As a full time Consultant Anaesthetist you will work closely with our existing 2 Consultant Anaesthetists as well as the Medical and Surgical teams of the Hospital, to deliver a first rate service to the population of and beyond.

Situated at the foot of Ben Nevis, Fort William is often referred to as the outdoor capital of the UK. The Belford Hospital has an established reputation in the management of trauma, particularly from mountaineering accidents, skiing and mountain biking and also undertakes a variety of appropriate elective surgery (including colorectal). The acute specialties of Medicine and Surgery are supported by modern, comprehensive Anaesthetic services. All specialties provide 24/7 (non resident) consultant cover (including cover for A&E), this is supported by a well-equipped HDU, comprehensive laboratory and radiology services (including on-site ultrasound and CT). The on call frequency is being offset by a lighter workload during the working week. We manage our emergency cases on site, care is consultant led and delivered. Resuscitation and stabilization of the critically ill or severely injured to allow transfer to definitive care, regional intensive care unit or tertiary centres forms a major part of our workload. Our clinical services are highly valued by the local population and also by the large tourist population. This post is offered on a ten programmed activity basis as outlined in the proposed job plan. The frequency of on-call is 1:3 for weekdays (Mon-Thu) and 1:4 for weekends (Fri-Sun). The on-call frequency attracts a supplement of 8% in line with the Terms and Conditions of Service. The successful applicant will be required to have wide knowledge and experience in anaesthesia and critical care. Fort William Information

The area of Fort William and Lochaber is a huge rural landscape and covers more than 2000 square miles of the West Highlands of Scotland. Fort William is the capital town by the foot of Ben Nevis, the highest mountain in the UK. The Lochaber population of about 20,000 often expands to several times this number in the peak summer periods with visitors from all over the world coming to experience "The Highlands".

Fort William and Lochaber is acknowledged as the Outdoor Capital of the UK due to the huge range of outdoor activities and sports one can do in the area. Activities ranging from scuba diving, sailing and canoeing, fly fishing and sea fishing to walking, cycling, mountain biking, rock climbing, ice climbing, skiing and snowboarding, to hang-gliding and flying. WORKING AT THE BELFORD HOSPITAL, FORT WILLIAM

THE BELFORD HOSPITAL

This Hospital is one of six rural general hospitals in Scotland; the others being at Oban, Wick, Shetland, Stornoway and Orkney. These hospitals are characterised by isolation from District General Hospitals and other colleagues, with the necessity to provide a wide range of Clinical services on-site. NHS Highland provides care for a land area the size of Wales, and the Belford Hospital is one of three providing acute services for the Board. The Board’s area extends from General Hospital at Wick in the north, to Lorne and the Islands Hospital in the south. In the centre there is , which functions as a typical District General Hospital, with all specialties on-site, apart from neurosurgery and cardiothoracic.

Due to the remote and rural location of the hospital, a wide range of general surgery is practiced including minimal access surgery, basic orthopaedics and trauma. The A&E Department is the busiest in the Highlands and Islands outwith Raigmore Hospital. ATLS training is desirable. The successful candidate would be expected to achieve ATLS, as soon as possible on taking up the appointment, if they do not already hold it.

GENERAL INFORMATION ABOUT THE DEPARTMENT OF ANAESTHETICS

Work of the Anaesthetics Department

The Department consists of three whole time consultants, with no junior staff, working a 1:3 on call with prospective cover. Services provided include a very broad range of general surgery with occasional urology lists. There are three all day lists on the Tuesday, Wednesday and Thursday which cover a broad range of general surgery including colo-rectal with regular endoscopy lists on Monday and Friday. The Wednesday lists are utilised twice a month by visiting surgeons from Raigmore Hospital. We are part of the hospital resuscitation team and provide a resuscitation, stabilisation and transfer service, usually either to Glasgow with head injuries, or Raigmore Hospital, , with ITU cases although this can be to any ITU with available beds in Scotland.

Belford Hospital consists of a 17 bedded Combined Assessment Unit which receives acute medical and surgical cases, a two bedded high dependency unit providing a facility for short-term ventilation, which can extend to overnight or longer depending on ITU bed availability in other facilities. There is also a regular need to transfer patients via EMRS or the Paediatric Retrieval team. There is a fully functional Pre-assessment Unit and a busy day surgery unit which undertakes 80% of the elective surgical workload. There is also a 15 bedded acute mixed/rehabilitation ward, which manages most long term conditions. There is a small community led maternity unit which can require anaesthetic emergency support for neonates. The Belford has an excellent on-site laboratory service providing basic biochemistry, haematology and cross-match services 24/7. There is also on site radiology with ultrasound scanning and CT facility.

The 1:3 on call (non- resident), although onerous is offset by the light elective workload. Almost all the surgery out of hours is carried out by consultant surgeons with consultant anaesthetists and it is therefore unusual to be working in the early hours of the morning unless essential. The rota is designed to ensure as far as possible that an elective list is not anaesthetised by the person on call the day before. Over the summer holidays there is a slight drop in the elective lists due to annual leave.

There is also a desire to provide a chronic pain clinic and any candidate who is interested will be actively encouraged to pursue an interest in chronic pain. The anaesthetists are part of the acute pain team and provide a thoracic epidural service for the major abdominal surgery including epidural opiates.

Despite its rural setting the department has to deal with a wide range of emergencies from the accident and emergency unit with appropriate resuscitation, stabilisation and transfer, this can include medical patients as well as surgical and good teamwork with our medical and surgical consultant colleagues is therefore essential.

The small midwife run maternity unit means that on very rare occasions there is a responsibility to help with neo natal resuscitation. There have only been a handful of caesarean sections in the last 20 years.

In view of the unique nature of the post with very wide ranging anaesthetic responsibilities there will be added provision for updating your anaesthetic skills on a regular basis which will be in addition to the normal 10 days study leave through integration with Raigmore Hospital or elsewhere as required.

Duties Medical Negligence NHS Highland takes responsibility for expenses and damages arising from medical negligence where they, as the employer, are vicariously liable for the acts and omissions of their medical and dental staff. However, the appointee is strongly advised to maintain separate medical defence or insurance cover for all work which does not fall within the scope of the Board's indemnity scheme, details of which are given in NHS Circular 1989(PCS) 32. Registration with Applicants must have full registration with the General Medical General Medical Council, a licence to practise and be eligible for inclusion on the Council GMC Specialist Register. Those trained in the UK should have evidence of higher Specialist Training leading to a CCT. Non UK applicants must demonstrate equivalent training.

Medical Fitness All prospective members of staff are asked to submit a confidential health questionnaire to the Occupational Health Service. On the basis of this, they may be passed fit, or an appointment for further information or screening may be required.
