1 Administration 1.1 Is there appropriate signage on entrances to labs and storage room/vessels (refer to Unit’s group register): a) Biological symbol; b) OGTR PC1/PC2 sign; c) PC1/PC2 signs for non- GMO labs; d) Access restrictions; e) QAP signage for use of material. 1.2 Are there copies of: a) SWPs for work involving biological b) RAs for work involving biological hazards c) AQIS permits for imported biological; d) Licences for dealing with NLRD GMOs; e) Risk group register for micro- and diagnostic specimens f) Training records of lab personnel? 1.3 Are all cultures or biohazardous materials correctly labelled? 1.4 Is the Lab Manual available in the lab 2 Laboratory Procedures 2.1 Is the correct PPCE clothing being worn while working with biological hazards? 2.2 Are lab coats/gowns disinfected and washed regularly? 2.3 Is there a supply of hospital grade antiseptic for washing hands in the lab? 2.4 Does all staff remove PPCE and then their hands before leaving the lab?

2.5 Is there a documented procedure for making up disinfectants? 2.6 Is diluted bleach stored away from heat and being kept in lightproof containers with preparation date displayed? 2.7 Are doors being kept closed while carrying out work in PC2 labs? 2.8 Are sharps being disposed of in approved sharps containers? 2.9 Is contaminated/infectious being placed in relevant bin (Biohazard/autoclave) lined with an appropriate bag (Biohazard/autoclave) then disposed of/autoclaved appropriate? 2.10 Are personnel aware of the correct procedures for dealing with a biological spill? 2.11 Is there an appropriate biological spill kit available? 2.12 Are bio-safety cabinets used when there is a likelihood or production? 2.13 Are bio-safety cabinets certified annually? 3 Physical Requirements 3.1 Are laboratory coat hooks: a) Provided adjacent to the laboratory access (within the laboratory) b) Equal to or greater than the number of persons working in the laboratory? 3.2 Are protective covers provided for all keyboards on workbenches?
