The goods and services shown within are those we can provide to our customers. You may choose only the items you desire. However, any funeral arrangements you select will Include a charge for our basic services and overhead. If legal or other requirements mean you must buy any items you did not specifically ask for, we will explain the reason in writing on the statement we provide describing the funeral goods and services you selected. The list does not include prices for certain items that you may ask us to buy for you, such as cemetery services, flowers and newspaper notices. Parke's Magic Valley Funeral Home began in 1997, when Mike and Catherine Parke founded the Funeral Home. In 1998, the Parke's purchased the historic Twin Falls Cemetery. The Parke family operated the funeral home and cemetery for 10 years. In the summer of 2007, Mike and Catherine sold the funeral home due to Mike's failing health. In Septem- ber of 2008, Mike received a liver transplant and began recovering. In May of 2009, the Parke's repurchased the funeral home. The Parke Family is proud to once again be able to assist families in the Magic Valley. Memorial Jewelry. We can arrange for a small portion of the cremated remains, a lock of hair, or earth from the grave site to be enclosed in a decorative piece of jewelry or other remembrance item.

Military Honors. To honor a person's service in defense of our freedoms, we frequently arrange for our nation's veterans to be remembered with special military observances. Depending on the branch of service, military detachments may provide a color guard, musicians, gun salute, ceremonial flag fold and presentation.

Memorial Release. A beautiful and symbolic way to say goodbye and remember the life lived. Releases can be part of a memorial or grave side service.

Stationary and Memorial Folders. As a lasting tribute to your loved one, we offer personalized stationary packages including items such as memory folders, thank you cards, and prayer or remembrance cards.

Invite Friends to Speak. You might ask one or more friends to offer special recollections, or read some of the notes or poems written by other people attending the service.

Create a Memory Table or Memory Board. A display of personal items or photos help people understand and appreciate the life that has been lived.

Video Tribute. A montage of family photos set to appropriate musical accompaniment can be a perfect enhancement to a visitation, funeral or memorial service.

Special Music. Everyone has a special song or piece of music that carries deep associations and memories. Special musical selections can enhance a service in a way that evokes a certain emotional response. You may wish to have recorded music, or even a live performance.

Themed Services. Whether her favorite pastime was gardening, or he was a military veteran, the service can be themed with special props and articles that truly reflect the person and the life. Our firm even offers a horse drawn hearse for those who desire the utmost in traditional services.

Living Online Tribute. We also create a living memorial to your loved one on our website. This memorial includes the life tribute as written by the family, in addition photos can be loaded on to the site along with video and audio tributes. When a death occurs, your primary responsibility is to yourself and the ones you love. There are a number of tasks which require attention simultaneously. Regardless of the day or hour, Parke’s is always prepared to respond to your needs quickly and competently. The funeral director’s main function is to assist you with the necessary details of the funeral process. Among the services funeral directors provide are:

1. Professional Services of Funeral Director and Staff a. Services of the Funeral Director and Staff...... $1,495.00 This is our fee for our basic services and overhead and will be added to the total cost of the funeral arrangements you select. This charge includes but is not limited to, arrangement conference; coordination with cemetery, clergy, newspapers and/or crematory for final disposition of the deceased, procurement, preparation and filing of necessary notices to newspapers and state agencies; general administrative and record keeping expense; and general overhead expenses, such as maintenance, equipment, inventory and insurance costs and governmental compliance. This charge is included in all of our package offerings. It does not include fees for outside vendors, such as newspapers, florists, state or county death certificates, cemeteries, or any other vendor we may purchase additional items for you or your family.

Affordable Funeral Options Direct Cremation…………………………………..$1295.00* This is a special discounted package available to assist those families who would like to handle their own service details and are willing to arrange for all services and forms via the internet. In this package the only time you will meet with a funeral director or their staff is when picking up the cremated remains after cremation. YOU MUST FILL OUT ALL DOCUMENTS VIA THE INTERNET. NO ARRANGEMENT CONFERENCE WILL BE HELD. You will order your own death certificates and file your own insurance and obituary notices. THIS IS A VERY LIMITED SERVICE PACKAGE. All charges will be paid via credit card on the internet. Removals will be preformed during regular business hours. Extra charges will apply for after hours removals and removals requiring more than one staff member. There will be no additional services or additions allowed in this package. The package includes weekday 8 a.m. to 5 p.m. removal, refrigeration of the remains until all legal authorizations are completed by the coroner and physician, the cremation process, basic cardboard cremation container, and basic cardboard urn. Not included in this package are items such as keepsake urns, memorial book, service folders, acknowledgement cards, obituary, death or service notices, death certificates, viewing of any kind, or any memorial or funeral services. This service package is not available on a pre-need or pre-paid basis prior to passing. The Functions of an Urn Vault

If you choose burial after cremation, it’s important to understand why you need an urn vault and that they do not all provide the same protection. There are several factors that challenge the integrity of an urn vault. Heavy Equipment necessary for maintaining the cemetery grounds may pass over the urn vault during cemetery operations. Ground water, too, is an important factor; A lined urn vault is constructed using high strength concrete. Lined urn vaults are reinforced in one of two ways; with durable plastic or with a combination of durable plastic and metals. 1. Gather vital statistic information. This should include such items as: Date and Place of Birth, Parent’s Names with Mother’s Maiden Last Name, Parents Birth Place, Social Security Number, VA Discharge Papers, Date and Place of Marriage, and a complete list of survivors and the cities in which they reside.

2. It is also recommended to visit with other family members to compile a list of lifetime accomplishments of your loved one. This list should include clubs and organizations, career accomplishments and hobbies your loved one enjoyed.

3. A photograph of your loved one is also helpful for our staff hairdresser and may be used in the local newspaper notices.

4. Clothing will need to be considered. The Funeral Home suggests bringing clothing that you would feel comfortable viewing your loved one in. We request all the usual garments, with the exception of shoes. These are a personal preference item.

5. You should gather all life insurance policies before the arrangement conference. The funeral director and staff can assist you in filing your claims for these policies.

Once these items are secured, you will be ready for your arrangement conference. It is customary to contact the funeral director and set up an appointment time. The most important thing to remember at this time is to take time for yourself, do not rush to make any decisions. Your funeral director can and will help put all the details together for you. A one-time payment of $255 may be payable to the surviving spouse if he or she was living with the beneficiary at the time of death, OR if living apart, was eligible for Social Security benefits on the beneficiary's earnings record for the month of death. VIDEO TRIBUTE INSTRUCTIONS

We at Parke's Magic Valley Funeral Home are happy to create a meaningful video tribute of your loved one to all families who choose our services. We have set the following guidelines for the video tribute. These guidelines assist both us and the family in properly preparing a meaningful tribute. We are sorry but these guidelines must be followed to successfully produce the tribute in a timely fashion.

1. We recommend that you have between 35 - 120 photos for the tribute. Previous videos have proven that if more than 120 photos are used that you lose people's interest in watching the video.

2. It is recommended that one standard length song for each 30 - 40 photos. We are trying to have the photo appear on the screen for a minimum of 4 seconds to a maximum of 8 seconds. Time has proven that the retention level of the audience fits this timeline. Please pro- vide the name of the song and the artist you prefer. Every attempt will be made to provide that music. If we do not have that version in our library, or cannot access it, we will contact you for an alternative.

3. Photos must be brought to the office in a format that can be scanned. Scrapbook pages are not acceptable, as they are often too large to scan and often contain multiple images. Regular single photos prove to work the best. All photos must be removed from the frames, scrapbook or photo albums, we will not remove photos, as damage beyond our control may occur. Computer printed photos do not scan clear and the image will degrade. We also can receive photos via e-mail, in a JPEG format, at [email protected]. These emailed images will be placed at the end of the scanned images, as they can not be sorted into the scanned images. It will not be possible for us to move the image order, once scanned into our system. So please number and stack photos in the order that you want them to appear, if order is important to you. Time has proven that people are more tuned into the video when the photos are in a random order. It is not possible to edit scrapbook pages or change the order of images.

4. In order for us to create the video timing is very important. The images and music must delivered on time! If the images are not delivered on time, or to add additional images once the initial video is produced, there will be an additional charge of $150.00. (NO EXCEPTIONS!)

5. Parke's will produce and give your family three courtesy copies of the video. You can copy them as you wish, or purchase additional videos from the funeral home at a charge of $15.00 per video.
