Community Integration for ALTSA and DDA Funded Clients in Adult Family Homes
PURPOSE: / To provide information and direction to field staff on Implementation of Collective Bargaining Agreement Article 7.7, Community Integration Payment, in Adult Family Homes
BACKGROUND: / Under federal rules, residents in Adult Family Homes (AFH) must not be isolated from their community or from other people who do not live in the AFH.
WHAT’S NEW, CHANGED, OR CLARIFIED: / The State of Washington (State) and the Adult Family Home Council (AFHC) have agreed to Articles 7.7 and 7.8 of the 2017-2019 Collective Bargaining Agreement (CBA). This change is effective July 1, 2017, dependent upon adoption of the CBA in the 2017-2019 biennial budget.
Community Integration
Effective July 1, 2017, Adult Family Home providers who have a contract with the State to provide support to residents with an assessed need for support to access and participate in the community will receive an adjusted daily rate to provide four (4) hours per month.
The rate adjustment for community integration may include the following supports provided by the AFH:
·  Assisting the resident to select what they want to do and where they want to go in the community
·  Assisting the resident to plan how they will get to the activity
·  Assisting the resident before the activity to problem solve any issues that may come up
·  Assisting the resident to become members of community organizations that interest them
·  Assisting residents to identify other people in the community who can accompany them at the event or provide support with transportation
·  Arranging for or providing transportation to and/or from the activity
·  Accompanying the resident during the event to provide personal care assistance
·  Looking for additional opportunities that the resident may want to participate in
Community Integration Mileage Reimbursement
Adult Family Home providers who transport clients to access and participate in the community as authorized in the client’s CARE assessment will be reimbursed per mile driven for up to 100 miles per month, per participating client based on the standard IRS mileage rate.
ACTION: / New CI screens will be available in CARE on July 1, 2017. At the next assessment or when requested by the resident, Case Managers will assess the AFH resident’s interest in and need for support to access and participate in their local community.

Notes:
·  When requested by the resident, case managers can update assessments to include CI rate adjustments and CI mileage reimbursement using an interim assessment prior to the next full assessment.
·  AFH Providers must include documentation of the resident’s choice to participate in community integration activities in the resident’s Negotiated Care Plan (NCP) when the need is also identified in the client’s CARE assessment.
·  CI rate adjustments and CI mileage reimbursement is not available for PACE clients.
·  When CI is assessed as a need in CARE, the CI adjusted rate will be automatically calculated when the authorization is created in CARE. Do not attempt to add rate manually with the exception of ECS clients.
·  Please see the AFH Add-On for ECS Service Code Data Sheet for instructions on adjusting the daily rate for Expanded Community Services residents. The ECS rate table will be adjusted effective July 1, 2017 to assist with authorizations.
·  Providers are expected to maintain documentation on the provision of assistance with community integration and this documentation must be made available to case managers upon request.
