Prime Minister’s Challenge Fund & Multi-speciality Community Providers (MCP) Vanguard Project

Background West Health & Wellbeing (known as ‘West Wakefield’) is a GP Federation formed by six GP practices in Wakefield, West – Church Street Surgery, Orchard Croft Medical Centre, Chapelthorpe Medical Centre, Prospect Surgery, Lupset Surgery and Middlestown Medical Centre.

As a first wave Prime Minister’s Challenge Fund pilot site covering 64,000 patients we have already set out to create an ambitious new model for primary care with our key partners and stakeholders listed below in order to create an ambitious new model for primary care:

• The 6 founding practices of West Wakefield Health and Wellbeing;

• NHS Wakefield CCG;

• Wakefield Council;

• Wakefield District Housing;

• South Partnership NHS Foundation Trust (SWYPFT);

• Wakefield HealthWatch;

• Mid Yorkshire Hospitals NHS Trust;

• Nova (VCS representative body);

• Yorkshire Ambulance Service (NHS 111 and 999 provider);

• Local Care Direct (GP out-of-hours provider).

Whilst our PMCF objectives remain clear, we have continued to learn from our evolving experience and developments in the wider local and national context which has enabled us to develop a more complete vision and greater set of ambitions that go beyond PMCF.

In recognition of this, West Wakefield has been selected as one of 29 Vanguard pilot sites across to develop one of the four models of care identified in Five Year Forward View. Known as the Multi-specialty Community Provider (MCP) model of care, our pilot programme will seek to blend primary care and specialist services in one organisation, create multi-disciplinary teams providing services in the community, and identify those patients in the community who will benefit most. The Network 6 bid was one of two proposed by the Health & Social Care community in Wakefield with NHS Wakefield CCG working in partnership with Wakefield Council and the Provider Alliance selected to develop a new model of care for Care Homes. It is a huge achievement for Wakefield to be selected to deliver 2 of just 29 Vanguard programmes being supported nationally and is recognition of the excellent work that has already taken place across the district to develop integrated care and maintain the well-being of local citizens by delivering co-ordinated care as close to home as possible.
