This is an appointment for a Locum Consultant Obstetrician and Gynaecologist at the United Lincolnshire Hospitals NHS Trust based at the Pilgrim Hospital, Boston. The successful candidate will be expected to work in Gynaecological oncology, in addition to general duties in Obstetrics and Gynaecology.

GENERAL INFORMATION

Lincolnshire is scenically and geographically varied. It includes the Lincoln escarpment on which the beautiful medieval Cathedral stands, the North Lincolnshire Wolds, a designated area of outstanding natural beauty, the extensive beaches of the coastal strip, and the vast farmlands in the south of the county.

Lincoln is a developing city with a recently established and successful University with which the Trust is developing research and teaching links. The city is known for the historic Cathedral and Lincoln Castle as well as the many social and leisure facilities available within this expanding city.

Boston is an attractive market town in South Lincolnshire with a growing population. It has a flourishing port on the River Witham dating back to the thirteenth century. Most of the town’s industry is linked to agriculture or the port.

Grantham is an historic market town with many attractions, including plentiful shops, restaurants and leisure facilities and is only just over an hour from London by train.

South Lincolnshire has retained the grammar school system, and has a reputation for providing excellent state school education. In addition, the cost of housing compares very favourably with many areas of the country.

THE HOSPITALS The United Lincolnshire Hospitals NHS Trust brought together Lincoln and Louth NHS Trust, Pilgrim Health NHS Trust and Grantham and District Hospital NHS Trust. It is responsible for a comprehensive range of health services to a local population of some 650,000 people.

COUNTY HOSPITAL, LINCOLN

Lincoln County Hospital is situated close to the city centre of Lincoln. It is a newly developed large and modern complex which caters for most of the major specialties and has 705 beds. There are 255 general medical and care of the elderly beds including coronary care and 6 renal beds. There is a large, well-staffed Accident and Emergency Department. There are at present 108 general surgical beds, 8 intensive care unit beds and 8 HDU beds, a 10 theatre suite and 3 orthopaedic wards. There is an Ophthalmic Unit, Adult Day Care Unit, an ENT department and a purpose built fully equipped department of Clinical Oncology.

PILGRIM HOSPITAL, BOSTON

Pilgrim Hospital is situated on the outskirts of Boston and is a modern District General Hospital, which was opened in 1977 with 750 beds. If offers a broad range of clinical and laboratory services for Boston and the surrounding area serving a population of approximately 280,000. It has close links with community hospitals in Skegness and Spalding.

The Hospital offers a comprehensive range of services including General Surgery, incorporating Vascular Surgery, Gastrointestinal Surgery and Breast Surgery, and is the main base for Breast Screening in South Lincolnshire. Other services on site include Obstetrics, Paediatrics, Ophthalmology and ENT services.

The hospital has a busy Accident and Emergency Department, a Clinical Decisions Unit, a combined ITU/HDU/CCU with up to12 beds. There is a local pathology service, which is likely to integrate with the Pathlinks consortium, which serves the rest of the county, in the near future.

The Radiology Department is well serviced with spiral CT and MR scanners. There is a high quality Medical Physics Unit.

The Hospital also incorporates the Bostonian, which is a private wing of the hospital, which offers a good range of private services and has 20 beds.

GRANTHAM AND DISTRICT HOSPITAL

Grantham and District Hospital is a small district general hospital with 194 beds, offering all major specialties and has close links with Boston, Lincoln and Newark hospitals as well as teaching centres in Nottingham and Leicester. Primarily it serves the people of Grantham, and Newark with a total catchment population of around 120,000.

Grantham provides Integrated Medicine, General Surgery, Orthopaedics, Anaesthetics and Accident and Emergency services. There is an ambulatory Paediatric service and Midwife Managed Maternity Unit. There is a fully equipped X-Ray Department with a modern CT scanner as well as a Pathology Department providing all pathological services on site. There is a Medical Physics Department which does all cardio-respiratory work as well as gamma camera work.

LOUTH COUNTY HOSPITAL

Louth County Hospital has approximately 120 beds consisting of Integrated Medicine, Surgical and Orthopaedic Wards. There are direct acute admissions only to the Medical Wards. The hospital has a High Dependency/Coronary Care Unit and a ward caring for severely mentally infirm patients. The hospital also has an Accident and Emergency Department, Endoscopy, Day Hospital and a busy Outpatient Department supported by X-Ray, Physiotherapy and Occupational Therapy Departments. There is an on-site education centre with lecture theatre, library and internet access.

JOHN COUPLAND HOSPITAL, GAINSBOROUGH

The John Coupland Hospital is in Gainsborough, approximately 18 miles from Lincoln. Scotter Ward is a rehabilitation/care of the older person ward. The hospital has an operating theatre, Outpatients Department and a Nurse Led Minor Injuries Unit.

JOHNSON HOSPITAL, SPALDING

This is a community hospital situated in the market town of Spalding, some 16 miles from Boston. There is an X-Ray Unit, Physiotherapy and Occupational Therapy Services, an active Outpatients Department and a Minor Injuries Unit.

SKEGNESS HOSPITAL

This community hospital has 15 GP beds and 15 beds managed by an Elderly Care Physician. There are Physiotherapy and Occupational Therapy Services on site and a busy Outpatients Department. The Accident and Emergency Department is supported by Associate Specialists 24 hours a day and is a very busy department, especially in the summer months.

DUTIES AND RESPONSIBILITIES

1 The Appointee will be expected, in conjunction with his / her colleagues, to provide a comprehensive service to the hospitals within Lincolnshire. They will be expected to maintain a high level of professionalism.

2 To provide input to the labour ward to ensure the department can achieve 40 hour labour ward cover for CNST. This involves taking part in a hot week rota on the labour Ward. Also to include comprehensive cover for the Colposcopy Service.

3 To provide on a reciprocal basis, emergency cover for Consultant colleagues during periods of absence and to take part in the Consultant rota for emergency duties.

4 To undertake such other duties as may be identified to meet the needs of the service, including Pan Trust working at peripheral sites.

5 To carry out teaching duties as required and take an active part in the Undergraduate and Postgraduate Medical Education programme and in the education of junior medical staff.

6 To have continuing responsibility for patients in his / her charge and for the proper functioning of the department.

7 To take part in departmental Audit programme.

8 To provide professional supervision and management of junior medical/dental and other staff.

9 To be involved in research where appropriate

10 To actively participate in the management of the services of the Directorate.

Labour Ward daytime cover is provided during a “hot week” session from 9:00 am to 17.00pm, Monday to Friday on a 1 in 8 week rolling rota. This amounts to 1.56 PA per week in the weekly job plan.

On-call Commitment

The on call rota is a 1 in 8 rolling rota with internal cover for periods of annual and study leave. The on call includes an evening shift on the Labour Ward from 17:00pm to 21.00 PM

The Weekend call is 1 in 8 and includes ward rounds from 9:00am to 12:00pm. The Weekend on call starts on Friday 9:000am and ends at 9:00am the following Monday.

It is anticipated that this timetable may be revised in due course

Teaching

It is expected that the successful appointee will provide professional supervision, teaching and management of the junior medical staff and also contribute to other educational activities that may be required.

There are close links between the Unit and the University of Nottingham and the University of Leicester. Regular groups of medical students come to Lincoln for training and the present Consultants are all Clinical Teachers to the University. It is anticipated the post holder will be likewise. There is a new fully equipped undergraduate education centre in the hospital.

The Trust has an excellent Post Graduate Medical Education Centre on site. It is equipped with state of the art audio-visual facilities, excellent library, meeting rooms as well as other resource rooms.

ST4/5 and career ST1/2 posts rotate to the hospital. Postgraduate trainees from University of Nottingham are posted for their training in Obstetrics and Gynaecology.

There are teleconferencing links with other Teaching Hospitals and other hospital sites within the Trust which facilitates regular meetings between all the sites.

Continuing Medical Education

The Trust is supportive of Consultant’s needs and requirements to meet the external CME targets.

Clinical Audit and Research

Clinical Audit and Research are actively encouraged within the Trust. The Trust has an active Research Unit and is actively involved in recruiting patients for multi-centre trials. Links are currently being developed with the University of Lincoln to improve research activities within the Trust. As part of Clinical Governance, audit is mandatory.

Teaching It is expected that the post holder will provide professional supervision, teaching and management of the junior medical staff and the post holder will also contribute to other educational activities that may be required.

The department has mortality and morbidity audits every month. In addition, process audits are held every two months and topic audits every six months. SHO and SPR are provided with specific topic audit to be completed during their period of employment. The Trust has a countywide process audit programme which is held on a rotational basis.

Appraisal, Revalidation & Mandatory Training: The post holder will be required to maintain their continuing professional development (CPD) to be able to successfully revalidate. It is a Trust requirement that the post holder has an annual appraisal and ensures that their mandatory training is kept up to date.

Health and Safety The United Lincolnshire Hospitals NHS Trust recognises its duties under the Health and Safety at Work Act 1974 to ensure, as far as is reasonably practicable, the health, safety and welfare at work of all its employees. In addition the business of the Trust shall be conducted so as to ensure that patient, their relatives, contractors, voluntary workers, visitors and members of the public having access to the Trust premises and facilities are not exposed to risks to their health and safety.

Main Conditions of Employment

The terms and conditions of employment will be determined by the United Lincolnshire Hospitals NHS Trust terms and conditions of employment. These are generally in line with the relevant provisions of the Terms and Conditions of Service of Hospital Medical and Dental Staff (England and Wales) and General Whitley Council Terms and Conditions of Service, as amended from time to time. Copies of these can be seen in the HR Department. This is a replacement post based at Pilgrim & Associated Hospitals the post will be fully based at Pilgrim. Occasionally the post-holder may need to travel to other sites to undertake ad-hoc duties, theatre cover or attend meetings which will always be mutually agreed in advance incl. travel arrangements. . In determining salary for the post, the Trust will pay no less favourable rates than those paid to NHS Consultant Medical and Dental Staff. If, in the future, the Trust establishes alternative negotiating arrangements you will be notified of those arrangements, and thereafter any agreements thus reached will form part of your Contract of Employment.

The post holder shall have medical clearance arranged by the Trust before taking up employment.

If the appointee elects to undertake a maximum part-time contract he/she will be required to devote substantially the whole of his/her time to the duties of the post and give them priority at all times.

Any consultant who is unable for personal reasons to work full time will be eligible to be considered for the post. If such a person is appointed, modification of the job content will be discussed on a personal basis with United Lincolnshire Hospitals NHS Trust in consultation with Consultant colleagues.

The successful candidate will work at their base Hospital, Pilgrim and Associated Hospitals. As a consequence he/she is expected to be aware of local policies and procedures and take note of the standing orders and financial instructions of the Trust. In particular, where the Consultant manages employees of the Trust he/she will be expected to observe the local employment and personal policies and procedures.

This job description will form a composite part of a Job plan which will include the post-holders main duties and responsibilities. The Job plan will be reviewed within 6 months from commencement of the post and subsequently agreed on an annual basis between the Clinical Director and the post holder. Annual job plan reviews will also be supported by the appraisal system which reviews a Consultant’s work and performance and identifies development needs as subsequently reflected in a personal development plan.

RESIDENCE The person appointed will be required to maintain his/her private residence in contact with the public telephone service and reside within reasonable access bearing in mind the duties of the post. The Trust requires the successful candidate to live within a distance of 10 miles by road of their main site.

OFFICE ACCOMMODATION Office accommodation will be available with Admin and Clerical Support from a Personal Secretary.

PROFESSIONAL The successful candidate must be on the GMC’s register.

The post holder will be required to maintain and expand his/her clinical knowledge and skills and attend educational meetings to maintain CME.

The Trust has established a policy of Performance Development Review. The post holder will be required to participate actively in the Trust education programme and to attend post- graduate and departmental meetings.

All consultants are expected to work with their clinical and management colleagues to maintain and develop the Trust’s services.
