Chapeltown General Practice Chapeltown Chapeltown is a vibrant energetic area of Leeds, set just north of the city centre. It is home to a mixed residential community with a strong history of community & citizen engagement, & partnership working. There are 7 GP practices in the NHS administrative area of Chapeltown, part of Leeds North CCG with a joint patient list of 48,000. There are many committed third sector organisations working in the area plus the Reginald Centre is the Council’s community hub. Chapeltown is home to Hospital, the Northern School of Dance, & the annual Leeds Carnival. The changing local population over time is demonstrated by the notable religious buildings including Polish & Irish Roman Catholic Churches; Anglican, Pentecostal, Methodist; Sikh Temple; Greek Orthodox; Central Jamia Mosque. The diversity of people living in the area makes clinical work varied & interesting; there is always something new to learn. We recently organised a staff networking event (see picture): social workers, OTs, nurses, mental health workers, GPs, among others, came together to celebrate working in Chapeltown. One said: “I was inspired by the passion of staff working in the area & the variety of knowledge, skills, experience”.

What is Chapeltown General Practice? It is an exciting time to be working in General Practice in Chapeltown as we engage with the changing NHS & its opportunities, to better serve our patients. Our 7 practices have committed to work together in a spirit of co-operation, openness & communication, focused on results & pragmatism – as set out in our shared Memorandum of Understanding. We have locality lead GP, Practice Nurse, Practice Manager, with facilitation from Leeds North CCG. We have successfully developed new shared services in diabetes & mental health, & developed good working relationships between practices & with other local health & care providers. We have been awarded £30,000 from NHS resilience funds to develop our network.

Patient profile – what are the care challenges?  Long Term Conditions: 7.6% of the population of Chapeltown have diabetes (Leeds average 4.8%). Diabetes prevalence is high & increasing.  Child obesity: 20.6% of Chapeltown children recorded as obese (Leeds average 19.4%). Chapeltown & Best Start Zone has the highest prevalence of obesity at reception age in Leeds at > 12% (Leeds average of <10%, the lowest in the city is 6%).  34% of households have multiple dimensions of deprivation, 13% of population has lived in UK for less than 10 years. 40% of housing stock is rented.  28% of the population belong to Black, Caribbean or African ethnicity groups. 27% of Leeds’ Caribbean population live in this area. There is a notable South Asian community & a 19% population of Muslims. There is an above average population identifying as “white other”.

We have a variety of roles/hours/locations to offer and will work with the right candidates to find the best work/life balance possible.

We are a group of surgeries looking for Practice Nurses with a can-do attitude and an appetite for doing things differently. This is an exciting new opportunity for a Practice Nurse(s) to work across a locality rather than for a single practice. Primary Care is changing as the NHS is transforming the care that is being delivered in the community rather than in hospital. The local practices are working together to deliver a shared vision for the local community. We hope that by working in this way we will help facilitate the sharing of best practice and the further development of a peer support network among practice nurses that will lead to better development and job satisfaction for the locality nursing team which will have the added benefit of delivering enhanced care for patients and local community.

You will be someone who has the desire to be part of a vibrant and forward-thinking locality, working across small teams to innovate new ways of delivering primary care. You are someone who relishes the opportunity to use your initiative and deliver your ideas. Right now, we don’t quite know what this might look like. It could involve running clinics in more than one practice if you have a specialism we need or you might help support and develop nursing colleagues across the locality. The most important thing is that you are happy to work with us to try some these ideas out and develop the ones that work best.

In order to deliver this we are looking for someone who is not only a good nurse, passionate about delivering good care, but someone who is keen to identify innovative ideas and to promote and share these across the district. Working across different practices and with different teams will need a confident yet flexible approach and someone who is self- motivated and reliable with a strong patient centred and work ethic. Communication with colleagues and managers will be key. This way of working is new and is likely to evolve over time and there will be an opportunity for you to help shape the future of this approach.
