Founded in 2010, The Kate Bennett Design Co. is an Oahu, Hawaii, full service event planning company that provides complete consulting services for , as well as private and corporate events. Kate Bennett has been in the industry for roughly ten years. She holds a Bachelor’s of Science in Marketing with a focus on Event Planning and has recently become certified as a Wedding and Event Planner from the Lovegevity Wedding Planning Institute. She intends to continue her education to become a Master Planner in the near future. Kate is a vibrant, full-of-life, planner that absolutely loves making her clients dreams come true. DAY OF SERVICE CONTRACT

Dear [ and Groom],

This letter follows our meeting on [Month, Day, Year], during which we discussed your wedding, and my professional role in helping you plan it. It is my understanding that, you will retain me as a Professional Wedding Consultant and Wedding Day Coordinator for your wedding scheduled on [Wedding Date].

Description of Services:

As Wedding Day Coordinator, my duties include:

 Visit to both and reception sites prior to wedding  3 consultations with client via telephone/email  Development of a detailed wedding timeline and floor plan for contracted vendors and bridal party  Follow up telephone calls to all contracted vendors 1 to 2 weeks before wedding day  Wedding Rehearsal Supervision (3 hours maximum)  On-site coordination and supervision at the ceremony site and during the reception for up to 8 hours on the day of your wedding  An additional event manager, on site, the day of the wedding

Please note that the services listed above do not include running any wedding related errands such as picking up or delivering attire, supplies or equipment, documents, etc. As the client, you will rely on me to work as many hours as may be reasonably necessary to fulfill my obligations under this agreement.

Pricing:

Wedding Day Coordinator Service Package - $575

Changes/Cancellations

Any changes made to this service contract must be made in writing and signed by all parties. You may cancel this agreement, in writing, for any reason. If the wedding is canceled, refunds are limited to unearned fees, funds in excess of unused or non-refundable fees and out-of-pocket expenses. If you cancel less than 30 days before the wedding – except for the death of a member of your immediate family – there will be no refund. If the wedding is not canceled, there will be no refund.

Acts of God

If an act of God, such as a fire, flood, earthquake, or other natural calamity shall cause you to cancel your wedding; I will require payment only for the time actually spent planning your wedding.

Fees & Payment Schedule:

For “Day Of” services, the client will pay the wedding day coordinator a total of $575.00.

Payment will be made as follows:

A non-refundable retainer in the amount of $125.00_ due upon signature of service contract.

$225.00_ due on ______(Date) (thirty days prior to wedding date)

$225.00_ due on ______(Date) (ten days prior to wedding date)

Term/Termination:

This agreement will terminate automatically upon completion of the services required by this service contract

Conditions:  I understand that my role will be that of advisor and coordinator. The client will make the actual selections of service providers and I will implement those selections.

 The client will make payments directly to the service providers/vendors and not to me. I do not accept any commissions from recommended vendors and cannot guarantee any service provider’s performance or product. If litigation occurs, it occurs in the jurisdiction where my office is located and the winning party will be reimbursed for attorney and legal fees and court costs.

 It is your responsibility to provide me with contact names, telephone numbers, and any scheduled timetables for all service providers involved in the wedding ceremony/ reception no later than 14 days prior to the wedding or upon the signing of this service contract.

 It is also your responsibility to notify me of any changes in a timely manner. I shall not be held liable for any changes made by you or your selected service providers.

 I will use my professional judgment when taking action concerning changes, weather, tardiness, nonperformance, etc. based on the situation, time limitations, and/or your wishes.

 In the event a venue coordinator is on site, I will work with you and the coordinator as needed. If your understanding parallels mine, please sign one copy of this letter and return it to me along with your payment in the amount of $125.00.

Mini Planner Package:

The Mini Planner Package is intended for the couple 4 to 6 weeks prior to their event, who have planned their wedding, but desire the assistance of a professional coordinator to execute their special day. The Mini Planner Package covers the responsibilities of managing the wedding, while executing the intended plan flawlessly. This includes a preliminary consultation, wedding design advice, vendor supervision and onsite and wedding day coordination. The Mini Planner Package includes the following:

The Wedding Coordinator and Wedding Planner Assistant will ensure that:

 The bridal party has refreshments during the preparations;  The bridal party will arrive on time (or fashionably late!);  The ceremony flowers are ready and setup; service booklets in place; musicians are ready and in place;  Boutonnieres are pinned correctly for groom’s party;  Prepare the bride and for the processional;  Transfer bride’s luggage to hotel and check-in;  Check on setup at the Reception and that suppliers are ready on time;  Work with venue to meet and greet guests on arrival;  Ensure bridal room is ready with refreshments;  Liaise with MC or venue re seating of guests;  Prep wedding party for their formal entry to the Reception;  Prompt MC to announce wedding party;  Liaise with MC, venue and speakers re speech announcements, cake cutting and .  Ensure that the venue transfers gifts and remaining to hotel room or to an appropriate family member.

Marriage License Information

For any person to lawfully marry in the State of Hawaii, a license for that purpose must be obtained from an authorized agent. Once the license has been issued, there is no waiting period before the marriage can take place.

A marriage license and marriage certificate are different documents. The marriage license permits a marriage to take place. The marriage certificate indicates that a legal marriage has been performed.

The marriage certificate is prepared and filed by your licensed marriage officiant or performer with the Department of Health. A newly married couple receives one (1) certified copy of the marriage certificate at no extra charge, upon payment of the marriage license fee, which will be sent to the couple by mail (60-120 days) after the marriage has been performed. If a marriage certificate is needed sooner than this, you can apply for one or more additional certified copies.

Marriage performers must be duly licensed by and in the State of Hawaii to perform the marriage ceremony. If desired, you may request the marriage performer to present his/her license for your inspection.

Getting Marriage License Information by Telephone : Information on marriage licenses may be obtained via the telephone system, any day or any time, by calling (808) 586-4545.

Who is Eligible to Apply for a Marriage License:

 There are no state residence or U.S. citizenship requirements.  Blood tests are not required.  The legal age to marry is 18 years for both applicants.  Applicants of 16 or 17 years of age who wish to marry require the written consent of both parents, legal guardian, or the family court. The parents or legal guardian do not have to be residents of the state. Consent forms may be obtained from a marriage license agent.  Applicants of 15 years of age who wish to marry require the written consent of both parents or legal guardian, and the written approval of a judge of the family court. The parents or legal guardian do not have to be residents of the state. Consent forms may be obtained from a marriage license agent.  Proof of age is required. A certified copy of a birth certificate must be presented for anyone 18 years of age or under. A valid I.D. or driver’s license may be presented for anyone 19 years of age or over.  If previously married, proof of original divorce decree or death certificate needs to be presented to the marriage agent by the applicant if the divorce or death was final within 30 days of applying for a marriage license.  Cousins may marry. However, the blood relationship between the prospective applicants cannot be closer than first cousins.

How to Apply for a Marriage License

The prospective applicants must appear together in person before a marriage license agent to apply for a marriage license. Proxies are not allowed.

The prospective applicants should be prepared to provide the necessary proof of age and present any required written consents and approvals. All of the necessary documents should be obtained prior to applying for a marriage license.

The prospective applicants must prepare an official application and file the application in person with the marriage license agent. The application may be completed online (see website below). The application will not be accepted if sent by either postal mail or e-mail.

Upon approval, a marriage license is issued at the time the application is made.

License Fees:

The marriage license fee is $60.00 (plus $5.00 portal administrative cost). The fee may be paid online or in- person at the time the application is submitted to a marriage license agent.

The marriage license is good only in, but is valid throughout, the State of Hawaii.

The marriage license expires 30 days from and including the date of issuance, after which it automatically becomes null and void.

Civil Union Information

Who will be eligible to apply for a civil union? Residency: There will be no state residence or U.S. citizenship requirements. Blood Tests: Blood tests will not be required. Age: The legal age to enter a civil union will be 18 years or older for both males and females.

Civil Union/Domestic Partnership: The new law establishes all unions entered into in other jurisdictions between two individuals not recognized under Hawaii’s marriage law shall be recognized as civil unions beginning January 1, 2012, provided that the relationship meets the eligibility requirement of Hawaii’s civil unions chapter, has been entered into in accordance with the laws of that jurisdiction, and can be documented. Those already in a domestic partnership or civil union in other jurisdictions who want to enter a civil union (either with another person than they are united to in the other jurisdiction or in a ceremony conducted by a Hawaii civil union performer) must first terminate the domestic partnership or civil union.

Marital Status: If previously married, proof of termination of that marriage must be presented to the civil union agent by the applicant if the divorce or death was final within 30 days of applying for a civil union license. Proof consists of a certified original divorce decree or a certified death certificate. Other credible proof of termination may be accepted at the discretion of the DOH. NO FAXES ARE ACCEPTED.

Blood Relationship: A civil union shall not be entered into and shall be void between the following persons: parent and child, grandparent and grandchild, two siblings, aunt and nephew, aunt and niece, uncle and nephew, uncle and niece, and persons who stand in relation to each other as ancestor and descendant of any degree whatsoever.

Where can I obtain my civil union license and where can I have my ceremony performed?

You may obtain your civil union license from an agent authorized by the department to issue civil union licenses. Agents are located in each judicial circuit in the state. Once you have obtained your license you can have your civil union ceremony anywhere in the state, subject to applicable property usage limitations.

Who will be eligible to perform or officiate a legal civil union ceremony?

A civil union shall become valid ONLY UPON COMPLETION of solemnization by a person LICENSED BY THE DEPARTMENT OF HEALTH IN ACCORDANCE WITH section 572B-4, Hawaii Revised Statutes. Couples are advised to verify that the person who will be performing their civil union ceremony is licensed. Couples are advised to ask to see the solemnizer’s license before attempting to solemnize their union.

What are the steps for getting a civil union?

There is a three-step process: First, you must apply for a civil union license. The license permits a civil union to take place. Second, you and your partner must appear in-person before a civil union agent to receive your license. Third, once you receive your civil union license, your legal civil union must be performed by a licensed civil union performer or officiant.

How do I receive a civil union license?

First, a civil union application must be completed. The application may be completed and printed online. The civil license application form is available in PDF format (see link below).

Both prospective partners in the civil union must appear together in person before a civil union agent to submit their official civil union application for a civil union license. Proxies are not allowed.

Applications will not be accepted if sent by either postal mail or e-mail.

The prospective partners may only obtain a civil union license from an agent in the county in which the civil union is to be solemnized or in which either prospective partner resides.

The prospective partners should be prepared to provide the civil union agent with the necessary proof of identification and age and present any required written consents and approvals. All of the necessary documents should be obtained prior to applying for a civil union license and appearing before an agent. A valid government issued photo I.D. or driver’s license may be presented.

Upon approval, a civil union license will be issued at the time an application is made.

After receiving my civil union license what will be the next step for registering my civil union with the Department of Health (DOH)?

The Civil Union Law becomes effective January 1, 2012. Civil union performed by a licensed officiant on or after January 1, 2012 will be registered by the DOH.

When you submit your application for your civil union license, your civil union agent will provide all the information you may need to complete the process for legally recognizing your civil union in Hawaii.

Once your civil union license has been issued, your ceremony may take place within 30 days of your license issuance or before the expiration date. You must have a civil union officiant licensed by the DOH perform your ceremony.

After completing the ceremony on or after January 1, 2012, the civil union officiant will record the event online with the DOH and, after DOH reviews and approves the information, your civil union will be registered.

Once the officiant enters the ceremony information into the system and it has been reviewed and accepted by DOH, a temporary on-line certificate of civil union will be available to you for a limited time period. When your on-line certificate is no longer available, you may request and obtain a certified copy of your certificate from the DOH by paying applicable fees

The Department of Health does not provide legal advice on benefits or entitlements. For information regarding benefits or entitlements, consult with your legal counsel, employer, provider, or related government or private agencies that address these issues.

The Department of Health can provide certified copies of legal records to verify civil union status in Hawaii to those who are entitled to receive copies pursuant to state law. Certified copies of vital records are acceptable for legal and government transactions.
