Request of Citizenship by marriage
I the undersigned ______, holding the ______nationality, wife of the Lebanese ______, Registry No. /______/______, District ______. Considering that our marriage is registered in Lebanon for more than a year, I am presenting this application to acquire the Lebanese nationality by marriage, and I kindly ask you to send my request to the Lebanese directorate of civil status.
Please attach the following original documents with 2 photocopies of each one: 1- Family civil registration (recent Ikhraj kayd) to show that the marriage was registered in the Lebanese civil census for more than one year. 2- Copy of the wife's passport valid for more than one year with the husband's name added on it. 3- Wife's birth certificate (translated to Arabic in case it is not in English or French). 4- Lebanese or Canadian marriage certificate if available. (You need to make sure that the wife’s place of birth is registered in the same way on her Birth certificate, Marriage certificate and Passport). 5- Husband's Lebanese passport if available. 6- Certificate of cohabitation issued by an official and religious authority. 7- The amount of $20.00 for the application. * Fees should be paid by money order payable to the Embassy of Lebanon (if by mail) 9- Commitment from the person attesting that she is not Lebanese and her father is not also (Annexed hereto). 10- 2 prepaid (registered) self addressed envelope. 11- For Syrian Citizen: An extract of personal registration (Bayan Kayd sealed and stamped by the Ministry of Foreign Affairs of Lebanon), with date of issuance not exceeding six months, including husband’s name and nationality and a photocopy of the Syrian Identity Card. 12- For Palestinian Citizen: An extract of personal registration (Bayan Kayd), with date of issuance not exceeding six months including husband’s name and nationality and a photocopy of the Blue Card including husband’s name and nationality.
N.B.: In case of not being able to come to the Embassy, all documents should be signed in front of a Notary Public and legalized by the Canadian Department of Foreign affairs.
