BANQUET TICKETS: Each award recipient will receive one (1) ticket to the awards banquet
(to be picked up at the banquet door that evening). Additional tickets
may be purchased at $60.00 each and should be ordered by June 5, 2015

INTRODUCTION RESUME:

Please fill out the attached form, which will be used by the presenter
when introducing you. Feel free to use additional paper to adequately
introduce you. Hall-of-Fame and Special Award Recipients will be
asked to say a few words.

AWARD PICTURES:

A photographer will be at the banquet for pictures. Group pictures will

be taken at the conclusion of the banquet. Individual pictures will be
taken during each presentation. All recipients will be in one group picture.

ROOM ACCOMMODATIONS: The Crowne Plaza Hotel will be the host for the CCA Banquet by
June 4th to reserve this special group rate. (1-888-233-9527).

For those of you flying into San Diego, there is a shuttle service to and
from the hotel, however the service is not free! (approximately $15). Parking is $6 per day special room rate for Cal Coaches.
