Often people ask me about how I reacted when I was told of Emma’s diagnosis. They expect me to say devastated, sad, or overwhelmed. They are surprised and confused when I answer “relieved”. What they don’t realize is that for four months, I questioned my abilities as a mother. To set the scene, it was the end of March, in Alaska, and my husband had been deployed when Emma was 10 days old.

In the beginning, Emma was a great sleeper, in fact, too good. She was only 5.5 pounds so the doctor wanted me to feed her every two hours (from the start of the process) around the clock. She had a lot of difficulty, so I had to try and breastfeed her, when that didn’t work I was to pump and feed her a bottle. If she didn’t take to the bottle, I had to wrap a feeding tube around my finger and “baby bird” feed her. Sometimes the hardest part of this whole process was just getting her awake enough to get it started. I was exhausted, frustrated, and upset with myself. I was a nurse for heaven’s sake, how could I not get this?! Women in third world countries had their babies in the middle of fields and could breastfeed while working the fields, yet me, a college educated ICU RN could not provide her baby one of her basic needs. Finally, I marched into the pediatrician’s office and refused to leave until a doctor saw Emma. The doctor examined Emma and took some blood. The day before Cedric was to return from deployment, I received her diagnosis. It was not that I was a bad Mom but that Emma had Cri du Chat Syndrome. I now knew what I was dealing with/fighting against. My “enemy” had a name. I could research and get to know everything I needed to develop my battle plan. I jumped on the computer and googled CdCs and the first information that popped up was “THE article”. Most of us know which one, the one that told us are kids would never walk, talk, or be interactive and that they would die before their first birthday.

Luckily, Emma’s geneticist gave me contact information to get a hold of the 5P- Society, and more specifically, Laura. Laura sent me copies of the research she had as well as contact information with a local (well as local as you can get in Alaska) family. Laura also gave me information on the yearly conference. While the first conference was a bit of a blur (it was July in St. Louis, translate HOT and HUMID, and I was about 8.5 months pregnant with my third child). I learned so much from the speakers but it was the connection with the families that had the biggest impact. It was the first time since Emma was born that I had truly relaxed. I didn’t have to explain Emma; everyone got us, and welcomed us. I received so much information from parents whose children were older. I went home with so much information and hope.

Knowledge and support are critical to the health of our children and our own mental health. Without my CdCs family, we would never be in such a good place. I could never thank the families that have given me their knowledge, their love, and their support. I figured to honor their gift to me; I would pay it forward, which eventually led to me being elected as a Board member and currently, the President. Laura, the Board of Directors, and the Professional Advisory Board are currently working on the final touches of the first questionnaire. We will be releasing the questionnaires one at a time. After we gather all the information, we will be releasing the findings as the Caregiver’s Guide. We are hoping this will be a comprehensive resource for genetics, medical issues, education, and therapies, as a start. When we release the first questionnaire, we are asking everyone to please take the time to fill it out. Our goal is to create a resource guide for families with the largest CdCs family participation out there. The key to the Caregiver’s Guide success is having data from a very large study population to capture all the variances and nuisances of 5P-. We are asking all our families to pay it forward. Be sure to watch our website and Facebook group for the link to the questionnaire. It will be wonderful when families receiving the cri du chat diagnosis for the first time can a comprehensive guide rather than “THE article”.

Conference Presentation Recap BY Michelle Myatt

At the California conference, I was able to attend a breakout session on “Head and Neck Manifestations of Cri-du- Chat syndrome.” Dr. Douglas R. Sidell, an ENT from Stanford University, was a wonderful speaker who really captivated the audience. He started with a brief anatomy lesson utilizing multiple slides. He was able to easily educate all of those in the room about issues our children may face.

One of the abnormalities he spoke about the most was a laryngeal cleft. He discussed how these hidden clefts may actually be common in children with 5p- and they can cause further complications with feeding and swallowing. I remem- ber thinking right then that I was going to take this information back to our doctor. It seems no matter how many confer- ences we attend, I always learn something new. I took in the conference booklet that had all of the pictures in it, as well as Dr. Sidell’s credentials, and explained my concern to our ENT. He listened attentively and looked at what I had brought in.

At my request, the doctor scheduled us for a swallow study. During this procedure, Delaney ate her typical food with an extra additive so that we could see what happened when she swallowed under x-ray. She did great with thick bo- luses of food, but aspirated liquid each time. This explained why she was choking so often when she was taking drinks. Although frustrating, I wondered if Delaney really did have a laryngeal cleft. Our ENT called later that day and asked us to come back to his office for another visit.

During this visit, he numbed her nose and throat and used a scope to look at everything. He said he couldn’t tell if there was a cleft, as he would have to look while she is under anesthesia. But he was able to tell me that one of her vocal cords was not moving. This was a huge disappoint- ment. Unfortunately, this is not a foreign concept to us. Delaney was born with bilateral vocal cord paraly- sis. Vocal cords are super important, as they open up when you breathe and close off your trachea when you swallow. This is supposed to protect food from going into your lungs. This was one of the reasons Delaney needed a tracheostomy for her first five years of life.

Delaney is scheduled for a sedated scope in a few weeks. Although my husband and I are very nerv- ous about what the doctor will find, we are also so thankful that we initiated this. I would never have thought to ask for the tests if I had not been to the con- ference in California and listened to Dr. Sidell speak. It’s always nice to make connections with other fami- lies, but this year the breakout session was invaluable to us. We hope and pray that this set back is temporary, but regardless feel so blessed to have known what to ask.

Caregiver’s guide for families

For the past year I have been hinting at the desire to create a Caregiver’s Guide for Families who have a loved one with Cri du Chat Syndrome. Over a year of planning and we are finally ready to begin accessing information from YOU to help prepare the guide. The goal of the guide will be to provide newly diagnosed families with practical information about raising a child with the syndrome.

YOUR participation is essential in providing up-to-date and real life information. To do this we will be providing you with several questionnaires to fill out about your child or adult with Cri du Chat.

 First questionnaire - Birth & Family History

 2nd questionnaire - Medical History

 3rd questionnaire: Educational History

 4th questionnaire: Therapies and Alternative Medications

 5th questionnaire: For adults only with Cri du Chat to discuss their needs and concerns

The first questionnaire link is https://form.jotform.com/62796530604964. The questionnaire shouldn’t take too long to fill out. Some of you may have filled out something similar before, but in order to collect as much information as possible, we are asking for you to provide us with the information again. One of the ESSENTIAL questions that we will need you to answer on the first ques- tionnaire is knowing your child’s breakpoint. Before you begin to answer the questions, please make sure you have this information. If you do not know it or can’t remember, please try to get it from your physician or geneticist. If you have paperwork, but are hav- ing a difficult time deciphering the information, please send a copy to me at [email protected] and I can help you figure it out.

Approximately two weeks after you submit the first questionnaire, you will be receiving the 2nd questionnaire. The 2nd question- naire will launch and be sent out to those who have completed the first questionnaire approximately 2 weeks later. The remaining questionnaires will also be sent out in 2-3 week intervals and will be sent out after each one is completed. If all goes to plan, all ques- tionnaires should be out and hopefully back in by mid year 2017.

After analyzing and compiling the information, the guide will be completed with the help of some parents who are also writers.

Please note, this is not a research project, there is no IRB approval. All questionnaires are being designed by a small committee, and approved by both the Professional Advisory Board and the Board of Directors. A fundraising project will be launched to help fund the final product.

Because the syndrome is rare, global responses are welcomed. You will be encouraged to participate along the way and will be the first to know of its completion. We hope to have it done by the end of 2017.

CBD Oil—an Alternative treatment By Rebekah Rocha

I am the mother of Gigi, who has CdCS and is 8 years old. As many of you have experienced aggressive behaviors with our kids, we had as well. Gigi started taking Concerta around 5 years old for hyperactivity. When she was seven, her aggressive behaviors wors- ened at home. Living in California, I wondered if Medical Marijuana would help her. I held out on that thought for sometime not acting on it, thinking it was too risky and unnecessary to pursue. However, her behaviors became so disruptive that I finally decided I had to look into it. I spoke with my daughter’s pediatrician at Kaiser, who said it was worth looking into. He told me that it would do no harm to try. His biggest warning was that it might not do anything, and to prepare myself for disappointment.

Again, living in California, getting permission for Medical Marijuana was a relatively easy thing to accomplish (and even easier now with the recent passage of Prop 64, which legalizes the use of marijuana in California). After a quick Google Search for a Medical Ma- rijuana Doctor in my area, I found Medi-Cann. They explained the process to me. I had to first get a letter from her Primary Care Physician stating that he was aware/supportive of the situation. I scheduled an appointment at Medi-Cann with Gigi and the Doctor recommended a Cannabinoid (CBD) Oil (after she about tore his office apart!). Cannabinoid Oil is extremely low in THC, which is the agent that makes people “high”. According to LeafScience.com, unlike THC, CBD does not cause a high. While this makes CBD a poor choice for recreational users, it gives the chemical a significant advantage as a medicine, since health professionals prefer treatments with minimal side effects. As it was explained to me, I could administer this tincture to Gigi and she would not get “high”, which is the first thing people think of when they hear Medical Marijuana. You may have heard of Charlotte’s Web, the popular oil/tincture from Colorado that is curing seizures in children. This is also a Cannabinoid Oil.

I was administered a Caregiver Medical Marijuana “card” (it was actually a letter) which would give me permission to pur- chase Medical Marijuana for Gigi. This document is what was (perhaps no longer with the passage of Prop 64) needed to enter the Marijuana Dispensary and purchase the product.

Trust me, it was a very strange experience going to the dispensary to purchase the CBD oil! I am not a marijuana user and I felt very out of place. I had called them before to ask if they had a product that would be helpful for Autistic like behaviors, and they said that they did have several clients who used their product. I felt excited and hopeful that this would help Gigi.

The warehouse reeked of marijuana, and there were people from all walks of life there purchasing different things. I was in awe at the “industry”, and how many different products were available. The staff at the dispensary was very helpful, and recommended a tincture (liquid) called Jayden’s Juice, which is a CBD oil. This tincture was made by a father whose son Jayden had a seizure disorder. Jayden’s Juice has made him seizure free. The results were im- mediate. She sat at the table and ate her entire dinner, using a fork the whole time (she had not done this before, was usually up and down from the table, getting frustrated with the fork after a few tries). She had no episodes of head banging that night. She let my husband change her pajamas (she usually fought him), and allowed us to brush her teeth easily. She went to sleep immediately.

Eventually, we decided to administer the tincture only when we thought she needed it. For exam- ple, if she was having a rough day, or we were going to a social event, etc. There is different infor- mation as to how often the medicine should be administered.

After a year of giving it to her off and on, we noticed that it didn’t really help as much as we had wanted it to. She was also older and now more aggressive. I spent a long time researching online and getting in touch with different individuals on Facebook who are using Medical Marijuana with their Autistic children. I reached out to people on their Facebook page and connected with individuals who were able to point me in the right di- rection in terms of what products to consider.

I was advised to continue with a CBD tincture, but one that had more THC in it. Gigi now uses a different combination of Jayden’s Juice as well as a 1:1 CBD/THC tincture. Through this process, I met a mom who pointed me to an organization called Myriam’s Hope: www.myriamshope.org.

Apparently you can call them and they will help you with questions about dosage. I have learned that no one really has the answers, you have to try different products and see what works. People I have met online have been very helpful, and I am happy to answer any questions as well.

I have also shared our story on the 5P Minus Facebook page. I know one mom in California (January Jimmy Klein) who immediately got her daughter Charlotte’s Web oil, and the results were amazing. She’s shared her videos on the 5P Face- book page.

Right now, we use these tinctures as needed and they continue to help calm her down and lessen aggression. She continues to use Concerta as well as Tenex, which also help. We have yet to give her the tinctures on a regular basis, but are consid- ering it as she is having more difficulty at school. If we did this, it would probably cost us about $1,000/month, as Medical Marijuana is not approved by the FDA and is not covered by medical insurance.

We do have another friend whose son has a seizure disorder and uses a CBD oil that has NO THC in it. This means it is totally “legal” in every state and it can also be ordered online. The Autism folks I have talked with seem to be in agreement that our kids do need a little of the THC, but CBD oil is worth a try if you live in a state where Medical Marijuana is not legal.

The journey continues. It is my hope that the FDA will allow for research on Medical Marijuana so that it may be a true aid to our children and their caregivers. Feel free to contact Rebekah if you have any questions at [email protected].
