Arkansas e-Link, through the UAMS Center for Distance Health (CDH). The CDH serves the many rural areas of the state through telemedicine, helping to reduce health disparities.

Description: Telemedicine allows patients in remote locations to access medical expertise quickly, efficiently, and without travel through real-time video technology. Telemedicine is being used in Arkansas to connect remote medical specialists in urban centers with patients in outlying and rural locations in Arkansas that do not have easy access to such specialty expertise. Arkansas boasts one of the largest and most successful statewide telemedicine networks in the nation, Arkansas e-Link. The University of Arkansas for Medical Sciences (UAMS) created and manages this network, the infrastructure by which all telemedicine is safely conducted by Arkansas’ healthcare providers and educators. To establish e-Link, UAMS aligned over 400 healthcare and higher education partners in the state, including health departments, community health centers, hospitals, two- and four-year colleges, and a multitude of others that deliver clinical care and medical education using real-time video technology. A unique aspect of the statewide network is its cross-institutional nature. In fact, many hospitals, clinics, and schools under varying ownerships operate under the same network infrastructure, making easy, HIPAA-compliant communication among all participating sites to help facilitate partnerships, patient transfers, provider education, and other activities that help each member mutually benefit from the involvement of diverse partners. The network has become an essential utility among its members, who are empowered to use this secure technology in their everyday patient and student encounters. Telemedicine services that have been offered through e-Link include telemental health consultations, tele-colposcopy screenings, emergency neurology consultations, high-risk pregnancy care, and much more. In 2018, Arkansas e-Link managed nearly 172 million hours of interactive video exchanged over the network. Arkansas e-Link also operates the statewide Arkansas Trauma Image Repository that transmitted over 4.5 million patients scans and other medical images in 2017 alone. Additionally, the network transmits numerous live, educational events for healthcare providers, nurses, and professionals, including virtual grand rounds, nurse case studies, and other multidisciplinary teleconferences that promote continuing learning and colleague collaboration. UAMS also helps each qualifying site receive FCC-discounted broadband connectivity, achieving a cost savings of $8.319 million in 2018-2019. This presentation will discuss the many clinical and educational programs being offered over Arkansas e-Link and how presentation participants may join these efforts or create new programs that can advance their public health activities serving the patients, students, and communities in Arkansas.

CONTRACTS

ANGELS: Contract with Arkansas Medicaid $22, 676,083 (recurring amount since 2003) Curtis Lowery serves a Medical Director and Founder of ANGELS (Antenatal and Neonatal Guidelines, Education, and Learning System), a program to educate and support health care providers who deliver care to high-risk obstetric patients. The program focuses on improving pregnancy outcomes and thus reducing the amount of care needed for infants and children with disabilities. Arkansas Medicaid administers this

$22,778,964 contract and the University of Arkansas for Medical Sciences (UAMS) provides service. This contract pays for adequate ancillary, professional, and clerical staff to provide the comprehensive, statewide service education, telemedicine and evaluation service. Funds are also being used to up-grade and expand the telemedicine network into the labor and delivery units of rural hospitals. Role: Medical Director, Founder Arkansas SAVES: Contract with Arkansas Medicaid $7,964,682 (recurring amount since 2008) Curtis Lowery serves as the Director of Arkansas SAVES (Stroke Assistance through Virtual Emergency Support), which presents an innovative solution to a statewide problem. Without the specialty support of a neurologist, many rural emergency rooms treating the potential stroke victim are reluctant to diagnose stroke and administer clot-busting medication. Arkansas SAVES is a vehicle by which the needed stroke knowledge and support is delivered directly in rural emergency rooms. This pilot program encompasses telemedicine participation at three rural hospitals consulting a large number of stroke patients, lacking ready access to a neurologist, and equipped with telemedicine technology, including T1 lines and Tandberg video units. Fashioned after a hub-and-spoke model, this program allows multiple sites to provide expert care throughout Arkansas. MEDICAL TECHNOLOGY COMPANIES

Angel Eye is a web-camera system installed in Neonatal Intensive Care Units that allows mothers to view their hospitalized infants. Part of a comprehensive web-based telemedicine program, Angel Eye enables a mother to be “virtually” present with her baby in the unit, with the goal of reducing maternal anxiety and promoting bonding that protects against the long-term risk of childhood abuse.

AirToco is a disposable air-filled plastic hockey puck-sized device that records uterine contractions externally on the mother’s abdomen, which replaces the costly and non-disposable guard ring created in the 1950s. The design of AirToco resulted in setting up a licensing and manufacturing agreement with Clinical Innovation (CI) of Utah, one of the leaders in the market for fetal monitoring devices in Labor and Delivery. This device has international patents, and we are awaiting US patent approval.
