000 community members without access to a doctor, medications, and education on medical condition(s). Our clinic’s patient population is not only increasing in size, but also in medically complexity. Many individuals who have secured primary care at St. Joseph’s Medical Clinic are seeking information about how to take appropriate care of their medical conditions. Our programming requires comprehensive patient triage and assessment processes, and health education opportunities to increase overall health literacy and knowledge of healthy lifestyles. As a result, we have arrived at certain key decisions:  A change in fiscal year to a 07/01-06/30 cycle to better align our programming objectives with grant and funding opportunities  The commencement of strategic planning to reinforce our value in being a responsive and responsible safety net provider addressing the healthcare gaps of today, and also tomorrow’s healthcare challenges  The opportunity to relocate to a new facility to expand and nurture our services toward the overall strategic vision of delivering healthcare in a rapidly changing environment In 2017, we kicked off our first Capital Campaign to help our organization move forward in securing a new facility. In 6 months, many of you helped us raise over $325,000 towards this goal. In celebrating our 40 years, we achieved our most successful annual gala yet by raising nearly $115,000. The healthcare landscaping may be changing in the future, we are moving along with it and on the ready to ensure that all community members have access to medical care. Standards of Excellence Award In November 2017, St. Joseph’s Medical Clinic was awarded the Bronze Provisional Seal of Excellence for achieving many “standards” outlined in the new “Standards of Excellence” accreditation process for WI’s free and charitable Clinics. The “Standards of Excellence” are developed and administered by the Wisconsin Association of Free and Charitable Clinics (www.wafcclinics.org). We applaud this achievement, and commend our Board of Directors, Staff, and volunteers for helping us obtain this goal. Volunteers Serve with Kindness and Heart Patients often times ask about the volunteers who are here to help them. We are grateful for the 150+ volunteers that help us deliver our pro- gram targets, fundraising goals, and administrative tasks. With nearly 2/3rds of our patients chronically ill with multiple medical conditions, providing health and wellness programs under our “Continuity of Care” model could not be effectively achieved without the dedication and com- passion of extraordinary volunteers. Together, We Make Waukesha County a Beautiful and Healthy Place There is no doubt that each of us is proud of the Waukesha County community. It is a wonderful place to live and work, and there are many like- minded, compassionate, and energetic individuals committed to stewarding our community. St. Joseph’s Medical Clinic remains thankful to many of you who donate and support our mission. You take those tender steps each and every day in helping us keep Waukesha County healthy. Programs and Patient Information

Healthcare Services

SJMC is a free clinic providing medical care to friends and neighbors who live, work, or attend school in the Waukesha County community. Our services include provider assessments, diagnosis and treatment planning, patient health education, labs and x-rays, medications, and health insurance navigation and enrollment assistance. We offer a once per month podiatry clinic primarily serving diabetic patients, a once per month women’s health clinic and a once monthly physical therapy clinic.

SJMC is a member agency of the Housing Action Coalition/Continuum of Care in Waukesha County. We have on staff a Registered Nurse acting in the capacity of Medical Case Manager to outreach to individuals and families who are homeless or at risk of homelessness. As of December 2017, SJMC’s Medical Case Manager encountered close to 130 individuals homeless who were marginally housed, in shelter or living on the street. Our services assist approximately 130+ individuals with medical care, patient health education, medications and referrals to gain housing, transportation, and food. 6% of those we serve are in shelter. Community Outreach Nursing in Waukesha

In 2017, SJMC furthered its partnership with The Salvation Army Waukesha by introducing a pilot program in Outreach Nursing. This program provides basic medical triage and health assessment for community members. The St. Joseph’s Medical Clinic Outreach Nurses are on-site once per week during evening meal time. Due to the success of the program, we are moving forward in facilitating a full service program to provide nurse triage, medical evaluation, and referral to SJMC’s “Continuity of Care” program and other community benefits programs. This helps forward the goal of providing all community members access to primary care.

Patient Demographics

SJMC was particularly inspired by a 62 year old man, who had no in- come, no insurance and was living at the Salvation Army shelter. He came to the clinic initially seeking wound care for his leg. As an un- medicated diabetic, SJMC assisted him with getting back on his medi- cation regimen, coordinating his care with the Waukesha Memorial Hospital Wound Care Unit and were able to avoid amputation of his leg. He continues as a patient of SJMC, coming in for regular follow- ups, continuing education and social-emotional support.

Before patients may receive healthcare services, we review patient household income and if they have insurance. Generally, we assist patients with household incomes up to 300% of Federal Poverty Level (FPL). For example, in 2017 a family of two may have household income of $48,720 per year or $4,060 per month before taxes. When considering the rising cost of housing, food, transportation, and child care; this is not much left at the end of the month.

The 2016 United Way ALICE Report1 measured in WI by county the number of households at or below poverty level BUT below cost of living. The Waukesha County snapshot illustrates that 6% of Waukesha County community members live at or below pov- erty level and 23% of households are considered “ALICE” or Asset-Limited, Income-Constrained, Employed. In other words, a household is above poverty level but continues to struggle affording and accessing basic needs. The majority of SJMC’s patients are from working households. From 2016-2017, the average household income of patients was $10,944 per year or approxi- mately $210 per week.
